  • The application of knowledge translation in health promotion

    Knowledge translation (KT) provides a paradigm to bridge the gap between knowledge and practice, which has critical instructive significance for health promotion. This article expounds on the connotation of KT by comparing it with similar terms. Next, it introduces three kinds of common KT theoretical models, including process models, determinant frameworks, and evaluation frameworks. Finally, its application and experiences in health promotion are summarized to provide references for the ongoing health promotion in China.

  • Factors influencing pulmonary rehabilitation outcomes in patients with COPD:a systematic review based on the new CFIR framework

    ObjectiveTo systematically evaluate the factors affecting the curative effect of pulmonary rehabilitation in COPD patients,and to provide theoretical basis for the implementation of rehabilitation. MethodsLiterature on the influencing factors of pulmonary rehabilitation efficacy in COPD patients was systematically searched in PubMed,Web of Science,Medline,ProQuest,Embase,CNKI and Wanfang databases from August 30,2023. According to the inclusion and exclusion criteria of the literature,the factors influencing the pulmonary rehabilitation effect of COPD patients were analyzed based on the 5 dimensions of the new implementation comprehensive framework. ResultsA total of 2620 COPD patients with 18 literatures were included in this study. Based on the new CFIR framework,11 promoting factors and 10 hindering factors of pulmonary rehabilitation efficacy were extracted and coded into 5 fields:innovation field,external factor field,internal factor field,individual characteristics field and implementation process field. ConclusionBy applying the new implementation comprehensive framework to evaluate the factors affecting the pulmonary rehabilitation effect of COPD patients,it provides an important reference for the development of rehabilitation strategies for COPD patients. It is suggested to adopt the comprehensive rehabilitation strategy of interdisciplinary,wide field and multi-team in order to promote the best benefit of both individual and social medical system in the pulmonary rehabilitation process of COPD patients.

  • Impact of World Health Organization multimodal hand hygiene improvement strategy on hand hygiene compliance among acupuncturists

    Objective To understand the effect of World Health Organization(WHO) multimodal hand hygiene improvement strategy on hand hygiene compliance among acupuncturists. Methods All the acupuncturists in departments (Department of Acupuncture, Department of Encephalopathy, Department of Orthopedics and Traumatology) with acupuncture programs in Xi’an Hospital of TCM were chosen in this study between September 2015 and August 2016. Based on the WHO multimodal hand hygiene improvement strategy, comprehensive measures were regulated among acupuncturists. Hand hygiene compliance and accuracy, and hand hygiene knowledge score were compared before and after the strategy intervention. Then, the effects of key strategies were evaluated. Results Overall hand hygiene compliance rate, accuracy and knowledge scores increased from 51.07%, 19.86% and 81.90±2.86 before intervention to 72.34%, 51.70%, and 98.62±2.92 after intervention (P<0.05). Hand hygiene compliance rates also increased in various occasions such as before contacting the patient, after contacting the patient, before acupuncture treatment, and before acupuncture needle manipulation (P<0.05). Conclusion Hand hygiene compliance in acupuncturists can be significantly improved by the implementation of WHO multimodal hand hygiene improvement strategy.

  • Deep residual convolutional neural network for recognition of electrocardiogram signal arrhythmias

    Electrocardiogram (ECG) signals are easily disturbed by internal and external noise, and its morphological characteristics show significant variations for different patients. Even for the same patient, its characteristics are variable under different temporal and physical conditions. Therefore, ECG signal detection and recognition for the heart disease real-time monitoring and diagnosis are still difficult. Based on this, a wavelet self-adaptive threshold denoising combined with deep residual convolutional neural network algorithm was proposed for multiclass arrhythmias recognition. ECG signal filtering was implemented using wavelet adaptive threshold technology. A 20-layer convolutional neural network (CNN) containing multiple residual blocks, namely deep residual convolutional neural network (DR-CNN), was designed for recognition of five types of arrhythmia signals. The DR-CNN constructed by residual block local neural network units alleviated the difficulty of deep network convergence, the difficulty in tuning and so on. It also overcame the degradation problem of the traditional CNN when the network depth was increasing. Furthermore, the batch normalization of each convolution layer improved its convergence. Following the recommendations of the Association for the Advancements of Medical Instrumentation (AAMI), experimental results based on 94 091 2-lead heart beats from the MIT-BIH arrhythmia benchmark database demonstrated that our proposed method achieved the average detection accuracy of 99.034 9%, 99.498 0% and 99.334 7% for multiclass classification, ventricular ectopic beat (Veb) and supra-Veb (Sveb) recognition, respectively. Using the same platform and database, experimental results showed that under the comparable network complexity, our proposed method significantly improved the recognition accuracy, sensitivity and specificity compared to the traditional deep learning networks, such as deep Multilayer Perceptron (MLP), CNN, etc. The DR-CNN algorithm improves the accuracy of the arrhythmia intelligent diagnosis. If it is combined with wearable equipment, internet of things and wireless communication technology, the prevention, monitoring and diagnosis of heart disease can be extended to out-of-hospital scenarios, such as families and nursing homes. Therefore, it will improve the cure rate, and effectively save the medical resources.

