  • 基于加速康复外科的综合护理模式对腰椎间盘突出症老年患者术后谵妄发生的效果研究

    目的 探究基于加速康复外科(enhanced recovery after surgery,ERAS)的综合护理模式对腰椎间盘突出症老年患者术后谵妄(postoperative delirium,POD)发生的效果研究。 方法 采取便利抽样的方法选取 2016 年 3 月—2017 年 2 月因腰椎间盘突出症行经皮内镜微创手术患者 80 例,将 2016 年 3 月—9 月的 40 例患者作为对照组,2016 年 10 月—2017 年 2 月的 40 例患者作为观察组。对照组采取常规围手术期护理措施,观察组采取基于 ERAS 的综合护理模式。对两组患者的 POD 发生率、术后住院时间、住院时间、患者满意度进行比较。 结果 观察组无 POD 发生,低于对照组[15%(6/40)],差异有统计学意义(χ2=4.505,P=0.034)。观察组患者术后住院时间[(1.0±0.5)d]和住院时间[(5.1±1.6)d]均低于对照组[(3.6±2.3)、(10.1±4.9)d],患者满意度[(99.8±0.5)分]高于对照组[(99.2±1.0)分],差异有统计学意义(P<0.05)。 结论 对腰椎间盘突出症老年患者实施基于 ERAS 的综合护理模式,降低了其 POD 的发生率,同时有利于缩短老年患者的术后住院时间、住院时间,提高患者满意度。

  • Analysis of associated factors of unplanned readmission within 30 days after discharge in colorectal cancer patients who underwent enhanced recovery after surgery mode

    ObjectiveTo understand the current situation of unplanned readmission of colorectal cancer patients within 30 days after discharge under the enhanced recovery after surgery (ERAS) mode, and to explore the influencing factors.MethodsFrom May 7, 2018 to May 29, 2020, 315 patients with colorectal cancer treated by Department of Gastrointestinal Surgery, West China Hospital, Sichuan University and managed by ERAS process during perioperative period were prospectively selected as the research objects. The general data, clinical disease data and discharge readiness of patients were obtained by questionnaire and electronic medical record. Telephone follow-up was used to find out whether the patient had unplanned readmission 30 days after discharge and logistic regression was used to analyze the influencing factors of unplanned readmission within 30 days after discharge.ResultsWithin 30 days after discharge, 37 patients were admitted to hospital again, the unplanned readmission rate was 11.7%. The primary cause of readmission was wound infection. Logistic regression analysis showed that the body mass decreased by more than 10% in recent half a year (OR=2.611, P=0.031), tumor location in rectum (OR=3.739, P=0.026), operative time ≤3 hours (OR=0.292, P=0.004), and discharge readiness (OR=0.967, P<0.001) were independent predictors of unplanned readmission.ConclusionsUnder the ERAS mode, the readmission rate of colorectal cancer patients within 30 days after discharge is not optimistic. Attention should be focused on patients with significant weight loss, rectal cancer, more than 3 hours of operative time, and low readiness for discharge. Among them, the patient’s body weight and discharge readiness are the factors that can be easily improved by clinical intervention. It can be considered as a new way to reduce the rate of unplanned readmission by improving the patients’ physical quality and carrying out discharge care program.

  • Analysis on differences of readiness for hospital discharge of colorectal cancer patients following enhanced recovery after surgery pathway by patients and nurses

