  • 改良显微缝线吻合血管在断指再植中的临床应用

    目的 总结改良显微缝线吻合血管在断指再植术中的优点及临床效果,为吻合微小血管提供一种新的显微缝线。 方法 2004 年4 月- 2008 年4 月,应用改良显微缝线及传统显微缝线于157 例202 指断指再植术中吻合微小血管。男137 例,女20 例;年龄16 ~ 47 岁。机器压伤102 指,机器割伤39 指,电锯锯伤39 指,其他伤22 指。离断指别:拇指27 指,示指63 指,中指56 指,环指30 指,小指26 指。完全离断162 指,不完全离断40 指。受伤至手术时间30 ~ 200 min。随机分两组,改良组78 例105 指,应用改良显微缝线吻合血管342 条;传统组79 例97 指,应用传统显微缝线吻合血管325 条。 结果 单针吻合时间改良组(20.0 ± 2.5)s,传统组(28.0 ± 3.5)s;每一吻合口吻合时间改良组(12.5 ± 2.5)min,传统组(18.5 ± 4.3)min;吻合血管后至完全再通时间改良组(10.0 ± 2.6)min,传统组(12.0 ± 3.5)min;术后改良组出现血管危象致血管栓塞5 指(4.76%),传统组10 指(10.30%)。术后两组再植指体成活率改良组95.23%,传统组89.69%。末节再植成活率改良组95.34%,传统组89.47%;非末节再植成活率改良组95.16%,传统组89.83%。两组各指标比较差异均有统计学意义(P lt; 0.05)。患者均获随访,随访时间3 个月~ 2 年,再植指外观、功能恢复均满意。 结 论 应用改良显微缝线吻合微小血管具操作简便、可靠、缩短血管吻合时间及血管再通时间、提高断指再植术成功率等优点。

  • Early effectiveness of arthroscopic repair of supraspinatus tendon tears with douple-pulley suture-bridge

    ObjectiveTo investigate the early effectiveness of arthroscopic repair of supraspinatus tendon tears with douple-pulley suture-bridge. Methods The clinical data of 38 patients with supraspinatus tendon tears who met the selection criteria between September 2020 and July 2022 were retrospectively analyzed, and all of them were treated with arthroscopic double-pulley suture-bridge technique. There were 15 males and 23 females, aged from 43 to 77 years, with an average of 61.5 years. There were 15 cases of left shoulder and 23 cases of right shoulder. Seven cases had a history of trauma, and the other 31 cases had no obvious inducement. The main clinical symptoms of the patient were pain in lifting the shoulder joint and hug resistance test (+). The time from onset of symptoms to admission ranged from 6 to 19 months, with an average of 10.3 months. Flexion, abduction, and external rotation of the shoulder were recorded before operation and at 3 and 12 months after operation. Pain and function of the shoulder were evaluated by visual analogue scale (VAS) score, University of California Los Angeles (UCLA) shoulder score, and American Society of Shoulder and Elbow Surgeons (ASES) score before operation and at 12 months after operation. Tendon healing, tendon continuity, and tension were evaluated by MRI at 3-6 months after operation, and patient’s satisfaction was evaluated at last follow-up. ResultsAll the incisions healed by first intention, and there was no complication such as incision infection or nerve injury. All patients were followed up 12-34 months, with an average of 23.3 months. VAS score, UCLA shoulder score, and ASES score at 12 months after operation were significantly better than those before operation (P<0.05). The external rotation range of shoulder joint significantly improved at 3 and 12 months after operation (P<0.05), and it further improved at 12 months after operation when compared with 3 months after operation (P<0.05). There was no significant difference in the range of flexion and abduction at 3 months after operation when compared with those before operation (P>0.05), but the range of flexion and abduction at 12 months after operation significantly improved when compared with those before operation and at 3 months after operation (P<0.05). MRI reexamination was performed in 28 patients at 3-6 months after operation. Among them, 25 patients had intact supraspinatus tendon structure, good tension, and tendon healing, and 3 patients had type 1 retear. The remaining 10 patients refused to undergo MRI reexamination because of the satisfactory effectiveness. At last follow-up, 29 patients (76.3%) were very satisfied with the results, 6 (15.7%) were satisfied, and 3 (7.8%) were not satisfied. ConclusionArthroscopic double-pulley suture-bridge technique can achieve the effect of suture bridge operation, reduce the cost of operation and the risk of type 2 retear, and the early effectiveness is satisfactory, but the shoulder joint movement is limited within 3 months after operation.

  • Improved technique for perforation of patella combined with suture anchor and non-tourniquet for repairing acute rupture of the bone tendon junction of quadriceps tendon

