  • Vitreoretinal surgery using the 25G transconjunctival sutureless vitrectomy system under topical anesthesia

    Objective To evaluate the indications, effectiveness and complications of vitreoretinal surgery using the 25G transconjunctival sutureless vitrectomy system (TSV25G) under the topical anesthesia. Methods The clinical and follow-up data of 22 eyes of 22 patients undergone vitreo-retinal surgery using TSV25G under the topical anesthesia were retrospectively analyzed. All of the patients were monocular sickened, including idiopathic macular hole in 10 eyes, idiopathic macular pucker in 6, vitreoretinal traction syndrome in 4, and vitreous hemorrhage associated with branch retinal vein occlusion in 2. Peeling of epiretinal membrane and/or internal limiting membrane, intra ocular laser coagulation, air-fluid exchange and tamponiding of C3F8 were performed according to the condition of diseases. The postoperative follow-up was 1-11 months, with the mean duration of 6.4 months. The effect of analgesia, cooperation with the patients, operative effect and complications in and after the surgery were observed. Results The operations finished successfully in all of the eyes under the topical anesthesia. The operation duration ranged from 20 to 25 minutes with average of 22 minutes. The patients cooperated with the doctor well without any discomfort. Two days after the surgery, edema of the wounded conjunctiva was found, and recovered 7 days later. A light pigment dot on the surface of the sclera could be seen at the first month. The complic ations included transient increasing of intraocular pressure in 2 eyes, feather-like opacity of lens in 5 eyes, vitreous hemorrhage in 1 eye, and air-bleb under conjunctiva in 2 eyes. No other complications related with the cut were fo und. The macular hole closed in 9 eyes with idiopathic macular hole, and the other 1 had the smaller but not closed hole. Idiopathic macular pucker, vitreoretinal traction syndrome, and vitreous hemorrhage associated with branch retinal vein occlusion were cured successfully. Conclusions Vitreoretinal surgery using the TSV25G under the topical anesthesia has many advantages such as simple procedure, short operation time, micro-invasion, less complications and rapid revovery, and mainly serves simple manipulation in some simple diseases such as idiopathic macular hole, vitreo-retinal traction syndrome, and simple hemorrhage.   • Epilepsy detection and analysis method for specific patient based on data augmentation and deep learning

    In recent years, epileptic seizure detection based on electroencephalogram (EEG) has attracted the widespread attention of the academic. However, it is difficult to collect data from epileptic seizure, and it is easy to cause over fitting phenomenon under the condition of few training data. In order to solve this problem, this paper took the CHB-MIT epilepsy EEG dataset from Boston Children's Hospital as the research object, and applied wavelet transform for data augmentation by setting different wavelet transform scale factors. In addition, by combining deep learning, ensemble learning, transfer learning and other methods, an epilepsy detection method with high accuracy for specific epilepsy patients was proposed under the condition of insufficient learning samples. In test, the wavelet transform scale factors 2, 4 and 8 were set for experimental comparison and verification. When the wavelet scale factor was 8, the average accuracy, average sensitivity and average specificity was 95.47%, 93.89% and 96.48%, respectively. Through comparative experiments with recent relevant literatures, the advantages of the proposed method were verified. Our results might provide reference for the clinical application of epilepsy detection.

  • Evaluation of Quality of Chinese Literature on D-dimer Diagnostic Methodology

    Objective To evaluate the quality of Chinese literatures on the methodology of D-dimer diagnostic test. Method We searched CNKI (1994 to 2006) and CBM (1978 to 2006) for articles involving the diagnostic tests of D-dimer for coagulation disorders. Result A total of 63 relevant articles were retrieved and 7 were included in our review. Only one of these provided useful data on two two table for the evaluation of diagnostic accuracy. Conclusions Few studies on the diagnostic tests of D-dimer have been performed and publ ished in China, all of poor quality. Further studies should focus on clinical diagnostic sensitivity and specificity, so as to provide more valuable information for readers.

  • New Method Applied to Dissection of Myocardial Band

    Abstract: Objective To solve the problem that myocardial band of stale hearts treated by formaldehyde cann’t be used for research study. Methods Twenty sheep’s hearts and 20 pig’s hearts were randomly distributed to 3 groups, group 1: fresh pig’s hearts(n=10) and fresh sheep’s hearts (n=10) were dissected by Torrent-Guasp’s method; group 2: pig’s hearts (n=5) and sheep’s hearts (n=5) treated by formaldehyde were dissected by Torrent-Guasp’s method; group 3: pig’s hearts (n=5) and sheep’s hearts (n=5) treated by formaldehyde were dissected by new method. Results All samples in group 1 were successfully unrolled to a myocardial band composed of basal loop and apical loop. The dissecting of samples in group2 were difficult. The root of pulmonary artery and ascending aorta failed to be unfolded because fibrous tissue was tough, right and left fibrous trigone were too firm to be solved by hand. Cardiac muscle fibers couldn’t be stripped along myofibrillar trajectory since they were prone to break because of their friability. On the other hand, being dissected along nature myofibrillar trajectory, all samples in group 3 were successfully unrolled to a myocardial band composed of basal loop and apical loop. Furthermore, myofibrillar trajectory was clear and the exposure of crossing angle between apical ascending segment and descending segment was ideal. Conclusion With Torrent-Guasp’s method, the dissection of myocardial band of hearts was successful in fresh heart, but not in stale heart. On the other hand, with new method, the dissection of myocardial band of hearts was achieved in stale heart group. Our method in this research is a reliable way on dissection of myocardial band in stale hearts treated by formaldehyde.

