  • Clinical histopathological and misdiagnostic analysis of 359 cases of intraocular tumors

    Objective To probe the clinical character,the histopathological classification and misdiagnoses of intraocular tumors. Methods The clinical and pathological data of 359 patients with intraocular tumor diagnosed clinically between 1980~2000 were retrospectively analyzed. Results There were 300 cases of malignant tumor and 23 cases of benign tumor respectively. Non-oncologic malady and benign tumor misdiagnosed as tumor or malignant tumor were 40 cases. The two leading malignant tumors were retinoblastoma and melanoma. Conclusion The clinical and pathological analysis of intraocular tumor is beneficial to the correct clinical diagnosis and treatment. (Chin J Ocul Fundus Dis,2002,18:28-30)

  • Histopathological observation after radial optic neurotomy in human eyes

    Objective To explore the histopathological changes of the radial optic neurotomy (RON) in human eyes, and to establish the theoretical foundation for the effective RON. Methods Ten patients with unaffected eyeball or optic disc who had undergone ophthalmectomy (7 patients) or orbital exenteration (3 patients) because of intraocular or orbital tumor were gathered. A double-incision in pars plana was performed. One was inserted into illuminating fiber, and an other was inserted into a standard microvitreoretinal (MVR) blade (unbent MVR blade in 4 patients and bent ones in 6; radial incision on nasal side of the optic disc in 4 patients and on both nasal and temporal side in 6). The histopathological examination was performed to observe the location and depth of the incision. Results Eleven incisions were found in 8 out of 10 patients, of which surgical spaces can be observed clearly. Three incisions were obliquely inserted into the optic nerves, 5 sieve plate lateral incisions had the surgical intervals conne cted with the subarachnoid spaces, and 3 incisions caused obvious damage of retinal ganglion cell axons due to the position close to the center. Nine incisions approached to the retrolaminal level, and the other 2 reached the laminal and pr elaminal level respectively. Injured retina beside the optic disc in 1 incision was found. Central retinal vessels were not damaged. Conclusions An optimal incision may be in the edge of the lamina cribrosa, whose depth should be in the level of lamina cribrosa and retrolamina, with surgical intervals connected with the subarachnoid spaces.   • Auto-cortex of crystalline lens-induced neovascular epiretinal memb rane national

    Objective To investigate auto-cortex of crystalline lens-induced neovascular epiretinal membrane(NVERM)by micro-injuring posterior c apsule of crystalline lens. Methods twenty four C57BL/6 mouse between 4-6 weeks were selected, and divided into two groups randomly: auto-cortex of crystalline group and the control group. The auto-cortex of crystalline group was treated by penetrating the posterior capsule of lens and washing out the lens cortex into the mouse vitreous using PBS (phosphate buffered solution), while the control group were injected PBS into vitreous merely. Clinical change s were followed by slit-lamp examination and photograph. The eye balls were enu cleated at the day of 3, 7, 14 and 28 after operation. Both HE and immunohistoch emistry were used to detect the pathological changes. Results postoperative one to three days, 11 of 12 mouse in autocortex of crystalline g roup, lens appear to alba turbid at different levels one after another, and then develop into highdensity chinaware white. Postoperative (po) three days, HE s taining shows cortex of lens debris transmigrated in vitreous cavity, and some o f which approached to internal limiting membrane and lead it to rough and discon tinue; Po7-14 days, the capillary in retina expanded, migrated and broke though t internal limiting membrane which got to the pro retina and became the new ves sels. And typical NVERM were observed. Po28 days, some vascularslike structure formed in vitreous cavity. None of mouse in control group developed NVERM. Conclusion Auto-cortex of crystalline lens can induced neovascular epiretinal membrane in C57BL/6 mouse.  (Chin J Ocul Fundus Dis,2008,24:118-121)

  • Clinical and Pathololgical Analysis of Bone Marrow Tuberculosis

    Objective To explore the pathology and clinical features of bone marrow tuberculosis. Methods 30 cases of bone marrow tuberculosis diagnosed in West China Hospital between January 2004 and December 2010 were recruited in the study. Their pathology and clinical data were retrospectively analyzed. Results The majority of the patients were young and middle-aged. The ratio of male to female was 1.5∶1. Fever was the main symptom, which often accompanied by cough, sputum, fatigue, anorexia, abdominal pain, bloating and diarrhea and other symptoms. Bone marrow tuberculosis involved multiple systems, accompanied by cytopenia and pulmonary tuberculosis. The ratio of sputumsmear positive for acid-fast bacilli was low. Bone marrow biopsy mainly showed granulomatous inflammation with or without caseous necrosis. The mortality could be significantly reduced by anti-tuberculosis treatment in time. Conclusion Bone marrow tuberculosis is serious and often accompanied by multiple systems tuberculosis. Early diagnosis is particularly important. Timely and regular treatment is in great needed.

  • Macular holes: clinical research and therapeutic efficacy

    Macular hole is a retinal hole locates in macular fovea, and can be idiopathic, traumatic and high myopic. Although its etiology, disease course, treatment and prognosis varied from case to case, enforcing macularhole closure and retinal reattachment are challenges to all cases. Completely removal of premacular vitreous cortex is the key to successful repair, and inner limiting membrane (ILM) staining and peeling can greatly help the removal of those cortexes. Selections and usages of different dyes, methods of ILM peeling, and strategies to promote macular retinachoroidal adhesion warrant further study to improve treatment and prognosis of macular holes.

  • Analysis of Mammographic Characteristics of Calcification in Breast Tumor Without Mass

    ObjectiveTo investigate the X-ray diagnostic significance of calcification of the breast tumor without mass. MethodsMammograms of 90 cases of breast tumor without mass confirmed pathologically were retrospectively analyzed. There were 55 cases confirmed benign breast tumor, and the rest cases were breast cancer. The shape, distribution, total number, location of calcifications in the breast, and asymmetric dense of the breast were recorded and watched. Results①The X-ray findings of calcification in benign breast tumors always presented as coarse granular (31), scattered shape (35) with small number, less with the asymmetric dense of the breast (7), and the change of side with axillary lymph node (2). ②Meanwhile, fine sand-like (32), showing the cluster-like distribution (24) with larger number, with the asymmetric dense of the breast (24) and the change of side with axillary lymph node (10). Both of the differences of the calcifications (the shape, the distribution, and the total number) were statistically significant (Plt;0.05). ConclusionsThe calcifications of benign and malignant breast tumors have their unique X-ray characteristics. And there is a great value in differentiating early benign and malignant breast tumor.
