  • Research on pulmonary nodule recognition algorithm based on micro-variation amplification

    Objective To develop an innovative recognition algorithm that aids physicians in the identification of pulmonary nodules. MethodsPatients with pulmonary nodules who underwent thoracoscopic surgery at the Department of Thoracic Surgery, Affiliated Drum Tower Hospital of Nanjing University Medical School in December 2023, were enrolled in the study. Chest surface exploration data were collected at a rate of 60 frames per second and a resolution of 1 920×1 080. Frame images were saved at regular intervals for subsequent block processing. An algorithm database for lung nodule recognition was developed using the collected data. ResultsA total of 16 patients were enrolled, including 9 males and 7 females, with an average age of (54.9±14.9) years. In the optimized multi-topology convolutional network model, the test results demonstrated an accuracy rate of 94.39% for recognition tasks. Furthermore, the integration of micro-variation amplification technology into the convolutional network model enhanced the accuracy of lung nodule identification to 96.90%. A comprehensive evaluation of the performance of these two models yielded an overall recognition accuracy of 95.59%. Based on these findings, we conclude that the proposed network model is well-suited for the task of lung nodule recognition, with the convolutional network incorporating micro-variation amplification technology exhibiting superior accuracy. Conclusion Compared to traditional methods, our proposed technique significantly enhances the accuracy of lung nodule identification and localization, aiding surgeons in locating lung nodules during thoracoscopic surgery.

  • Preoperative drainage and complication prevention strategies for jaundice caused by extrahepatic cholangiocarcinoma

    Partial or complete blockage of the bile outflow tract by extrahepatic cholangiocarcinoma often leads to jaundice, which not only causes skin itching in patients, but also destroys the body environment through a series of pathophysiological processes, reduces the surgical tolerance of patients with resectable tumors, and affects the prognosis. Preoperative jaundice reduction can reduce jaundice and relieve biliary obstruction, but the various complications that follow will also adversely affect the treatment. This article elaborates on the disadvantages of different methods for jaundice reduction, the indications for preoperative jaundice reduction, the jaundice reduction options for different types of extrahepatic cholangiocarcinoma, the complications and treatment methods of preoperative jaundice reduction for extrahepatic cholangiocarcinoma, aiming to provide a reference for clinicians, so that patients can better benefit from preoperative jaundice reduction.

  • Preoperative Assessment of Vascular Invasion in Pancreatic CancerValue of CT

    Objective To probe CT grading criteria of vascular invasion in pancreatic cancer. Methods Retrieved articles in CNKI and PubMed about value of CT in preoperative assessment of vascular invasion in pancreatic cancer last ten years. Results Multislice helical CT is considered the best imaging method to assess the invaded peripancreatic vessels in pancreatic cancer. There are different CT criteria of vascular invasion in pancreatic cancer based on extension of hypodense tumor and its relation to blood vessels, on the degree of circumferential contiguity of tumor to vessel, on the degree of lumen stenosis, and on the degree of contiguity between tumor and vessels combined vascular caliber. Conclusion CT grading criteria are not uniform, each one has defects.

  • 加速康复外科护理在喉癌患者术前准备中的应用

    目的 观察加速康复外科(FTS)护理在喉癌患者术前准备中的有效性。 方法 对2011年3月-7月实施全喉切除术加颈淋巴结清扫术的30例喉癌患者(A组),按FTS要求,通过采用制定详细的健康教育计划及缩短术前禁食、禁饮时间等方法予以护理,并与2010年同期采取传统护理方式的30例患者(B组)资料进行比较,评价两组患者术后依从性、胰岛素抵抗发生率及住院时间。 结果 两种护理方式相比,A组患者术后依从性为85%,较B组的52%高;A、B两组术后胰岛素抵抗发生率分别为3.3%、13.3%,差异有统计学意义(P<0.05);A组平均住院日为(11.47 ± 1.25)d,B组为(14.3 ± 1.36)d,两组差异有统计学意义(P<0.05)。 结论 按FTS理念护理,安全有效,对加速患者术后康复有较好的积极作用。

