  • RESEARCH PROGRESS OF CELLS AND CELL-TRANSPLANTATION METHODS FOR PERIODONTAL TISSUE ENGINEERING

Objective To introduce the cells and cell-transplantation methods for periodontal tissue engineering. Methods Recent l iterature about appl ication of cell-based therapy in periodontal tissue engineering was extensively reviewed, the cells and cell-transplantation methods were investigated. Results Mesenchymal stem cells were important cell resourcesfor periodontal tissue engineering, among which peridontal l igament stem cells were preferred. Bone marrow mesenchymal stem cells had several disadvantages in cl inical appl ication, and adipose-derived stem cells might be a promising alternative; different transplantation methods could all promote periodontal regeneration to some extent. Single-cell suspension injection could only promote a l ittle gingival regeneration, and tissue engineered scaffolds still needed some improvement to be used in periodontal regeneration, while cell sheet technique, with great cell loading abil ity and no need of scaffolds, could promote regeneration of cementum, periodontal l igament, and alveolar bone under different conditions. Conclusion Multipotent stem cells are fit to be used in periodontal tissue engineering; improvement of cell-transplantation methods will further promote periodontal regeneration.

  • Effectiveness of Intranasal Lidocaine Spray before Nasogastric Tube Insertion: A Meta-analysis

ObjectiveTo systematically review the efficacy and safety of intranasal lidocaine spray before nasogastric tube insertion. MethodsWe searched PubMed, EMbase, The Cochrane Library, WanFang Data, VIP, CBM and CNKI databases concerning randomized controlled trial (RCT) of the efficacy and safety of intranasal lidocaine spray before nasogastric tube insertion from their inception to January 2014. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, and assessed methodological quality of included studies. Meta-analysis was then conducted using RevMan 5.2 software. ResultsSix RCTs involving 384 patients were included. The results of meta-analysis showed that there were no significant differences between the lidocaine group and the saline group in pain and discomfort scores (MD=-25.35, 95%CI -30.37 to -24.33) and first successful insertion rate (RR=1.38, 95%CI 1.21 to 1.57). ConclusionIntranasal lidocaine spray before nasogastric tube insertion could reduce patient pain and discomforts related to the procedure, and improve the first successful insertion rate.

  • Optical coherence tomography distribution patterns of diabetic macular edema and its correlations with diabetic retinopathy stages and systemic factors

Objective To investigate the distribution patterns of diabetic macular edema (DME) based on optical coherence tomography (OCT), and explore its correlation with diabetic retinopathy (DR) stages and systemic factors. Methods A total of 135 patients (242 eyes) with type 2 diabetes were included in this retrospective study. There were 75 males (138 eyes) and 60 females (104 eyes), the ages were from 29 to 83 years, with an average age of (58.8±11.1) years. The general information such as height, weight, smoking history and blood glucose [such as glycosylated hemoglobin (HbA1c)], blood pressure, blood lipid, 24 hours urine protein and other examinations were collected. The diagnosis of DR and DME were made, and the staging of DR and typing of DME were performed based on fundus color imaging and OCT. DR were divided into mild non-proliferative DR (NPDR), moderate NPDR, severe NPDR and proliferative DR (PDR). DME were categorized into 4 types including sponge-like retinal swelling (SME), cystoid macular edema (CME), serous retinal detachment (SRD) and posterior hyaloid traction (PHT). The correlation between DME types and DR staging were analyzed byχ2 test and Fisher exact test. Multivariate logistic regression analysis was used to analyze the correlation between DME types and systemic factors. Results In 242 DR eyes the proportions of mild, moderate, severe NPDR and PDR were 30.99%, 32.64%, 23.14% and 13.23%, respectively. There were 199 eyes (82.23%) with DME. There were statistically significant differences in the proportion of DME in different stages of DR (χ2=21.077,P<0.01). In the 199 eyes with DME, There were 165 eyes (68.18%) of SME, 22 eyes (9.09%) of CME, 7 eyes (2.89%) of SRD and 5 eyes (2.07%) of PHT. The distribution of DME patterns in different stages of DR was statistically significant (χ2=156.273,P<0.01). Logistic regression analysis showed that the duration of diabetes, HbA1c and macroalbuminuria were independent risk factors for DME [odds ratio (OR)=1.090, 1.510, 4.123;P<0.05], and were also independent for SME (OR=1.092, 1.445, 3.942;P<0.05); HbA1c was an independent risk factor for SRD (OR=2.337,P<0.05). Conclusions There are differences in the distribution of different DME types in each stage of DR. The duration of diabetes, HbA1c and macroalbuminuria were independent risk factors for DME and SME, and macroalbuminuria and HbA1c for CME and SRD.

  • Trend of disease burden and incidence prediction of type 2 diabetes nephropathy in China from 1990 to 2021

ObjectiveTo analyze the epidemiological trends and current status of disease burden of type 2 diabetes related chronic kidney disease (CKD-T2D) in China from 1990 to 2021 and predict its future trends. MethodsThis study was based on the global burden of disease 2021 (GBD 2021) database to obtain the disease burden data of CKD-T2D in China, including the data of incidence, mortality and disability-adjusted life years (DALY), and used the Bayesian age-period-cohort (BAPC) model to predict the change trends until 2035. ResultsFrom 1990 to 2021, the standardized incidence rate of CKD-T2D in China showed an overall upward trend, while the standardized mortality rate and DALY rate showed an overall downward trend, with the average annual percentage change (AAPC) being 0.04%, −0.04%, −1.11% (P<0.05). Moreover, CKD-T2D in the whole country presented the characteristics of high incidence, high mortality and high DALY burden in the elderly population. From 2022 to 2035, all disease burden indicators of CKD-T2D in China are expected to show an overall downward trend. It is expected that the standardized incidence rate, mortality rate and DALY rate will drop to 16.21/100 000, 4.62/100 000 and 99.35/100 000 respectively in 2035. The overall decline of each indicator in the female population in China is greater than that in the male population. ConclusionThe prevention and treatment situation of CKD-T2D in China is severe, the burden caused by disability is relatively heavy and the burden of male and elderly populations is particularly heavy.
