  • Research on multi-scale convolutional neural network hand muscle strength prediction model improved based on convolutional attention module

    In order to realize the quantitative assessment of muscle strength in hand function rehabilitation and then formulate scientific and effective rehabilitation training strategies, this paper constructs a multi-scale convolutional neural network (MSCNN) - convolutional block attention module (CBAM) - bidirectional long short-term memory network (BiLSTM) muscle strength prediction model to fully explore the spatial and temporal features of the data and simultaneously suppress useless features, and finally achieve the improvement of the accuracy of the muscle strength prediction model. To verify the effectiveness of the model proposed in this paper, the model in this paper is compared with traditional models such as support vector machine (SVM), random forest (RF), convolutional neural network (CNN), CNN - squeeze excitation network (SENet), MSCNN-CBAM and MSCNN-BiLSTM, and the effect of muscle strength prediction by each model is investigated when the hand force application changes from 40% of the maximum voluntary contraction force (MVC) to 60% of the MVC. The research results show that as the hand force application increases, the effect of the muscle strength prediction model becomes worse. Then the ablation experiment is used to analyze the influence degree of each module on the muscle strength prediction result, and it is found that the CBAM module plays a key role in the model. Therefore, by using the model in this article, the accuracy of muscle strength prediction can be effectively improved, and the characteristics and laws of hand muscle activities can be deeply understood, providing assistance for further exploring the mechanism of hand functions.

  • EFFECTIVENESS OF RETROGRADE ISLAND NEUROCUTANEOUS FLAP PEDICLED WITH LATERAL ANTEBRACHIAL CUTANEOUS NERVE IN TREATMENT OF HAND DEFECT

    ObjectiveTo explore the effectiveness of retrograde island neurocutaneous flap pedicled with lateral antebrachial cutaneous nerve in the treatment of soft tissue defect of the hand. MethodsBetween October 2011 and December 2013, 17 cases of skin and soft tissue defects of the hands were treated. There were 8 males and 9 females, aged 23-62 years (mean, 44 years). Of them, defect was caused by trauma in 13 cases, by postoperative wound after degloving injury in 2 cases, and by resection of contracture of the first web in 2 cases; 13 cases of traumas had a disease duration of 2-6 hours (mean, 3.5 hours). The defect sites located at the back of the hand in 5 cases, at the radial side of the palm in 4 cases, at the first web in 2 cases, at the palmar side of the thumb in 4 cases, and at the radial dorsal side of the thumb in 2 cases. The bone, tendons, and other deep tissue were exposed in 15 cases. The defect size varied from 3 cm×3 cm to 12 cm×8 cm. The size of the flaps ranged from 3.6 cm×3.6 cm to 13.2 cm×8.8 cm. The lateral cutaneous nerve of the forearm was anastomosed with the cutaneous nerve of the reci pient sites in 9 cases. The donor sites were repaired by free skin graft or were sutured directly. ResultsThe other flaps survived, and obtained healing by first intention except 2 flaps which had partial necrosis with healing by second intention at 1 month after dressing change. The skin graft at donor site survived, and incisions healed by first intention. All patients were followed up 5-30 months (mean, 12 months). The flaps had good color and texture. Flap sensory recovery of S2-S3+ was obtained; in 9 cases undergoing cutaneous nerve flap anastomosis, the sensation of the flaps recovered to S3-S3+ and was better than that of 8 cases that the nerves were disconnected (S2-S3). The patients achieved satisfactory recovery of hand function. Only 2 cases had extended limitation of the proximal interphalangeal joint. At last follow-up, according to the Chinese Medical Society of Hand Surgery function evaluation standards, the results were excellent in 15 cases and good in 2 cases. ConclusionRetrograde island neurocutaneous flap pedicled with lateral antebrachial cutaneous nerve is an effective way to repair skin defects of the hand, with the advantages of rel iable blood supply and simple surgical procedure.

