Objective To study the therapeutic effect of Roux-en-Y gastric bypass (RYGB) on type 2 diabetes mellitus (T2DM) rats and explore the possible mechanism of vaspin in RYGB on T2DM. Methods Twenty SD rats with T2DM and 20 age- and sex-matched normal SD rats were randomly divided into 4 groups according to the random digits table:T2DM-RYGB group, T2DM-sham operation (SO) group,RYGB group, and SO group,10 rats in each group. Fasting plasma glucose (FPG) level, serum insulin (INS) level, vaspin level, and homeostasis model of insulin resistance (HOMA-IR) were determined before operation and on week 4,8 after operation, respectively. At the same time, the correlation between vaspin and the indicators (FPG,INS,or HOMA-IR) was analyzed. Results Compared the indicators after operation with before operation, the FPG level,INS level, vaspin level, and HOMA-IR were not significantly different between the T2DM-RYGB group and T2DM-SO group (P>0.05) or between the RYGB group and SO group (P>0.05),but the FPG level,INS level, vaspin level, and HOMA-IR in the T2DM-RYGB group and T2DM-SO group were significantly higher than those in the RYGB group (P<0.05) and SO group (P<0.05),respectively. On week 4 after operation, the FPG level,INS level, vaspin level, and HOMA-IR decreased in the T2DM-RYGB group, except for the FPG level, the other indexes had no significant differences as compared with the values before operation. On week 8 after operation, the FPG level,INS level, vaspin level, and HOMA-IR further decreased in the T2DM-RYGB group, there were significant differences of these indicators between before operation and on week 8 after operation. Compared the indicators after operation with before operation, the FPG level,INS level, vaspin level, and HOMA-IR were not statistically significant (P>0.05) in the T2DM-SO group,RYGB group,or SO group. The changes in serum vaspin level correlated positively with those in INS and HOMA-IR before operaion and on week 4,8 after operaion in the T2DM-RYGB group and T2DM SO group rats (P<0.05),respectively. Conclusions RYGB surgery has a therapeutic effect on T2DM rats, and serum vaspin level decreases and insulin resistance is improved after RYGB surgery, which may be one of the mechanisms of the treatment for T2DM.

Abstract: Objective: To discuss the relationship between diabetes and pedopathy of type II diabetes and insulin resistance. Methods:The diabetes type II patients were divided into group A (combined with pedopathy) and group B (without pedopathy). The blood glucose and insulin of empty stomach, BMI,Alc and lipid were detected. The insulin resistance index (HOMAIR) was calculated and compared between two groups. Results:The HOMAIR was higher in group A than that in group B (Plt;0.05).The duration of disease,LDL and BMI was positive related with diabetes pedopathy. Conclusion:The insulin resistance was more worse in pedopathy of Type II diabetes.
Patients with type 2 diabetes mellitus often face significant treatment burden, which substantially impacts their quality of life and health outcomes. Reducing treatment burden represents a critical component for improving patient prognosis and enhancing treatment adherence. Based on the cumulative complexity model, this article systematically examines the conceptual connotation and multidimensional characteristics of treatment burden in type 2 diabetes mellitus patients, explores the theoretical extension and application value of cumulative complexity model in the type 2 diabetes mellitus field, elucidates its specific applications and recent advances in treatment burden research, evaluates the limitations of existing assessment tools while proposing a multidimensional assessment framework, and ultimately develops cumulative complexity model based intervention strategies. The findings provide theoretical references for optimizing patient-centered diabetes management approaches and offer novel perspectives for treatment burden intervention.