    Objective To compare the differences in evaluating readiness for hospital discharge between nurses and colorectal cancer (CRC) patients following enhanced recovery after surgery (ERAS) pathway. Methods A cross-sectional survey was conducted in Department of Gastrointestinal Surgery, West China Hospital, Sichuan University. Patient-reported Readiness for Hospital Discharge Scale (RHDS) and nurse-reported RHDS were delivered to 130 CRC patients and 40 nurses respectively. All patients were followed ERAS pathway during perioperative periods. The differences were compared in evaluating readiness for hospital discharge between nurses and CRC patients. Results This study investigated 130 CRC patients and 40 responsible nurses. The scores of RHDS from nurses and patients were 162.86±27.95 and 149.86±33.65 respectively. When evaluating whether patients were ready to go home after discharge, the consistency between nurses’ results and patients’ results was weak(κ=0.365, P<0.001). Items in patients’ RHDS scoring ranking from high to low were expected support, coping ability, knowledge, and personal status. Items in nurses’ RHDS scoring ranking from high to low were expected support, knowledge, coping ability, and personal status. Besides the " social support” dimension, the scores of other 3 dimensions from nurses were significantly higher than those from patients (P<0.05). Conclusion There is a gap between the assessment of RHDS from nurses and patients, nurses overestimated patients’ discharge readiness level.

  • Influence of enhanced recovery after surgery on intestinal flora of patients with colorectal cancer based on high-throughput sequencing technology

    ObjectiveTo explore the influence of enhanced recovery after surgery (ERAS) on intestinal flora in patients with colorectal cancer.MethodsBy convenient sampling method, 60 patients with colorectal cancer were selected from August 2018 to December 2019 in the Department of Gastrointestinal Surgery of West China Hospital of Sichuan University and randomly divided into ERAS group and traditional treatment group (traditional group). Among them, the perioperative clinical management was carried out according to the ERAS management and traditional treatment process in the the ERAS group and in the traditional group, respectively. The fresh fecal samples were collected within 24 h after admission and the first natural defecation after operation. The bacterial 16 Sr DNA V3–V4 region was sequenced by Illumina MiSeq sequencer, and the results were analyzed by bioinformatics.ResultsA total of 60 patients with colorectal cancer were included, 30 cases in the traditional group and 27 cases in the ERAS group (3 people temporarily withdrew from the study). There were no significant differences in the basic informations between the two groups (P>0.05). ① Before or after operation, there were no significant differences in Shannon index and Simpson index between the two groups. The difference between preoperative and postoperative comparison in the same group was also not statistically significant (P>0.05). ② Beta diversity analysis showed that there was no significant difference in community composition between the traditional group and the ERAS group before operation, and there was a clear boundary between the traditional group and the ERAS group after operation. ③ At the phylum level, compared with the preoperative abundance, the postoperative abundance Firmicutes decreased by 26.5% and 5.5% in the traditional and ERAS group, respectively; Bacteroidetes increased by 21.6% and 4.7% in the traditional and ERAS group, respectively; Proteobacteria increased by 7.2% and 2.2% in the traditional and ERAS group, respectively. At the genus level, compared with the preoperative abundance, the postoperative abundance of Bacteroides in the traditional group increased by 17.6% and in the ERAS group decreased by 1.6%; Bifidobacterium decreased by 1.8% and 1.3% in the traditional group and in the ERAS group, respectively.ConclusionsERAS does not affect species diversity of intestinal flora. Although ERAS has some damage to structure of intestinal flora, it is weaker than traditional process, so it is more conducive to reconstruction and restoration of intestinal microecological environment.

  • Status quo and correlation between readiness for discharge and quality of discharge teaching in lung cancer patients based on enhanced recovery after surgery

    ObjectiveTo explore the current situation of the readiness for hospital discharge and the quality of discharge teaching in lung cancer patients based on enhanced recovery after surgery, and to analyze their correlation.MethodsWe conducted a cross-sectional study and 141 postoperative patients with lung cancer in our hospital from July to August 2018 were investigated by general information questionnaire, readiness for hospital discharge scale and quality of discharge teaching scale. There were 65 males and 76 females at age of 18-85 (55.35±12.15) years.ResultsTotally 88.65% of postoperative patients with lung cancer reported that they were ready for discharge, and the total score of readiness for hospital discharge was 78.36±16.48, and the total score of quality of discharge teaching was 90.94±18.62. There was a positive correlation between the readiness for hospital discharge and the quality of discharge teaching in postoperative patients with lung cancer (r=0.57, P<0.01).ConclusionThe readiness for hospital discharge of postoperative patients with lung cancer keeps in medium level, while the quality of discharge teaching is good, and the readiness for hospital discharge is positively correlated with the quality of discharge teaching. Medical staff should pay more attention to the cognitive level of lung cancer patients' disease-related information and their physical function recovery, and enrich the discharge guidance content, in order to improve the readiness of postoperative patients with lung cancer.