  • Clinical application of 25G transconjunctival sutureless vitrectomy system

    Objective To evaluate the practicability and safety of 25-gauge (25G) transconjunctival sutureless vitrectomy system (TSV25G) and describe the preliminary experience. Methods Eighteen patients underwent TSV25G. The time of procedures of setting-up the three-port cannulae and closing the cuts were recorded. The pre- and post-operative ocular tension, visual acuity and complications were observed. Results The average time of setting-up the three-port cannulae and closing the cuts was 1 minutes 24 seconds and 32 seconds, respectively. The average preoperative ocular tension was 16.3 mm Hg(1 mm Hg=0.133 kPa); the average postoperative ocular tension at the first day, first week, and first month was 13.0, 15.9, and 16.4 mm Hg, respectively. The visual acuity before and one month after operation the was HM/ 20 cm~0.2 and HM/50 cm~0.6, respectively. No postoperative complication was found. Conclusion TSV25G may simplify the operation, minimize the surgical induced trauma, and decrease the operating time and the postoperative inflammatory response.   • Introduction to Systematic Reviews of Animal Studies

    New interventions are rapidly progressing into clinical practice through the preclinical study stage. The animal experiments should be comprehensive and critically evaluated for their pivotal role in this process. To integrate the evidence of animal studies by systematic review throws light on intervention outcomes and could decrease risk of participants in human trials. The methodological weakness of animal studies is easy to highlight but robust methods to synthesize evidence is essential. Here, we discuss some problems in systematic review of animal experiments and whether animal models of diverse species contribute to bias of meta-analysis conclusions.

  • An assessment of methodological quality of multi-center randomized controlled trials of stroke treatments conducted in Chinese Mainland

    ObjectiveTo evaluate the current status and trend of methodological quality of multi-center randomized controlled trials (RCTs) of stroke treatments in Chinese Mainland.MethodsMulti-center RCTs of stroke treatments conducted in Chinese Mainland published in Chinese or English language from January 2000 to December 2019 were retrieved from seven databases including PubMed, Cochrane Central Registry of Controlled Trials, Embase, China Biology Medicine, China National Knowledge Infrastructure, Chinese Science and Technique Journals Database, and Wanfang Database. The basic information was collected. Methodological items were referred to the Cochrane Collaboration’s tool for assessing risk of bias. The definitions of Wade were used to assess the outcome measure.ResultsA total of 90 multi-center RCTs were included, of which 39 were published from 2000 to 2009, and 51 were published from 2010 to 2019. The total number of trials published from 2010 to 2019 was 1.31 times of that published from 2000 to 2009. The research subjects were ischemic stroke patients in 58.9% (53/90) of the RCTs, intracerebral hemorrhage patients in 14.4% (13/90) of the RCTs, and ischemic stroke patients as well as hemorrhagic stroke patients in 26.7% (24/90) of the RCTs. There were 55.6% (50/90) drug trials, and 44.4% (40/90) non-drug trials. There were statistically significant differences in the loss of visit report (P=0.005), primary and secondary outcome indicators report (P=0.027), and adverse reaction report (P=0.007) between the two periods; there was no statistically significant difference in reported adequate randomized methods (P=0.341), allocation concealment (P=0.611), blindness (P=0.551), used intentionality analysis (P=0.573), or follow-up time (P=0.061) between the two periods.ConclusionIn the past 20 years in Chinese Mainland, the quality of stroke treatment RCTs improves slowly, and more attention should be paid to develop the RCTs of true randomization, blinding, and better patient outcome measures.

  • Understanding the pathogenesis of various type in exudative AMD, using the rational therapy

    Up-regulation of vascular endothelial growth factor (VEGF) is demonstrated to be a key role in formation process of intraocular neovascularization. Anti-VEGF treatment is the breakthrough of intraocular neovascular diseases therapy. Intrav itreal injection of antineovascularization drug looks to be an effective method on ocular neovascular diseases which with the advantages of good biocompatibility, low prices and longer intravitreal half-time etc. However, at present, it lack of multi-center study; the long-term efficacy and the systematic safety needs the further clinical verification. Various types of CNV showed the different therapeutic reactions to either PDT or Anti-VEGF agent, the treatment methods for exudative AMD include laser, PDT, and drug like Triamcinolone Acetonide,several anti-VEGF preparations. Therefore, understanding the pathogenesis of neovascular AMD and choosing a reasonable therapeutic methods are necessary. We should try to explore a safe, effective, economic, new approach.
