  • Treatment outcomes of reduced-dose intravitreal ganciclovir for acquired immunodeficiency syndrome patients with cytomegalovirus retinitis

    ObjectiveTo evaluate the efficacy and safety of reduced-dose intravitreal ganciclovir for the treatment of acquired immunodeficiency syndrome (AIDS) patients with cytomegalovirus retinitis (CMVR).MethodsA prospective observational cohort study observed 15 AIDS patients (28 eyes) who suffered from CMVR onset between January 2016 and December 2018 at Nanning Aier Eye Hospital. Among this 28 eyes, BCVA of 6 eyes (21.4%) were between moving hand to counting finger, 15 eyes (53.6%) were between 0.02 to 0.1 and 7 eyes were better than 0.1 (25.0%). All eyes received intravitreal injection 0.1 ml of ganciclovir at 4 mg/ml (contain ganciclovir 0.4 mg). The induction regimen was twice weekly for 2 weeks and a maintenance period of the same dose weekly. The mean number of injections was 7.1±1.7 times. For hospitalized patients who had no contraindicated received a 14-day twice daily intravenous ganciclovir (IVG) 5.0 mg/kg·d until complete resolution of CMVR. All patients were divided into intravitreal ganciclovir (IVTG) group and IVTG+IVG group according to different treatment plans, which were 5 cases with 8 eyes and 10 cases with 20 eyes, respectively. The follow-up was more than 6 months. BCVA, complete resolution or stable of the lesion and complications were observed.ResultsSix months later, 20 eyes (71.4%) had a obvious reduced or disappeared of the anterior chamber and vitreous inflammation, and the retinal lesions became stable or complete resolution. 24 eyes showed improvements of BCVA and 4 eyes showed stable. 2 eyes (7.1%) presented with BCVA ≤ counting finger, 7 eyes (25.0%) were 0.02 - 0.1 and 19 eyes were ≥ 0.1 (67.9%). Compared with before treatment, the ratio of BCVA that less than or equal to counting finger and between 0.02 to 0.1 decreased (21.4% vs 7.1% and 53.6% vs 25.0%, respectively), but the ratio of BCVA better than 0.1 increased (25.0% vs 67.9%). When IVTG+IVG group was compared with IVTG group, the average time-to-resolution of CMVR were 83.2±25.2 and 85.3±24.4 days respectively. There was no significant difference in resolution times (Z=0.17, P=0.87). The ratio of retinal lesions became stable or complete resolution were 75.0% (15 eyes) and 62.5% (5 eyes), there was no evident difference in time-to-resolution between the two groups (F=0.42, P=0.51). No recurrence was seen during the follow-up period. In cases of unilateral CMVR, there were no patients with a second eye involvement during the follow-up period. No endophthalmitis, vitreous hemorrhage, retinal detachment were found in our study.ConclusionReduced-dose intravitreal ganciclovir is a safe and effective treatment option for CMVR.

  • Clinical effect of minimally invasive vitreoretinal surgery combined with a modified suprachoroidal drainage surgery for retinal detachment associated with choroidal detachment

    Objective To observe the clinical effect of minimally invasive vitreoretinal (MIV) surgery combined with a modified suprachoroidal drainage surgery for retinal detachment associated with choroidal detachment (RRDCD). Methods A prospective clinical study. A total of 27 patients (27eyes) diagnosed as RRDCD were recruited in this study. There were 16 males and 11 females, with an average of (53.67±14.82) years. The mean intraocular pressure (IOP) was (8.2±2.1) mmHg (1 mmHg=0.133 kPa) and best corrected visual acuity (BCVA) of minimum resolution angle logarithm (logMAR) was 1.87±0.58. All subjects underwent 23G MIV combined a modified suprachoroidal drainage surgery, which 23G stab knife and 1 ml syringe needle were used for surgery. The visual outcome, IOP, rate of retinal reattachment and complications were comparatively analyzed preoperatively and postoperatively. Results At 1 day, 10 days, 1 month and 3 months after surgery, the average of logMAR BCVA were 1.62±0.67, 1.51±0.63, 1.39±0.54, 1.32±0.56 and the mean of IOP were (13.47±5.06), (14.43±4.09), (14.89±4.30), (15.38±3.37) mmHg, respectively. There were significant differences of logMAR BCVA and IOP between before and after surgery (F=6.19, 15.21; P<0.05). Retinal reattachments were achieved in 27 eyes (100%) at 1 day and 10 days after surgery. At 1 month and 3 months after surgery, the rate of retinal reattachment were 88.89% (24 eyes) and 85.19% (23 eyes), respectively. No severe complications such as endophthalmitis and choroidal hemorrhage were found at follow-up visits. Conclusion MIV combined with a modified suprachoroidal drainage surgery is an effective and safe treatment for RRDCD, which can promote retina tear closure, improve visual acuity.