  • Analysis of risk factors for failure of enhanced rehabilitation surgery in patients undergoing partial hepatectomy

    ObjectiveTo explore the risk factors for failure of enhanced recovery after surgery (ERAS) in partial hepatectomy. MethodsA retrospective analysis was performed on 344 patients who underwent partial hepatectomy at the Division of Liver Surgery, Department of General Surgery in West China Hospital of Sichuan University from January 2019 to December 2019. All patients were treated with ERAS after partial hepatectomy. Univariate and multivariate logistic regression analysis were used to evaluate the risk factors associated with failure of the ERAS after partial hepatectomy. ResultsA total of 344 patients were included in the study, including 44 patients in the ERAS failure group and 300 patients in the ERAS success group. Multivariate logistic regression showed that combined with chronic diseases [OR=2.32, 95%CI (1.07, 4.93), P=0.03] and intraoperative fluid replacement volume ≤2 475 mL [OR=2.16, 95%CI (1.06, 4.42), P=0.03] were risk factors for failure of ERAS. ConclusionChronic diseases and intraoperative fluid volume ≤2 475 mL are risk factors for the failure of ERAS after partial hepatectomy and can affect prognosis.

  • 加速康复外科理念在良性前列腺增生围手术期护理中的应用

    目的 探讨加速康复外科(enhanced recovery after surgery,ERAS)理念在良性前列腺增生患者围手术期护理中的效果。 方法 纳入 2013 年 6 月—2015 年 4 月实施前列腺电切手术患者 60 例,随机分为研究组和对照组各 30 例。研究组按照 ERAS 理念进行护理,对照组给予常规护理。对两组患者术后下床时间、术后排气时间、术后住院时间、尿管留置时间、住院费用,以及术后并发症进行观察比较。 结果 研究组术后下床时间、术后排气时间和术后住院时间分别为(20.3±5.7)h、(23.4±7.3)h 和(3.4±0.6)d,较对照组的(25.2±6.6)h、(27.8±6.6)h、(3.7±0.7)d 缩短,差异有统计学意义(P<0.05);尿管留置时间两组之间差异无统计学意义(P>0.05);对照组总体并发症发生率(40.0%)和住院费用[(1.3±0.3)万]高于研究组[16.7%,(1.1±0.2)万],差异有统计学意义(P<0.05)。 结论 将 ERAS 理念应用于良性前列腺增生围手术期护理,能促进患者康复,减轻患者痛苦,有利于缩短住院时间,降低并发症发生率和患者经济负担,值得临床推广。

  • Application of self-assessment of pain in perioperative pain management of total knee arthroplasty

    Objective To explore the effect of self-assessment of pain in perioperative pain management of total knee arthroplasty (TKA). Methods A total of 140 patients undergoing TKA from March 2016 to March 2017 were randomly divided into the control group and the trial group. The patients in the two groups were received the same education relating to pain knowledge. The intensity of pain was assessed by nurses in the control group, while in the trial group, it was assessed by patients themselves. According to the assessment of pain, treatments were given to both groups. Time of pain assessment, types and frequencies of temporary rescue medicine, pain intensity, the score of Self-efficacy for Rehabilitation Outcome Scale (SER) and the range of motion (ROM) of knee were observed and recorded. Results There were 132 patients who completed the final observation, with 67 in the trial group and 65 in the control group. There were significant differences between the two groups in evaluation time of pain (t=–2.736, P=0.007), types and frequencies of temporary rescue medicine (χ2=10.276, P<0.05), the overall postoperative pain score (Z=–2.146, P=0.032), average hospitalization time after surgery (t=–2.468, P=0.015), SER scores 7 days after surgery (F=2.390, P=0.018) and 14 days after surgery (F=3.427, P=0.001), and ROM at the postoperative day 7 (F=2.109, P=0.037); there were no significant differences in postoperative daily pain scores (Z=–1.779, P=0.077), SER scores at the postoperative day 3 (F=1.010, P=0.314), ROM at the postoperative day 1 (F=1.319, P=0.189) and day 14 (F=1.603, P=0.111). Conclusion Self-assessment of pain can motivate TKA patients to take part in pain management, and more accurate response to the pain intensity will help to optimize the management of perioperative pain and reduce the workload of the health staff, thereby contributing to enhanced recovery.

