  • 急性心肌梗死合并焦虑抑郁患者的心理干预

    【摘要】 目的 评价心理干预措施在急性心肌梗死治疗中的意义。方法 2008年2月—2009年2月,将50例急性心肌梗死合并焦虑抑郁患者随机分为对照组和观察组,对照组常规治疗,观察组在对照组的基础上联用心理干预措施。对两组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)、满意度等情况进行比较。结果 两组患者负性情绪均有所好转,但治疗后观察组SAS评分为(32.15±6.17)分,明显低于对照组的(46.28±7.35)分(Plt;0.05);观察组SDS评分为(27.59±6.31)分,明显低于对照组的(38.75±5.69)分(Plt;0.05)。出院时观察组总满意率为64%,明显高于对照组的总满意率(36%)(Plt;005)。结论 在采用常规治疗的基础上联用心理干预措施治疗急性心肌梗死合并焦虑抑郁有提高疗效的作用,建议临床进一步推广。

  • 乳腺癌患者围手术期心理问题及护理干预

    目的探讨乳腺癌患者的心理问题及有效的心理干预措施。 方法选择2010年1月-2012年5月65例确诊为乳腺癌并行根治性手术患者,对其围手术期的心理问题进行评估,并通过有效沟通、鼓励与支持、提供科学方法、充分调动家庭和社会支持系统等手段给予相应的心理干预。 结果经干预,患者的心理问题得到了疏导,能主动配合治疗,健康指导依从率高,患侧肢体淋巴水肿发生率1.54%,以积极乐观的生活态度重返社会。 结论重视乳腺癌患者的心理问题,提供有效的心理干预,对于完善护理人文关怀,促进患者全面康复、重返社会有着积极的意义。

  • A Clinical Study of Fluoxetine Plus Mental Intervention in Cardiovascular Disease with Anxiety and Depression

    目的:探讨氟西汀联合心理干预治疗心血管疾病患者伴焦虑抑郁症状的临床疗效。方法:选择伴有抑郁、焦虑情绪障碍的85例冠心病患者(心功能Ⅱ、Ⅲ级),并将其随机分成研究组和对照组。观察6w,对照组患者仅给予常规的治疗,研究组患者在常规治疗基础上给予氟西汀联合心理干预治疗。观察治疗前后两组患者采用汉密顿焦虑量表( Hamilton Anxiety Scale,HAMA) 评定焦虑症状,汉密顿抑郁量表( Hamilton Depression Scale,HAMD) 评定抑郁症状;并对治疗后心功能分级(NYHA)恢复到Ⅰ级的例数及左室射血分数进行分析评价。结果:治疗6 周 后,研究组均较对照组的汉密顿焦虑量表及汉密顿抑郁量表评分下降显著( P lt;0.01),研究组抗焦虑显效率为87.16%、抗抑郁显效率为82.26%,对照组分别为43.75%、45.36%,研究组均显著高于对照组( P lt;0.01);研究组患者的左室射血分数显著高于对照组 ( P lt;0.05),心功能分级(NYHA)恢复到Ⅰ级的例数显著高于对照组 ( P lt;0.05)。结论:氟西汀联合心理干预治疗不仅能改善心血管疾病伴焦虑抑郁患者的焦虑、抑郁情绪障碍,还能改善患者的心功能,疗效显著优于单用心血管药物治疗。

  • Effectiveness of Psychological Intervention on Post-stoke Depression: A Systematic Review

    Objective To assess the effectiveness of psychological intervention on post-stoke depression. Methods Such databases as the JBI Database of Systematic Review (1980 to June, 2010), The Cochrane Library (1980 to June, 2010), PubMed (1966 to 2010), CINAHL(1982 to May, 2000), CBM (1978 to 2010), and CNKI (1979 to 2010) were searched to collect randomized controlled trials (RCTs). In accordance with the predefined inclusion and exclusion criteria, the quality of included studies was evaluated, and then meta-analyses were performed by using RevMan 5.0 software. Results A total of 33 RCTs were included. The results of meta-analyses showed: (1) Compared with the control group, the short-term effect of psychological intervention was more effective in decreasing depression score. The subgroup analysis showed that the intervention effects at the time of four weeks, six weeks, eight weeks, and 12 weeks were better than those of the control group. (2) The long-term effect of psychological intervention was more effective in decreasing depression score. The subgroup analyses showed that the intervention effects at the interval of eight weeks, 24 weeks, and 48 weeks were better than those of the control group. (3) The combined or single application of either cognitive-behavioral psychotherapy or supportive psychotherapy was more effective in decreasing depression score than the control group. However, there was no significant difference between the general psychological treatment group and the control group. (4) The subgroup analyses showed that the different qualities of the included studies were more effective than those of the control group. Conclusion Various psychological intervention is effective in decreasing the patient’s depression score, and cognitive-behavioral therapy and supportive psychotherapy, especially, can significantly improve the depression state and promote recovery.