  • Comparison of 23G and 20G vitrectomy for diabetic retinopathy

    Objective To compare the efficacy of 23G and 20G vitrectomy for diabetic retinopathy. Methods Thirty-eight eyes of 33 patients with diabetic retinopathy who underwent primary vitrectomy were enrolled in this study. Twenty eyes of 18 patients underwent 23G microincision vitrectomy (23G group). Eighteen eyes of 15 patients underwent 20G vitrectomy (20G group). The followup ranged from 6 to 12 months. The surgical time, number of iatrogenic retinal tears, times of coagulation for hemorrhage, instrumental change for fibromembrane removal, intraocular pressure (IOP) at 1st, 2nd and 3rd days after surgery, postoperative complications, anatomic success rate, and best corrected visual acuity (BCVA) were observed. Results The mean surgical times were (63.5±21.7) and (83.9±27.5) minutes in 23G and 20G group which was a significant difference (t=2.55, P=0.02). The numbers of iatrogenic retinal tears were six and 14, while the mean times of coagulation were 71 and 104 in 23G and 20G group respectively. There were three and 12 eyes needing instrumental change for fibromembrane removal in 23G and 20G group with significant difference (χ2=10.58, P=0.001). At the 1st, 2nd and 3rd days after surgery, IOP were (10.2±2.0), (13.4±2.5), and (14.9±2.3) mm Hg (1 mm Hg=0.133 kPa) in 23G group, (16.5±2.9), (19.3±2.4), and (20.1±2.9) mm Hg in 20G group. Compared with each other, the differences were statistically significant (t=7.71, 7.41, 6.18; P<0.01). There was one eye with intraocular hemorrhage, one eye with neovascular glaucoma (NVG), and one eye with retinal detachment (RD) in 23G group. There were two eyes with intraocular hemorrhage, two eyes with NVG, and one eye with RD in 20G group. Compared with each other, the differences were not statistically significant (χ2=0.49, 0.49, 0.01; P>0.05). The anatomic success rates were 950% and 94.4% in 23G and 20G group without significant difference (χ2=0.01, P=0.94). There were no complications such as endophthalmitis or choroidal detachment. In 23G group, BCVA was <0.05 in one eye, 0.05 – 0.09 in five eyes, 0.1 -0.2 in 10 eyes, ≥0.3 in two eyes. In 20G group, BCVA was <0.05 in one eye, 0.05 - 0.09 in nine eyes, 0.1 -0.2 in six eyes, ≥0.3 in two eyes. Compared with each other, the differences were not statistically significant (χ2=2.70, P>0.05). Conclusion 23G microincision vitrectomy is beneficial to shorten the operation time, reduce instrumental changes, decrease intraoperative complications, and enhance visual rehabilitation.

  • The Influence of High-quality Nursing Care on the Treatment and Control of Asthma

    ObjectiveTo analyze the impact of high-quality nursing care on adult patients with asthma and asthma control. MethodsWe randomly chose 100 patients with asthma from June 2012 to July 2013, and the patients were randomly divided into experimental group (n=50) and control group (n=50). Patients in both the two groups received treatments based on the characteristics of the patients' condition. The control group received conventional care, while the experimental group received high-quality nursing care aimed to help them establish the concept of Asthma Prevention on the basis of the conventional care. Then, we analyzed the effect of nursing intervention in both groups. ResultsBefore the treatment, the forced expiratory volume in 1 second (%) and peak expiratory flow were not significantly different between the two groups (P>0.05). After treatment, both groups had improvement in their lung function. However, improvement in lung function of the experimental group was significantly better than the control group (P<0.05). The disease remission level in the experiment group and the control group was respectively 86.0% (43/50) and 64.0% (32/50) (χ2=6.453, P=0.011). According to the Asthma Control Questionnare (5-item version), in the experimental group, 46 patients completed the assessment and the total control of asthma accounted for 34.8% (16/46), well-controlled asthma accounted for 50.0% (23/46), and uncontrolled asthma accounted for 15.2% (7/46); while in the control group, 48 patients completed the assessment and the total control of asthma accounted for 18.8% (9/48), well-controlled asthma accounted for 43.8% (21/48), and uncontrolled asthma accounted for 37.5% (18/48) (Z=-2.533, P=0.011). The average hospital stay for the experimental group and the control group was respectively 11.24 days and 12.16 days. ConclusionHigh-quality nursing care can improve the quality of life of adult asthma patients, improve the patients' lung function, and enhance the control of asthma.