  • Analyses of status and influencing factors of early ambulation in patients with gastric cancer under enhanced recovery surgery mode

    ObjectiveTo investigate the completion of early ambulation in patients with gastric cancer under the enhanced recovery after surgery (ERAS) management mode in the West China Hospital of Sichuan University, and analyze the influencing factors. MethodsFrom November 1, 2021 to March 31, 2022, the patients with gastric cancer who met the inclusion criteria of this study in the West China Hospital of Sichuan University were selected as the survey objects. At 48 h after the operation, the patients were enquired at the bedside and the electronic medical records were accessed to collect the general information, diseases information, etc. of the patients. The postoperative data were also investigated, and the time of early ambulation was investigated, and the influencing factors were analyzed by logistic regression. ResultsAccording to the inclusion and exclusion criteria of this study and the sample size requirements, 140 eligible patients with gastric cancer were investigated, 34 of whom got out of bed early, and the rate of early ambulation was 24.3%. The results of binary logistic regression analysis showed that indwelling urinary catheter within 48 h after operation [OR=10.031, 95%CI(1.037, 97.061), P=0.046] and American Society of Anaesthesiologists (ASA) grade Ⅲ [OR=4.209, 95%CI(1.792, 9.886), P=0.001] decreased the probability of early ambulation after operation in patients with gastric cancer. ConclusionsFrom the results of this survey, the completion rate of early ambulation in patients with gastric cancer under ERAS mode is lower, which may be improved by reducing the placement of the urinary catheter or shortening the indwelling time of the urinary catheter. For patients with ASA grade Ⅲ having functional decline before surgery, doctor or nurse needs to evaluate their mobility after surgery and help them to finish early ambulation or exercise on hospital bed within their tolerances.

  • The opinion of nurse on enhanced recovery after surgery in thoracic surgery: A questionnaire survey

    Objective To analyze the nurses' current view and perceptions of enhanced recovery after surgery (ERAS) by a questionnaire and to promote the clinical application of ERAS. Methods We conducted a questionnaire study for nurses who attended the First West China Forum on Chest ERAS in Chengdu during September 26-27, 2016 and 259 questionnaires were collected for descriptive analysis. Results (1) The application status of ERAS: There were 13.5% responders whose hospital took a wait-an-see attitude, while the others' hospital took different actions for ERAS; 85.7% of nurses believed that ERAS in all surgeries should be used; 58.7% of nurses believed that the concept of ERAS was more in theory than in the practice; 40.2% of nurses thought that all patients were suitable for the application of ERAS; (2) 81.9% of nurses believed that the evaluation criteria of ERAS should be a combination of the average hospital stay, patients’ comprehensive feelings and social satisfaction; (3) 70.7% of nurses thought that the combination of subjects integration, surgery orientation and surgeon-nurse teamwork was the best model of ERAS; 44.8% of nurses thought the hospital administration was the best way to promote ERAS applications; (4) 69.1% of responders believed that immature plan, no consensus and norms and insecurity for doctors were the reasons for poor compliance of ERAS; 79.5% of nurses thought that the ERAS meeting should include the publicity of norms and consensus, analysis and implementation of projects and the status and progress of ERAS. Conclusion ERAS concept has been recognized by most nurses. Multidisciplinary collaboration and hospital promotion is the best way to achieve clinical applications.
