  • 支气管扩张症的抗生素治疗

    支气管扩张症(简称支扩)是指由多种原因引起支气管壁肌肉和弹力支撑组织的破坏而引起的中等大小支气管的不正常扩张,是一种肺组织结构破坏性疾病,表现为反复呼吸道感染,大量脓痰,可出现咯血。支扩有许多相关因素及病因,总体来说,其发生可归结于以下两个因素:① 感染的持续刺激;②气道阻塞、支气管引流功能损害和防御功能的缺陷。两种因素可以同时存在,互为因果。支扩的病程一般呈进行性发展,其机制可用Cole[1] 在1984年提出的恶性循环(vicious circle)假说来解释,该假说认为感染或定植的细菌触发气道上皮的炎症反应,释放炎症介质和酶,持续的慢性炎症反应引起支气管壁和肺组织的损伤,破坏气道纤毛上皮的清洁功能,进一步加重感染和细菌定植,形成一个周而复始的恶性循环。因此,抗生素几乎是现阶段治疗支扩的主要药物之一。

  • Progress in Antimicrobial Prophylaxis for Thoracic and Cardiovascular Surgery

    Abstract: Appropriate prophylactic administration of antibiotics for thoracic and cardiovascular surgical procedure can reduce postoperative morbidity and decrease the overall cost due to infections. Prophylactic antibiotics should be given within 30 minutes preceding incision intravascularly. Serum levels of free drug above the minimal inhibitory concentration (MIC) for common contaminating bacteria should be maintained for the entire surgical period. Prophylactic antibiotics after operation are useless for patients without risk factors predisposing to postoperative infection. Heart and(or) lung transplant patients should be given antiviral and antifungal prophylaxis. Selection of antibiotics must be based on the pharmacokinetic, pharcodynamic and pharmacoeconomic properties of antibiotics and features of surgery. The policy of antibiotic prophylaxis must be modified in response to alterations in antibiotic resistance pattern which is constantly changing in hospital.

  • SIMPLE CHOLECYSTECTOMY AND USING ANTIBIOTIC IN PERIOPERATIVE PERIOD(ANALYSIS OF 642 CASES)

    目的 了解单纯胆囊切除术围手术期应用预防性抗生素的作用。方法 对我院普外一科1998年1月至1999年12月2年间所施行的642例择期单纯胆囊切除术围手术期抗生素的使用、细菌培养结果和切口感染情况进行回顾性分析。结果 642例根据其抗生素应用情况分为未用抗生素组(n=220)、术前30分钟一次性给药组(n=200)和术后3天持续用药组(n=222) 3组,3组患者之性别、年龄构成情况、全身状况及发生术后切口感染情况间的差异均无显著性意义(P>0.05); 对642例中随机选择52例行术中胆囊胆汁培养,结果47例无细菌生长。结论 对单纯胆囊切除术患者术前无感染前提下,不用或仅术前30分钟一次性使用抗生素是安全可行的,这对缩短患者住院日,减少其医疗费用具有积极的意义。

  • Analysis on the Characteristics and Drug Resistance of Ventilator-associated Pneumonia Pathogens

    目的 分析呼吸科重症监护病房(RICU)中呼吸机相关性肺炎(VAP)病原菌的来源、分布特点及耐药情况, 为在RICU中更好地控制VAP提供科学依据。 方法 回顾性分析2005年1月-2011年12月期间,医院RICU收治的行机械通气时间>48 h,住院时间>7 d的136例VAP患者病原菌分布情况和耐药性等临床资料。 结果 RICU中VAP的发生率为42.8%(136/318),病死率为47.8%(65/136)。共分离出病原菌507株,其中G−菌占74.8%(379/507),铜绿假单胞菌居首位,其次为鲍曼不动杆菌和肺炎克雷伯菌;G+球菌占10.3%(52/507),以金黄色葡萄球菌为主;真菌占14.9%(76/507),以白色念珠菌为主。G−杆菌对常用抗菌药物表现出较高的耐药率,其中对青霉素类、头孢菌素类抗菌药物高度耐药,而对碳青霉烯类耐药率相对较低;耐甲氧西林的金黄色葡萄球菌对多种常见抗菌药物高度耐药,但对万古霉素敏感。 结论 RICU的VAP病原菌以G−菌为主,耐药情况比较严重,铜绿假单胞菌、鲍曼不动杆菌和肺炎克雷伯菌为RICU中的主要致病菌。