Objective To evaluate the efficacy and safety of saxagliptin in type 2 diabetes patients. Methods The following databases as The Cochrane Library (Issue 2, 2011), PubMed (1978 to May 2011), EMbase (1974 to May 2011), CNKI (1978 to May 2011), VIP (1989 to May 2011) and CBM (1978 to May 2011) were searched. The quality of included randomized controlled trials (RCTs) was assessed according to the Cochrane Collaboration system review, and then meta-analysis was performed using RevMan 5.0. Results A total of 7 RCTs were included. The results of meta-analyses showed that HbA1c was significantly reduced in the saxagliptin group than that in placebo group (MD= –0.69, 95%CI –0.78 to –0.60, Plt;0.000 01). There was no significant difference in the incident rate of adverse reaction between two groups (RR=1.02, 95%CI 0.98 to 1.06, P=0.26). Conclusion Saxagliptin is effective and safe for type 2 diabetes. But its long-term efficacy and safety still need to be confirmed by performing more high quality, large sample RCTs with long-term follow-up.
Objective To systematically review the efficacy of short message intervention on glycosylated hemoglobin (HbAlc) and blood glucose levels on patients with type 2 diabetes mellitus. Methods We electronically searched databases including PubMed, EMbase, The Cochrane Library, CNKI and CBM from inception to August 2016, to collect randomized controlled trials (RCTs) about short message intervention in type 2 diabetes mellitus patients. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. Then meta-analysis was performed using Stata 12.0 software. Results A total of 17 RCTs involving 2 879 patients were included. The results of meta-analysis showed that, on the basis of basic treatment, the use of text messages could lower the levels of HbA1c (SMD3 months=–0.50, 95%CI –0.67 to –0.34, P<0.001; SMD6 months=–0.33, 95%CI –0.59 to –0.07, P=0.012), FBG (SMD3 months=–0.28, 95%CI –0.45 to –0.11, P=0.001; SMD9 months=–0.99, 95%CI –1.44 to –0.54, P<0.001), PBG (SMD3 months=–0.99, 95%CI –1.54 to –0.44, P<0.001; SMD6 months=–0.81, 95%CI –1.25 to –0.36, P<0.001; SMD9 months=–0.78, 95%CI –1.23 to –0.34, P=0.001). Conclusion The current evidence shows that the use of SMS can improve glycosylated hemoglobin and blood glucose levels on type 2 diabetes patients. Due to limited quality and quantity of the included studies, the above conclusions are needed to be verified by more high quality studies.
Objective To investigate effect of metabolic surgery on type 2 diabetes mellitus (T2DM) patients with body mass index (BMI) 27.5–32.5 kg/m2. Methods The clinical data of 43 T2DM patients with BMI 27.5–32.5 kg/m2 underwent metabolic surgery from October 2014 to October 2016 in the Third Xiangya Hospital of Central South University were analyzed retrospectively. The related indexes such as BMI, blood glucose level, blood lipid level were analyzed before and after metabolic surgery. Results All the patients underwent metabolic surgery successfully. Among them, 35 cases underwent laparoscopic gastric bypass surgery while 8 cases underwent laparoscopic sleeve gastrectomy without related complications after operation. Compared with preoperative indexes, the BMI, fasting blood glucose, HbA1c, triglyceride, and total cholesterol on the postoperative different time were all significantly decreased (P<0.05) except for the HbA1c on the postoperative 1-week, the high density lipoprotein level on the postoperative 12-month was significantly increased (P<0.05). The OGTT 30, 60, and 120 min blood glucose levels on the postoperative 1-week and 3-month, and 60 and 120 min blood glucose levels on the postoperative 6-month and 12-month were all significantly decreased (P<0.05). The OGTT-IRT 60 min insulin level on the postoperative 3-month and the 30 min insulin levels on the postoperative 6- and 12-month were all significantly increased (P<0.05). The levels of OGTT-CRT 30 and 60 min C peptide on the postoperative 6-month and the level of 30 min C peptide on the postoperative 12-month were all significantly increased (P<0.05). Conclusions Metabolic surgery is effective in treatment of T2DM patients with BMI 27.5–32.5 kg/m2, and levels of blood glucose and blood lipids can be improved significantly. Synthesis and release of insulin by islet cells can be ameliorated.