  • Scoping review and thematic analysis of factors and strategies for the sustainability of evidence-based health promotion program

    ObjectiveTo identify factors and strategies for the sustainability of evidence-based health promotion programs. MethodsThe articles related to program sustainability factors and strategies were retrieved from the CNKI, WanFang Data, VIP, PubMed, and Web of Science database. The search period for English literature was from June 19, 2018, to December 3, 2024, while no start date was set for Chinese literature, the end date was the same. Three researchers independently screened studies based on predefined inclusion and exclusion criteria and subsequently extracted the basic characteristics from the included studies. Thematic analysis was conducted to identify the sustainability factors and strategies from the included studies. ResultsA total of 65 articles were included in the scoping review. Through thematic analysis, 23 factors were identified and classified into six categories: program characteristics, organization characteristics, human resources, material resources, community resources and policy resources, along with 44 strategies grouped into six categories: programming strategies, collaborative strategies, capacity-building strategies, resource mobilization strategies, communication and dissemination strategies, and evaluative strategies. ConclusionThe sustainability factors and strategies identified in this study exhibit conceptual and structural alignment with existing frameworks and strategy sets in implementation science. Concurrently, by incorporating a full life-cycle perspective, this study delineates context-specific factors and strategies tailored to the unique requirements of sustaining programs over time, thereby contributing to a more nuanced understanding of program sustainability and offering practical guidance for strengthening the longevity of evidence-based health promotion programs.

    近年来许多研究发现间充质干细胞(mesenchymal stem cells,MSC)具有向肿瘤部位趋化并对肿瘤细胞的增殖、侵袭甚至转移发挥直接或间接的促进作用;然而 MSC 的这些肿瘤原特性仍有待考证,甚至有可能发展为对肿瘤细胞相反的作用。MSC 对肿瘤细胞亦敌亦友:一方面,MSC 可以通过调节免疫监视、肿瘤细胞生长和血管生成来促进肿瘤发展;另一方面,MSC 可以依靠抑制信号通路来抑制肿瘤细胞生长。同时,由于 MSC 具有向肿瘤部位“归巢”和分泌细胞因子的特性,还被认为是一种理想的基因治疗载体。该文就 MSC 与肿瘤相互关系的近期研究进行了综述。

  • Child and adolescent obesity: challenges and responses in China

    ObjectiveTo systematically review the current prevalence, key determinants, and prevention strategies of child and adolescent obesity in China. MethodA systematic review of the epidemiological trends, health impacts, risk factors, and intervention strategies for child and adolescent obesity was performed by analyzing recent domestic and international literature and policy documents, supplemented with national surveillance data and developments in clinical practice. ResultsThe prevalence of child and adolescent obesity in China has been rising continuously over the past four decades, with particularly high rates observed among primary school children and boys. Recent trends show a “provincial reversal” and an “urban-rural reversal”. This high-risk environment stems from complex interactions among family, school, and societal factors. China has developed a relatively comprehensive policy framework for obesity prevention and control and is promoting a three-tiered prevention strategy. Clinically, pharmacological and surgical interventions are being gradually adopted. However, challenges remain, including limited health insurance coverage for obesity management. ConclusionsChild and adolescent obesity has become a significant public health issue in China, threatening national health and social development. Future efforts should focus on enhancing intersectoral governance, improving early intervention capabilities, expanding health insurance support, and standardizing clinical practices. A multi-level, multi-pathway integrated prevention and control system is essential for effectively curbing child and adolescent obesity under the healthy China strategy.