  • Efficacy and safety of antibiotic de-escalation therapy for severe pneumonia: a meta-analysis

    Objectives To systematically review the efficacy and safety of de-escalation therapy for severe pneumonia. Methods We searched PubMed, EMbase, The Cochrane Library, CBM, CNKI, VIP and WanFang Data databases and the Chinese Clinical Trial Registry (www.chictr.org.cn) to collect randomized controlled trials (RCTs) of de-escalation therapy for patients with severe pneumonia from inception to June, 2017. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was then performed using RevMan 5.3 software. Results A total of 13 RCTs involving 1 860 patients were included. The results of meta-analysis showed that: the de-escalation therapy group was superior to the control group on clinical cure rate (RR=1.28, 95%CI 1.20 to 1.35, P<0.000 01), the total hospitalization time (MD=–6.86, 95%CI –9.12 to –4.59,P<0.000 01), remission time of complications (MD=–6.26, 95%CI –8.43 to –4.10,P<0.000 01) and mortality (RR=0.48, 95%CI 0.28 to 0.82,P=0.001). Reported cases of adverse reactions were rare, in which the degree of reactions ranged from mild to moderate. The safety was fairly satisfactory. Conclusions Current evidence shows that de-escalation therapy for patients with severe pneumonia has improved efficacy compared with conventional treatments, and can significantly shorten the total hospitalization time and reduce mortality. Due to the limited quality and quantity of the included studies, more high quality studies are required to verify above conclusions.

  • THE TREATMENT OF BACTERIAL ENDOPHTHALMITIS USING VITRECTOMY AFTER INTRAVITREAL ANTIBIOTICS AND CORTICAL STEROIDS

    PURPOSE: To investigate the treatment of severe bacterial endophthalmitis. METHODS:The curative effects of vitrectomy after intravitreal antibiotics and steroids (IVAS)for the treatment of 23 patients with bacterial endophthalmitis (group I)and vitrectomy and IVA at the same time for the treatment of 28 patients with bacterial endopbthalmitis (group I)were analyzed retrospectively. RESULTS: The rate of curative effects of two groups were similar,while the marked curative effects in group I (47.8% )was significantly higher than that of the group I (17.9%). The average period of eliminating infiamation of group I was longer than that of group I , and the incidence of postoperative retinal detachment of group Ⅱ was 3 times more than that of group I . CONCLUSION :It was indicated that vitrectomy after IVAS may increase the security of vitrectomy and the curative effects of severe bacterial ndophthalmitis.

  • Analysis of Bionomics and Antimicrobial Susceptibility in 102 Staphylococcus Aureus

    摘要:目的: 金黄色葡萄球菌(金葡菌)的感染近年来已成为医院内的主要致病菌,而其耐药性也呈逐渐升高的趋势,为了解该菌在我院的感染和耐药情况,为临床合理使用抗生素提供科学依据。 方法 : 用经典生理生化鉴定方法,对各种临床标本主要来源于痰液和各种伤口脓液标本分离到的102株金葡菌进行生物学特性及药敏试验。 结果 : 从我们医院2007年5月至2009年8月所分离出来的102株金葡菌中青霉素耐药性8923%,氨苄青霉素耐药率为9385%,没有发现万古霉素耐药菌。 结论 : 除万古霉素外,耐药率较低的依次是利福平、苯唑青霉素、环丙沙星、呋喃妥因、阿米卡星、磺胺甲基异恶唑、红霉素,而青霉素G、氨苄青霉素、四环素耐药性情况非常严重,并且多重耐药,耐药性强,应引起临床的高度重视。Abstract: Objective: To analyze the bionomics and antimicrobial susceptibility of staphylococcus aureus, which was the main pathogenic bacterium with high drug tolerance in our hospital, in order to provide the rational use of antibiotics. Methods : Samples of one hundred and two staphylococcus aureus cases from sputamentum and pus were evaluated by classic physiology and biochemistry methods to test the bionomics and antimicrobial susceptibility. Results : The drug resistance rate to penicillin, penbritin and vancomycin was 8923%, 9385% and 0, separately. Conclusion : Besides vancomycin, the drug resistance rate of rifampicin, oxazocilline, ciprofloxacin, furadantin, amikacin, sulfamethoxazole and sulfamethoxazole increased one by one. The resistance to penicillin G, penbritin and tetracycline was serious, including multidrug resistant, which should be paid highly attention.