  • 156例手部深度烧伤的早期整形修复

    目的 探讨手部深度烧伤后早期整形修复的治疗方法及临床效果。 方法 1994年12月~2004年12月,收治手部深度烧伤156例(198手)患者。男119例155手,女37例43手。年龄21~52岁。深Ⅱ度105例121手,Ⅲ度51例77手。伤后24 h内手术29例36手,1周内手术94例117手,3周内手术33例45手。患者均采用中厚皮片、全厚皮片植皮或皮瓣修复,术后进行早期功能锻炼。 结果 术后无并发症发生,创面Ⅰ期愈合,皮瓣及植皮均成活。供区Ⅰ期愈合。145手获随访6个月~5年。参考王澍寰(2002)手功能评价标准:优109手(75.2%),良18手(12.4%),中11手(7.6%),差7手(4.8%)。结论 手部深度烧伤后宜早期行整形修复手术,可有效预防手部瘢痕形成,功能得到良好恢复。

  • EXPERIENCE OF APPLICATION OF SKIN FLAP IN REPAIRING 112 PATIENTS WITH SEVERE THERMO PRESSURE INJURY OF HAND

    OBJECTIVE: To investigate the clinical effect of skin flaps repairing severe thermopressure injury of hand. METHODS: From January 1989 to December 1998, 112 patients with severe thermopressure injury of hand were repaired by various skin flaps transfer, the size of skin flaps was 6 cm x 8 cm to 12 cm x 18 cm. Postoperative patients were treated by combined rehabilitation in early stage. RESULTS: All the flaps were survived with satisfactory effect. Sixty-six patients were followed up 6 to 12 months, skin flaps all showed better colour and texture, and function of the hand was satisfactory. CONCLUSION: Different skin flaps are adopted to repair severe thermopressure injury of the hand according to different skin defects of the hand, combined early rehabilitation treatment, to achieve good recovery of function and appearance of the hand to the greatest extent.

  • 双套血供的前臂尺背侧逆行岛状皮瓣修复手部创面

    目的总结双套血供的前臂尺背侧逆行岛状皮瓣修复手部创面的临床疗效。 方法2010年3 月-2012年6月,采用带尺动脉腕上皮支和骨间背侧血管双套血供的前臂尺背侧逆行岛状皮瓣修复手部创面21例。其中男13例,女8例;年龄19~58岁,平均33.5岁。机器绞伤9例,挤压伤7例,热压伤5例。创面部位:虎口3例,手背侧12例,手掌侧6例。创面范围6 cm × 5 cm~13 cm × 10 cm。伤后至手术时间1.5 h~11 d,平均5.5 d;其中一期修复11例,二期修复10例。术中皮瓣切取范围7 cm × 6 cm~16 cm × 12 cm。 结果术后1例皮瓣以远1/3发生坏死,经二期植皮后成活;其余皮瓣及供区植皮均顺利成活,切口Ⅰ期愈合。术后14例获随访,随访时间6~18个月,皮瓣色泽、质地好,温、痛、触觉恢复。末次随访时,手功能采用中华医学会手外科学会上肢部分功能评定试用标准评定:获优8例,良4例,中1 例,差1例,优良率85.7%。供区无功能影响。 结论双套血供的前臂尺背侧逆行岛状皮瓣修复手部创面具有操作简便、可切取面积大、血运可靠、术后外观及功能恢复良好等优点。