  • 心理干预对耳鸣患者疗效的临床观察

    目的 探讨心理干预对耳鸣的治疗效果。 方法 2006年6月-2009年3月对耳鼻喉科门诊以耳鸣为第一主诉的患者87例按单、双号分为两组,治疗组45例接受心理干预和药物治疗,对照组42例仅接受药物治疗。治疗前后耳鸣程度用症状自评量表SCL-90及主观评估进行比较,并作两组间的比较。 结果 两组患者治疗前SCL-90评分的差异无统计学意义(Pgt;0.05)。治疗组治疗前后SCL-90在躯体化、人际关系敏感、抑郁、焦虑、敌对、恐怖和偏执因子的差异均有统计学意义(Plt;0.05)。对照组治疗前后各因子差异无统计学意义(Pgt;0.05)。两组治疗前后躯体化、人际关系敏感、抑郁、焦虑,敌对、恐怖、偏执因子比较,差异均有统计学意义(Plt;0.05)。两组患者疗效均有改善,而治疗组改善更为明显(Plt;0.05)。 结论 心理干预对治疗耳鸣患者有较好的效果。

  • Investigations of the Hospitalized Wounded Psychosomatic State in 5·12 Sichuan Earthquake and the Psychological Interventions of Acute Phase

    目的:了解地震住院伤员的身心状态,进行针对性心理干预。方法:采用方便抽样,应用作者根据应激理论设计的应激身心反应调查表[1],通过对地震伤员的观察和访谈,对在2008年5月12日至5月30日在四川大学华西医院住院的371名地震伤员进行身心状态的评估,并进行有针对性的心理干预。结果:①地震伤员灾后急性期心身应激反应较严重,其中出现频率最多的症状是睡眠困难,对突然的噪音或声音感到紧张害怕,心情沮丧,感到身体紧张,心情难以平静;②不同性别地震伤员身心反应存在一定差异,女性反应症状重于男性;③年龄在小于12岁、12~18岁和41~50岁年龄段地震伤员的身心反应与其他年龄段地震伤员的身心反应存在差异;④地震伤员的身心反应与其性别、年龄、文化、个人受伤情况有关。结论:地震伤员灾后身心反应严重,急性期给予针对性心理干预十分重要。

  • 汶川地震灾民心理健康调查

    摘要:目的: 了解汶川地震后灾民的心理健康状况,为对灾民进行心理干预提供科学依据。 方法 :采用创伤后应激障碍症状清单平民版(PCLC)问卷对四川省绵阳市安县京安小区安置点的1 100名灾民进行了随机调查。 结果 :安县灾民PCLC均分者占调查人数的8.8%。女性PCLC 均分高于男性,不同年龄段PCLC均分差异无统计学意义。文化程度方面文盲受损程度较高,其他不同学历组间PCLC均分差异无统计学意义。地震前存在躯体疾病者PCLC均分较高,地震中有掩埋经历者PTSD症状较严重。 结论 :地震灾害造成灾民的心理健康问题较为突出,加强心理卫生教育及对灾民的心理干预是非常必要的,尤以女性、震前存在躯体疾病以及有掩埋经历者应给予重点关注。

  • 心理干预在体检人员采血晕针中的应用

    目的 在体检采血中对体检人员实施心理干预,有效解除体检人员的心理障碍,达到预防晕针的目的。 方法 选择2011年1月-6月行健康体检的33 372人(对照组),2011年7月-12月行健康体检的46 796人(干预组)进行比较分析。对照组采用常规体检服务程序方式,干预组采用灵活的个性化心理干预方法,观察两组体检人员采血时晕针发生率。 结果 对照组和干预组在接受采血时晕针发生率比较,干预组晕针发生率低于对照组,差异具有统计学意义(P<0.05)。 结论 使用灵活的个性化心理干预方法对采血体检人员进行语言疏导,在注意力分散及松弛疗法的情况下完成采血过程,可降低体检人员晕针发生率, 值得临床推广。

  • Application of peer education and group psychological intervention in patients with spinal cord injury

    ObjectiveTo explore the application and effect of peer education combined with group psychological intervention in patients with spinal cord injury.Methodspatients with spinal cord injury admitted to the Rehabilitation Medicine Center of West China Hospital, Sichuan University from April to June 2019 were selected. According to the random number table method, the patients were randomly divided into the control group and the experimental group. The patients in the control group received routine psychological nursing intervention; while those in the experimental group were given routine psychological nursing intervention, supplemented by peer education and group psychological intervention. Before and 1 month after the intervention, the self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), the Short Form-36 (SF-36), and the compliance of rehabilitation treatment were compared between the two groups.ResultA total of 51 patients with spinal cord injury were admitted and 40 were eventually included, with 20 in each group. Before the intervention, there was no statistically significant difference between the two groups in SAS, SDS, SF-36 or rehabilitation treatment compliance (P>0.05). After the intervention, SAS [(28.60±3.30) points], SDS [(33.35±2.32) points], SF-36 [(86.60±4.56) points], and the rehabilitation treatment compliance [(83.28±5.07) %] in the experimental group were significantly improved compared with those in the control group [(34.75±6.17) points, (45.90±3.81) points, (80.90±5.19) points, (75.61±5.94) %; t=−3.932, −12.580, −3.694, 4.397, P<0.001]. After the intervention, SAS and SDS of the experimental group decreased compared with those before the intervention (P<0.05); while SF-36 and rehabilitation treatment compliance were higher than those before the intervention (P<0.05). Compared with those before the intervention, the SAS, SDS, SF-36 and rehabilitation treatment compliance of the control group after the intervention were not statistically significant (P>0.05).ConclusionGroup psychological intervention combined with peer education can effectively promote the psychological rehabilitation of patients with spinal cord injury and improve the curative effect and patients' quality of life.