  • 核磁共振图像后处理技术在癫痫术前评估中的应用

    在临床工作中,约有30%的患者为难治性癫痫患者。手术治疗有时可能是难治性癫痫最后的治疗方案。然而对于核磁共振(MRI)阴性的癫痫患者,手术治疗有时会十分困难,这类患者术后癫痫无发作率也明显低于MRI有病灶的患者。应用全脑MRI图像后处理技术,对于发现微小病灶有巨大帮助。现就MRI后处理技术在难治性局灶性癫痫术前评估中的应用进行综述,旨在提高临床医师对该病的认识并促进其运用。

  • 地塞米松联合用药方案在原发性甲状腺功能亢进患者术前准备中的临床实践经验

    目的探讨地塞米松联合用药方案在原发性甲状腺功能亢进症(简称 “甲亢” )患者术前准备中应用的效果。方法回顾性收集郑州大学第一附属医院甲状腺外科2021年1月1日至2022年10月1日期间收治的原发性甲亢手术患者。采用地塞米松联合碘剂和β-受体阻滞剂联合应用的7 d术前准备方案(简称 “7 d方案” ):连续口服7 d复方碘溶液(3次/d,0.75 mL/次),在口服复方碘溶液的第5、6、7天时同时加用地塞米松注射液缓慢静脉滴注(1次/d,20 mg/次,上午10∶00开始,每次输入间隔时间24 h),手术前1 d口服β-受体阻滞剂普萘洛尔(剂量因人而异),于术前准备完成后第2天(第8天时)手术。观察入院时及使用地塞米松后的第1、2、3、4、5天时血清促甲状腺激素、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)及游离甲状腺素(free thyroxine,FT4)水平的变化情况以及并发症情况。结果本研究共收集到58例患者,使用地塞米松后第1、2、3、4、5天时的FT3及FT4水平总体一直呈持续下降趋势(F=88.355,P<0.001;F=21.291,P<0.001),并且使用地塞米松后第2天开始FT3均值一直维持在正常水平,FT4均值虽未完全达到正常水平但比较接近正常水平;促甲状腺激素水平总体比较差异无统计学意义(χ2=1.607,P=0.900)。术后未发生甲状腺危象。结论从本研究结果看,“7 d方案”进行术前准备,较传统术前准备方法缩短了术前准备时间,对原发性甲亢的围术期管理安全、有效。

  • Artificial intelligence assisted total hip arthroplasty for patients with Crowe type Ⅳ developmental dysplasia of the hip