  • 前臂骨间背侧动脉逆行岛状皮瓣修复手部创面

    目的 总结前臂骨间背侧动脉逆行岛状皮瓣修复手部创面的临床疗效。 方法 2004 年3 月-2010 年3 月,采用前臂骨间背侧动脉逆行岛状皮瓣修复手部创面25 例。男19 例,女6 例;年龄5 ~ 57 岁,平均32.7 岁。机器绞伤8 例,压砸伤7 例,挤压伤2 例,热压伤1 例,虎口挛缩组织切除后7 例。左侧11 例,右侧14 例。创面部位:虎口7 例,腕掌侧2 例,手背15 例,手掌尺侧小鱼际部1 例。创面范围3 cm × 3 cm ~ 9 cm × 7 cm。除虎口挛缩患者择期手术外,其余患者受伤至手术时间为1.5 h ~ 11 d,平均5 h。术中皮瓣切取范围为3.5 cm × 3.5 cm ~ 10.0 cm × 8.0 cm。供区直接缝合或中厚皮片移植修复。 结果 术后2 周1 例皮瓣以远1/3 发生坏死,经二期植皮后愈合;其余皮瓣及供区植皮均顺利成活,切口均Ⅰ期愈合。术后14 例获随访,随访时间6 个月~ 3 年。皮瓣色泽、质地好,温、痛、触觉恢复。虎口挛缩患者术后30 d 开大虎口夹角度≥ 50°,可完成拇指外展、对掌功能。 结论 前臂骨间背侧动脉逆行岛状皮瓣修复手部创面具有手术操作简便、术后功能恢复良好等优点。

  • CORTICOPLASTY FOR MULTIPLE ENCHONDROMATOSIS OF HAND

    ObjectiveTo evaluate the results of corticoplasty for multiple enchondromatosis of the hand. MethodsBetween February 2003 and January 2011, 6 patients with multiple enchondromatosis were treated. Of 6 cases, 1 was boy and 5 were girls with an average age of 10.8 years (range, 9-12 years); 5 cases presented with painless mass as first symptom, and 1 case was found to have mass by X-ray film because of hand injury. Physical examination at admission showed multiple mass on the hands, fingers deformity, and limited range of motion; X-ray film results showed large lesions in the phalanges and metacarpals. Corticoplasty and simple curettage without bone grafting were performed on 24 fingers (60 bones) with multiple enchondromatosis of the hand. The fingers active range of motion was used to evaluate the finger function; the diameter of the tumors was measured on the X-ray films; and according to Tordai's classification, tumor recurrence and new bone formation were observed. ResultsOnce, twice, or three-time operations were performed in 2 cases, respectively. All patients were followed up 17-83 months with an average of 52.2 months. At last follow-up, the active range of motion was significantly increased from (230.8±53.2)° at preoperation to (255.0±28.7)° at postoperation (t=—3.829, P=0.001); the tumor diameter was significantly decreased from (15.6±5.8) mm at preoperation to (10.7±3.7) mm at postoperation (t=8.304, P=0.000). Of 60 bones, 34 (56.7%) were rated as Tordai grade 1, and 26 (43.3%) as Tordai grade 2. During follow-up, clinical manifestation, characteristics of radiology and pathological examination showed no pathological fracture or malignant change. ConclusionCorticoplasty is a safe and effective treatment for multiple enchondromatosis of the hand in children. The procedure can improve appearance and motion function of the hand.

  • REPAIR OF MULTIPLE FINGERS DEGLOVING INJURY WITH ABDOMINAL“S”-TYPE SKIN FLAP

    OBJECTIVE: To explore a new surgical management of multiple fingers degloving injury. METHODS: In 1994 to 1997, 47 cases with multiple fingers degloving injury were sutured by two reverse "s"-type skin flaps on abdominal flank. RESULTS: The skin flaps in 46 cases survived and the wounds obtained primary heal. CONCLUSION: The application of abdominal flank "s"-type skin flap is reliable and convenient in the treatment of multiple fingers degloving injury.

