  • The methodological assessment of cross-sectional surveys about Chinese medicine syndrome in a population at potential risk of cerebrovascular diseases

ObjectiveTo evaluate the methodological quality of cross-sectional surveys about Chinese medicine syndrome in a population at potential risk of cerebrovascular diseases. Methods The CNKI, WanFang Data, CBM and PubMed databases were electronically searched to collect cross-sectional surveys about Chinese medicine syndromes in a population at potential risk of cerebrovascular diseases from inception to December, 2022. The methodological quality was assessed using the JBI scale. Results A total of 105 studies were included. The average reporting rate of JBI was 52.06%, and the items with the highest scores included "sufficient coverage of the identified sample in data analysis" (100%), "description of study subjects and setting" (92.38%), and "using valid methods for the identification of the condition" (86.67%). Items with the lowest scores included "adequate sample size" (13.33%), "adequate response rate or low response rate managed appropriately" (14.29%), and "study participants recruited in an appropriate way" (20.95%). Subgroup analysis suggested that type of publication and number of implementation centers were potential factors influencing methodology quality (P<0.05). Conclusion The methods essential to a cross-sectional survey such as sampling, sample size calculation and handling with the response rate, and the syndrome diagnosis scales specific to Chinese medicine require further improvement.

  • Arteriosclerosis in patients with hypertension defined by AHA and classical diagnostic criteria: a cross-sectional study

ObjectiveTo evaluate the level of arteriosclerosis in patients with hypertension defined by the American Heart Association (AHA) and classical diagnostic criteria. MethodsA total of 3 815 residents were enrolled in 10 communities in north Shanghai. According to the classic diagnostic criteria of hypertension (systolic blood pressure≥140 mmHg and/or diastolic blood pressure≥90 mmHg) and AHA diagnostic criteria (systolic blood pressure≥130 mmHg and/or diastolic blood pressure≥80 mmHg), the population was divided into normal blood pressure group, AHA diagnosis standard hypertension group, and classic methods of diagnosis of hypertension group. The differences in cervical-femoral pulse wave velocity (cf-PWV) and brachial-ankle pulse wave velocity (ba-PWV) among the three groups were compared. SPSS 13.0 software was then used for data analysis.ResultsCompared with the patients who met the standard criteria, patients who met AHA criteria had lower mean ages (70.2±7.4 vs. 71.4±7.9 year, P<0.001), more history of hypertension (48.8% vs. 72.7%, P<0.001) and lower body mass index (24.1±3.5 vs. 24.7±3.9 kg/m2, P<0.001), low-density lipoprotein (3.07±0.92 vs. 3.15±0.97 mmol/L, P=0.033), cf-PWV (8.7±2.7 vs. 9.8±3.0 m/s, P<0.001) and ba-PWV (1 647.7±610.1 vs. 1 797.2±729.7 cm/s, P<0.001). ConclusionsThe degree of arteriosclerosis of patients who meet AHA standards is between that who meet the standard criteria and the normal population. For these patients, blood pressure should be actively controlled to delay the progression of arteriosclerosis.

  • Risk Factors for Hypertension and Diabetes among Floating Population in Hubei Province: A Cross-sectional Survey

ObjectiveTo explore the prevalence and risk factors of hypertension and diabetes in floating population in Hubei province, so as to provide reference for the prevention of chronic disease of floating population. MethodA multi-stages sampling was performed among six counties in Hubei province in 2012, according to the different occupations. A questionnaire survey was conducted to collect the demography, sociology and health-related behavioral characteristics of subjects. Then a univariate analysis and a multivariate analysis were conducted by SPSS 20.0 software. ResultsA total of 1 800 individuals were surveyed, and the prevalence of hypertension and diabetes in floating population was 22.7% and 4.9%, respectively. Univariate analysis indicated that significant associations were found between hypertension and such factors as age, occupations, smoking and life satisfaction (all P values < 0.05); and significant associations were found between diabetes and such factors as occupations, educational level and BMI (all P values < 0.05). The results of further logistic regression analysis showed that age (OR=2.194, 95% CI 1.940 to 2.483, P=0.001) and life satisfaction (OR=0.291, 95% CI 0.248 to 0.341, P=0.002) were independent risk factors of hypertension, and educational level (OR=3.219, 95%CI 2.016 to 7.565, P=0.011) and gender (OR=0.568, 95% CI 0.323 to 0.999, P=0.049) were independent risk factors of diabetes. ConclusionsThe prevalence of hypertension and diabetes are relative high in floating population of Hubei Province. Increasing age and low life satisfaction are independent risk factors of hypertension in floating population, while male and low educational level are independent risks factors of diabetes.

  • Prognosis of fetal isolated hyper echogenic kidneys on prenatal ultrasound examination: a meta-analysis

ObjectiveTo systematically review the prognosis of fetal isolated hyper echogenic kidneys (IHEK) on prenatal ultrasound examination. MethodsPubMed, EMbase, Web of Science, WanFang Data, and CNKI databases were electronically searched to collect cross-sectional studies on the prognosis of fetal IHEK on prenatal ultrasound examination from January 1990 to January 2021. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies; then, meta-analysis was performed by using R 3.5.2 software. ResultsA total of 9 cross-sectional studies involving 348 fetuses were included. The results of meta-analysis showed that the incidence rate of live births was 79% (95%CI 69% to 88%), termination of pregnancy/neonatal mortality (TOP/NND) was 30% (95%CI 15% to 45%), normal kidneys was 34% (95%CI 15% to 53%), autosomal recessive polycystic kidney disease (ARPKD) was 21% (95%CI 12% to 30%), autosomal dominant polycystic kidney disease (ADPKD) was 13% (95%CI 5% to 21%), and multicystic dysplastic kidney (MCDK) was 4% (95%CI 2% to 7%). Subgroup analysis showed that the prognosis of normal amniotic fluid subgroup was significantly superior to that of reduced amniotic fluid subgroup. ConclusionCurrent evidence shows that the incidence of adverse pregnancy outcomes in patients with IHEK on prenatal ultrasound examination is high, the prognosis is superior when IHEK with normal amniotic fluid volume, and is worse when with small amniotic fluid volume. Due to limited quality and quantity of the included studies, more high-quality studies are required to verify the above conclusion.

Methods In 13 provinces or municipalities, 104 medical institutions, including tertiary, secondary and primacy medical institutions, were investigated by questionnaires. Kinds of drugs, dosage forms, requirement types and requirement levels of drugs were analyzed and compared in different levels of medical institutions. Results Tertiary medical institutions had higher demands than other levels of medical institutions for the number of drugs types, dosage forms and level of demand on each drug (P < 0.05), secondary medical institutions were higher than primacy medical institutions (P < 0.05), but community health service centers were the same as township health centers. Conclusion The demands of dosage forms and specifications of essential medicine for children in different levels of medical institutions are different. So future studies should develop the essential medicines list for children depend on different levels of care.