    ObjectiveTo investigate the early effectiveness of artificial intelligence (AI) assisted total hip arthroplasty (THA) system (AIHIP) in the treatment of patients with Crowe type Ⅳ developmental dysplasia of the hip (DDH).MethodsThe clinical data of 23 patients with Crowe type Ⅳ DDH who met the selection criteria between May 2019 and December 2020 were retrospectively analyzed. There were 3 males and 20 females, the age ranged from 44 to 74 years, with an average of 52.65 years. The absolute value of the lower limbs discrepancy before operation was (15.17±22.17) mm. The preoperative Harris score was 62.4±7.2. The AIHIP system was used for preoperative planning, and the operations were all performed via conventional posterolateral approach. Thirteen patients with difficulty in reduction during operation underwent subtrochanteric shortening osteotomy (SSOT). The operation time, hospital stay, and adverse events were recorded; Harris score was used to evaluate the function of the affected limb at 1 day before operation and 1 week and 6 months after operation; pelvic anteroposterior X-ray film was taken at 1 day after operation to evaluate the position of the prosthesis. The matching degree of prosthesis was evaluated according to the consistency of intraoperative prosthesis model and preoperative planning.ResultsThe matching degree of acetabular cup model after operation was 16 cases of perfect matching, 4 cases of general matching (1 case of +1, 3 cases of –1), and 3 cases of mismatch (all of them were +2), the coincidence rate was 86.96%. The matching degree of femoral stem model was perfect matching in 22 cases and general matching in 1 case of –1, and the coincidence rate was 100%. One patient had a periprosthesis fracture during operation, and was fixed with a wire cable during operation, and walked with the assistance of walking aid at 6 weeks after operation; the rest of the patients walked with the assistance of walking aid at 1 day after operation. The operation time was 185-315 minutes, with an average of 239.43 minutes; the hospital stay was 8-20 days, with an average of 9.96 days; and the time of disengagement from the walking aid was 2-56 days, with an average of 5.09 days. All patients were followed up 6 months. All incisions healed by first intension, and there was no complication such as infection, dislocation, refracture, and lower extremity deep venous thrombosis; X-ray films at 1 day and 6 months after operation showed that the acetabular and femoral prostheses were firmly fixed and within the safe zone; the absolute value of lower limbs discrepancy at 1 day after operation was (11.96±13.48) mm, which was not significantly decreased compared with that before operation (t=0.582, P=0.564). All osteotomies healed at 6 months after operation. The Harris scores at 1 week and 6 months after operation were 69.5±4.9 and 79.2±5.7 respectively, showing significant differences between pre- and post-operation (P<0.05). At 6 months after operation, the hip function was evaluated according to Harris score, and 13 cases were good, 9 cases were fair, and 1 case was poor.ConclusionAIHIP system-assisted THA (difficult to reposition patients combined with SSOT) for adult Crowe type Ⅳ DDH has high preoperative planning accuracy, easy intraoperative reduction, early postoperative landing, and satisfactory short-term effectiveness.

  • Randomized Controlled Trial of Preoperatively Combinative Assessment of Upper Rectal Cancer in Prediction to Operative Strategies

    Objective To determine the influence of combinative assessment of 64 multi-slice spiral computer tomography (MSCT) and serum amyloid A protein (SAA) on the selection of operative procedures of upper rectal cancer in multi-disciplinary team. Methods Prospectively enrolled 110 patients, who were diagnosed definitely as upper rectal cancer (distance of tumor to the dentate line gt;7 cm) at West China Hospital of Sichuan University from August 2007 to October 2008, randomly assigned into two groups. In one group named MSCT+SAA group, both MSCT and SAA combinative assessment were made for the preoperative evaluation. In another group named MSCT group, only MSCT was made preoperatively. Then, the pooled data were analyzed for the correlative relationship between the choice of surgery strategy and clinicopathologic factors. Furthermore, the preoperative staging and predicted operative procedures were compared with postoperative pathologic staging and practical operative procedures, respectively. Results According to the criteria, 106 patients with upper rectal cancer were randomly assigned into MSCT+SAA group (n=52) and MSCT group (n=54). The baseline characteristics of two groups were statistically identical. When analyzing the proportion of multiple clinicopathologic factors in different operative procedures of upper rectal cancer, there were statistical differences in the preoperative N staging (P=0.003), M staging (P=0.022), TNM staging (P=0.003), serum level of SAA (P=0.005) and general category of tumor (P=0.027). For MSCT+SAA group the accuracies of preoperative staging T, N, M and TNM were 84.6%, 86.5%, 100% and 86.5%, respectively; For MSCT group the corresponding rates were 83.3%, 2.9%, 100% and 64.8%, respectively. There were statistically significant differences accuracies of preoperative N staging and TNM staging (P=0.005, P=0.009, respectively) in two groups. There was a statistically significant difference of the accuracy of prediction to operative procedures in two groups (96.2% vs. 81.5%, P=0.017). Conclusion Combinative assessment of 64 MSCT and SAA could improve the accuracy of preoperative staging, and thus provide higher predictive coincidence rate to operative procedures for surgeon.