  • FOREARM FREE ARTERIALIZED VENOUS FLAP IN REPAIRING SOFT TISSUE DEFECT OF HAND

    ObjectiveTo observe the effectiveness of the forearm free arterialized venous flap in repairing soft tissue defect of the hand. MethodsBetween December 2008 and January 2013, 49 cases of soft tissue defects of the hand were treated. There were 39 males and 10 females, aged 16-52 years (mean, 34 years). Defect was caused by crush injury in 34 cases, cutting injury in 7 cases, avulsion injury in 5 cases, and hot crush injury in 3 cases. The locations were index finger in 21 cases, middle finger in 14 cases, ring finger in 10 cases, little finger in 1 case, and the first web space and the dorsal palm in 3 cases. The duration of injury and admission was 2-10 hours (mean, 4.5 hours). The size of defects ranged from 2.5 cm×1.5 cm to 6.0 cm×4.5 cm. Of them, 46 cases had fracture of metacarpal or finger bone and/or injury of tendon and nerve. Emergency operation was performed in 43 cases and selective operation in 6 cases. All defects were repaired by free arterialized venous flap from the ipsilateral forearm, in which the proximal ends of veins were anastomosed to artery and vein of the finger. The flap size ranged from 3.5 cm×2.5 cm to 7.5 cm×5.3 cm. The donor site was directly sutured. ResultsSeven flaps survived which was similar to physiological free flap. Mild or medium swelling and blister were observed in 39 flaps and heavy swelling and partial necrosis occurred in 3 flaps after operation. The patients were followed up 6 months-2 years (mean, 13.5 months). The flaps had soft texture, slightly bulky appearance, and deeper color than normal skin. At last follow-up, the two-point discrimination was 16-22 mm (mean, 20 mm). According to the standard for functional evaluation issued by Hand Surgery Association of Chinese Medical Association, the results were excellent in 21 cases, good in 21 cases, fair in 3 cases, and poor in 4 cases. ConclusionIt is an ideal method to repair soft tissue defect of the hand to use forearm free arterialized venous flap. It has the advantages of massive area, no major blood vessel needed to be sacrificed, safe and easy operation, and satisfactory appearance.

  • Clinical application of perforator-based propeller flaps in repair of hand wound

    Objective To investigate the effectiveness of the perforator-based propeller flaps (PPFs) based on digital artery (DA) and dorsal metacarpal artery (DMA) in repairing hand wounds. Methods The clinical data of 45 patients with hand wounds between January 2018 and March 2023 were retrospectively analyzed. There were 27 males and 18 females with an average age of 41.2 years (range, 14-72 years). The causes of injury included twist injury in 15 cases, crush injury in 19 cases, and cut injury in 11 cases. The injured parts included 32 cases of digits, 10 cases of dorsal hand, and 3 cases of palmar hand, all of which had tendon, joint, and bone exposure. The time from injury to operation ranged from 2 to 8 hours (mean, 4.3 hours). The wound sizes after debridement ranged from 1.8 cm×1.0 cm to 5.0 cm×3.5 cm. Twenty-eight cases were repaired by the PPFs based on DA and 17 cases were repaired by the PPFs based on DMA. The flap size ranged from 2.5 cm×1.1 cm to 8.5 cm×4.0 cm. The defects of the donor sites in 14 patients were closed directly and the defects in the left 31 patients were resurfaced with free full-thickness skin graft from the proximal medial forearm.Results All the flaps survived after operation. Two cases of the PPF based on DA and 1 case of the PPF based on DMA underwent partially blisters at the distal end and healed after dressing change. The incisions in the donor site healed by first intention and the skin grafts survived. All patients were followed up 10-33 months, with a mean of 15.4 months. At last follow-up, the static two-point discrimination of the PPFs based on DA and DMA were 4-14 mm and 8-20 mm with the averages of 8.1 mm and 13.3 mm, respectively. According to the Michigan Hand Outcomes Questionnaire, 20 patients were very satisfied with the appearance of the PPF based on DA and 8 patients were satisfied; 8 patients were very satisfied with the appearance of the PPF based on DMA and 9 patients were satisfied. Based on the Vancouver Scar Scale (VSS), the appearance scores of the donor site of the PPFs based on DA and DMA were 2-7 and 4-9, with the averages of 4.2 and 6.1, respectively. ConclusionThe two kinds of PPFs are reliable in blood supply and easy to harvest, which provide a good method for emergency repair of small and medium area wounds in the hand.
