  • Grasping the basic principle and getting rid of the misunderstanding of clinical application of visual electrophysiological technology

    Commonly clinical visual electrophysiological detection methods include flash ERG, pattern ERG, VEP and multifocal ERG, etc. These inspection methods play an important role in disease identification and visual function judgment. However, lacking of understand the basic principles of electrophysiological testing, misunderstandings of the clinician influence the choice of examination methods or interpretation of examination results. The understanding of the basic principles of electrophysiological testing and analysis of the causes of these misunderstandings have important clinical significance in the correct application of visual electrophysiological testing technology.

  • 特发性息肉状脉络膜血管病变

    特发性息肉状脉络膜血管病变(idiopathic polypoidal choroidal vasculopathy, IPCV)是近年认识的一种与渗出型老年黄斑变性(age-related macular degeneration, AMD)表现较为相似的眼病,但两种疾病在流行病学、病情转归、视力预后及治疗等方面均有较大的不同。因此IPCV诊断及鉴别诊断具有较为重要的临床意义。IPCV的吲哚青绿血管造影(indocyanine green angiography,ICGA)显示有分支状脉络膜血管网及末梢的息肉状血管扩张等特征性表现,为IPCV诊断与鉴别诊断提供了重要的依据。 (中华眼底病杂志, 2002, 18: 310-312)

  • AN INVESTIGATION OF AGE-RELATED MACULAR DEGENERATION BY USING DENSE MATRIX MAPPING IN MACULA

    PURPOSE:To approach the changes of visual function by using the dense matrix mapping in macula(DMMM)in patients with age-related macular degenerationl(AMD). METHODS:The DMMM was designed using Humphrey-640,thee study included 51 patients(71 eyes)in dry type,AMD,23 patients(27 eyes)in wet type and 16 normal elderly(21 eyes). RESULTS:In 2.5 degree of macula:the mean light sensitivity(MLS)was 35.61plusmn;1.37 in normal elderly.33.30plusmn;2. 49 in dry type group and 20.67plusmn;8.30 in wet type group,and differences were highly significant statistically among them. In S degree the MLS is 34.93plusmn;1.46 in normal elderly.32.95plusmn;1.82 in dry type group, 22.11plusmn;7.74 in wet type group ,and the differences were highly significant statistically among them. CONCLUSIONS:The MLS of DMMM declined at early stage of dry type AMD and there was relatively high sensitivity of MLS of DMMM in evaluating the visual function in early AMD.   • AN INVESTIGATION OF APPROACHES OF TRANSPLANTAITION OF RETINAL PIGMENT EPITHELIUM

    PURPOSE:To investigate the approaches for transplanting retinal pigment epithelium. METHODS,Retinal pigment epithelial eells(RPR)of pigmented rabbits' eyes prepared by rotalne preparation of our institute,were transphmted in 18 unpigmemed rabbits'eyes.Eight eyes were undergone outer approach, i.e., transplanting the RPR cells to the subretinal space of recipient eyes by way of perforating sclera and choroid;while 10 eyes were undergone internal approach by way of the routine procedure of vitrectomy with making artificial localized retinal delachment. Light and transmisskm electrone microscopy examination were done at 10th, goth, 40th and 90th day after the operation. RESULTS: In internal approach group,tbe operated eyes,revealed no difference in thickness of the neural retinal layer in transplanted and non-transplanted area 40 days after operation tinder light microscope. Transmission electrone microscopy revealed postoperatively the transplanted RPE cells attached to the Brucb's membrane and the outer segments of photoreeeplive ceils located at a normal position at the 40th dayland the secondary lysozymes with engulfed outer segment were found in the Iransplamed cells at the 90th day. Tbe outer approached operations in eight eyes were failed owing to ehoroid hemorrhage or perforation of retina. CONCLUSION:The internal appraach procedure is much effebtive and practical for transplantation of RPE cells. (Chin J Ocul Fundus Dis,1997,13:160-162)

  • THE INDOCYANINE GREEN ANGIOGRAPHY AND AGE-RELATED MACULAR DEGENERATION WITH MACULAR HEMORRHAGE

    PURPOSE:To search for the occult choroidal neovascularization(CNV)of age-related macular degeneration (AMD)with macular hemorrhage using indocyanine green angiography(ICGA). METHODS:FFA and ICGA were performed in a series of 22 cases(24 eyes)of AMD with macular hemorrhage,and the findings of both angiograms were compared each other. RESULTS :ICGA was found to be superior than FFA in evaluating the occult CNV of AMD with hemorrhage owing to the following outstanding findings in this series,i.e,in judging the presence,position,number and range of the occult CNV. CONCLUSION:ICGA is an important technique in diagnosing the subretinal occult CNV in AMD with macular hemorrhage,and useful in selecting therapeutic measures including photocoagulation and surgical treatment.   • THE FOLLOW-UP OF VISUAL FUNCTION AND MACULAR LESIONS IN DRY TYPE OF AGE-RELATED MACULAR DEGENERATION

    OBJECTlVE:To investigate Ihe changes of macuiar lesions in dry type of age re[amd maeuJar clegcneration(AMD)and search for a sensitive melhod for detecting tile development of the disease. METHODS:The fundus fluoreseein angiography(FFA) ,visual acuity,FM 100-hue test and photopie electroretinogram(ERG)were used to examine a series of 60 patients(111 eyes)with dry AMD aged 50~80 years with the visual acuity of le;1.0.The patients were felhwed tip in 3~74 months(average 30.2 months). RESULTS:In 68 eyes undergone FFA examination and followed llp for Ihe average period of 25.6 months ,the macular lesions were found worsened in 25%, The visual acuity in follow-up periods was found decreasing more than 2 lines in 18% of the fotal 111 affectd eyes.There were not any statistically significat difference in photopic ERG between the initial and final cxaminations in 63 eyes tested. The tolal error score of FM 100-hue test had a statistically significant difference between the initial test and the test taken two years afterwards(Plt; 0.01 )in 81 eyes examlnccl. CONCLUSIONS:Most of the macular lesions and visual acuity in dry type of AMD revealed a [avorahle prognosis,but occasionally complicated with ehoroidal neovaseularization. The total error score of FM 100-hue test might be a sensitive method for monitoring the development of dry type of AMD. (Chin J Ocul Fundus Dis,1997,13: 150-152)

  • The analysis of mitochondrial DNA point mutation at position 3243 in age-related macular degeneration

    Purpose To detect whether a 3243 point mutation existed in age-related macular degeneration (AMD). MethodsTwenty-six cases of wet form AMD patients, ten cases of dry form AMD patients were selected,and compared with twenty nomal controls. After collecting anti-coagulated blood samples, total cellular DNA were extracted and purified. Using polymerase chain reaction and restriction fragment long polymorphism techniques, the mtDNA Ararr;G point mutation at position 3243 were detected. Results After cleaveded by restriction endonuclease Apa I, a 294 bp fragment remained only in all detected DNA samples including twenty-six wet form AMD, and ten dry form AMD. No any other fragment appeared. The result showed that there was no Ararr;G mutation at position 3243 found in AMD. Conclusion It is suggested that mtDNA 3243 point mutation due to maternal inheritance might be not concerned with both wet form AMD and dry form AMD. (Chin J Ocul Fundus Dis,2000,16:231-232)