  • Macular structure changes after surgery and its relationship with visual function in patients with idiopathic macular hole

    Objective To observe the macular structure changes and its relationship with visual function in patients with idiopathic macular hole (IMH). Methods Forty-seven patients (47 eyes) with IMH who underwent pars plana vitrectomy were enrolled in this study. All patients were examined including bestcorrected visual acuity (BCVA), slit-lamp microscopy, indirect ophthalmoscopy, B-scan ultrasonography, optical coherence tomography (OCT) and MP-1. All the patients underwent a standard three-port pars plana vitrectomy. The BCVA, mean light sensitivity (MS) in macular area, macular hole diameter, the photoreceptor inner and outer segment (IS/OS) junction defect, external limiting membrane (ELM) defect were observed on the 1st, 3rd and 6th months after surgery, and then the relationship of IS/OS junction defect, ELM defect, sensitive and BCVA were analyzed. Results The 1st, 3rd and 6th months after surgery, the logarithm of minimal angle of resolution (logMAR) BCVA (t=16.4, 35.7, 20.7; P<0.05) and MS (t=-13.8, -17.9, -2.5; P<0.05) were improved significantly; the macular hole diameter (t=7.7, 7.7, 7.7;P<0.05), IS/OS junction defect (t=24.1, 19.3, 27.4; P<0.05) and ELM (t=20.5, 6.7, 15. 8; P<0.05) defect were decreased significantly. Preoperative IS/OS junction defect and ELM defect were both related to sensitive (r=-0.55, -0.53; P<0.05), but uncorrelated with BCVA (r=0.13, 0.13; P>0.05). IS/OS junction defect and ELM defect 1st, 3rd and 6th months after surgery were both related to MS and BCVA (P<0.05). Conclusions The logMAR BCVA and MS increases, while IS/OS junction and defect ELM defect decreases after surgery in IMH patients. IS/OS junction defect and ELM defect after surgery were both related to sensitive and BCVA.

  • Laparoscopic Operation of Gastric and Gastroesophageal Junction Disease (Report of 59 Cases)

    Objective To investigate the feasibility and safety of laparoscopic operation of gastric and gastroesophageal junction diseases. Methods Between May 2004 and June 2009, 59 patients with gastric and gastroesophageal diseases were treated laparoscopically. The operative methods and maneuvers were evaluated and perioperative interventions, complications and efficacy of patients were analyzed. Results All operations were successfully completed laparoscopically except for one patient with gastric cancer who required a conversion to open surgery. No short-term complications occurred in all cases. No port transplant metastasis occurred for the patients with gastric cancer after an average of 36 months (1-60 months) follow-up. One patient died of liver metastasis 12 months after operation. The 3-year survival rate was 93.3% (14/15). Conclusion Laparoscopic surgery of the gastric and gastroesophageal junction diseases is feasible and safe with minimal invasiveness, which is worth popularizing.

  • Five-year Clinical Outcome of the Clincal Pathway for Geriatric Hip Fracture

    ObjectiveTo introduce the clinical pathway for geriatric hip fracture regulated by our hospital and report the five-year outcomes after the implementation of the pathway. MethodsThe geriatric hip fracture patients treated between September 2003 and August 2012 were followed up. We did not implement the clinical pathway until January 2007. Statistical analysis was done to evaluate the effect of the clinical pathway on patient outcomes by comparing rate of complication, mortality, and length of hospital stay before and after the implementation of the clinical pathway. ResultsAfter the implementation of the pathway, the in-hospital mortality, one-year mortality and the rate of complication were significantly lower. Besides, the time from admission to operation and the total length of stay in hospital were obviously shortened. ConclusionThe use of clinical pathway for geriatric hip fracture can reduce the rate of complication and mortality, and shorten hospital stay, and the five-year outcomes after the implementation of the pathway are satisfying.