    Objective To investigate effectiveness of a improved technique for perforation of patella combined with suture anchor and non-tourniquet for repairing acute rupture of the bone tendon junction of quadriceps tendon. Methods Between June 2010 and June 2016, 15 patients with acute rupture of the bone tendon junction of quadriceps tendon were treated with the improved technique for perforation of patella combined with suture anchor and non-tourniquet. Of them, 14 were male and 1 was female, aged from 19 to 74 years (mean, 44 years). Seven cases were sports injuries, 5 cases were caused by slipping, and 3 cases were caused by violent trauma. The duration of quadriceps tendon rupture ranged from 3 hours to 3 days (median, 2 days). The right side was involved in 9 cases, and the left side in 6 cases. All of the cases were closed injuries. The patients had no sequelae of limb dysfunction and no limb joint surgery in the past. All the patients received anteroposterior and lateral X-ray films of the knee joint and patella axial films during follow-up. Knee function of the patients after operation was evaluated by Lysholm scoring system and Kujala scoring system. Results The operation time was 50-60 minutes (mean, 55 minutes). The intraoperative blood loss was 50-150 mL, with an average of 87 mL. Primary healing of incision was obtained in all patients without complications. All patients were followed up 12-24 months (mean, 18 months). At 1 year after operation, the knee joint function of all patients recovered well; the knee Lysholm score was 92-96, with an average of 94 and the patellofemoral joint Kujala score was 90-95, with an average of 93. There was no re-rupture of quadriceps tendon or loosening of internal fixation during follow-up. According to the lateral and axial X-ray films at 1 year after operation, the patella depth index, femur trochlea depth, sulcus angle, and Insall-Salvati index were 3.62-4.09 (mean, 3.84), 4.45-6.50 (mean, 5.56), 137-145° (mean, 142°), and 0.90-1.18 (mean, 1.06). The lateral patellofemoral angle increased, and the patellar tilt angle and the patella lateral shift distance reduced, all showing significant differences when compared with preoperative ones (P<0.05). Conclusion The improved technique for perforation of patella combined with suture anchor and non-tourniquet for repairing acute rupture of the bone tendon junction of quadriceps tendon can reconstruct a stable patellofemoral joint, increase the strength of fixing, disperse stress effectively and evenly, and increase the area for tendon-bone healing.

  • Study on the Role of the Coated VICRYL Plus Antibacterial Suture in the Prevention of Infection of Appendectomy Incision

    摘要:目的: 探讨在阑尾切除术中应用抗菌薇乔缝线以减少阑尾切口感染的可能性。 方法 : 将我院2007年4月至2009年3月所有阑尾切除术病例1425例随机分为抗菌薇乔缝线组和丝线组,比较其切口感染发生率。 结果 : 统计中按阑尾未穿孔、阑尾穿孔以及总计分别计算切口感染率,在抗菌微乔线组感染率分别为017%、072%、028%,丝线组分别为154%、781%、267%,两组间分别予以X2检验,其〖WTBX〗P 值均小于001,具有显著性差异。 结论 : 缝线是辅助产生切口感染的一个危险因素,在阑尾切除术中使用抗菌薇乔缝线可以显著降低切口感染率。Abstract: Objective: To investigate the application of Coated VICRYL Plus Antibacterial suture in order to reduce the possibility of infection of appendectomy incision. Methods : Hospital from April 2007 to March 2009 appendectomy patients in all 1425 cases were randomly divided into Coated VICRYL Plus Antibacterial suture group and silk group,compared to the incidence of incision infection. Results : The statistics are not in accordance with perforated appendicitis, perforated appendicitis, as well as calculation of the total, respectively, incision infection, the infection rate in the Coated VICRYL Plus Antibacterial suture group were 017%, 072%, 028%, silk group were 154%, 781%, 267% between the two groups separately X2 test, the P value of less than 001, with a significant difference. Conclusion : The suture is to assist the incision produced a risk factor for infection in appendectomy,Coated VICRYL Plus Antibacterial suture can be used in a significant reduction in incision infection rates.

  • EFFECTIVENESS COMPARISON OF TWO ARTHROSCOPIC DIFFERENT FIXATIONS FOR ANTERIOR CRUCIATE LIGAMENT TIBIAL EMINENCE AVULSION FRACTURES

    Objective To compare the effectiveness of arthroscopic screw and suture fixations in treatment of anterior cruciate ligament tibial eminence avulsion fractures. Methods Between January 2002 and January 2009, 43 patients with freshanterior cruciate ligament tibial eminence avulsion fracture were treated, which were rated as types II and III according to Meyers- McKeever-Zaricznyj classification. Fractures were fixed with either screw (screw group, n=21) or nonabsorbable suture (suture group, n=22). There was no significant difference in sex, age, disease duration, and fracture type between 2 groups (P gt; 0.05). The range of motion (ROM) and Lysholm score were compared between 2 groups, and the knee stabil ity was evaluated based on the Lachman test and KT-2000 measurement. Results The operation time was 48-60 minutes (mean, 51.6 minutes) in the screw group, and 55-68 minutes (mean, 63.2 minutes) in the suture group, showing significant difference (t=4.645, P=0.032). Incisions healed by first intention and no compl ication occurred in 2 groups. All patients were followed up (5.7 ± 0.6) years in the screw group and (5.3 ± 0.5) years in the suture group. The fracture healed completely in both groups; the heal ing time was (3.3 ± 0.6) months in the screw group and (3.2 ± 0.4) months in the suture group, showing significant difference (t=3.723, P=0.019). Between the screw group and the suture group, no significant difference was found in ROM [(128.6 ± 10.1)° vs. (130.2 ± 14.1)°, P gt; 0.05] and Lysholm score (94.6 ± 14.5 vs. 95.1 ± 17.2, P gt; 0.05). The stabil ities based on KT-2000 measurement were also similar between 2 groups at last follow-up [(0.9 ± 0.3) mm vs. (1.0 ± 0.4) mm, P gt; 0.05]. Lachman test of 2 groups were negative. Conclusion Boththe screw and nonabsorbable suture fixation techniques for anterior cruciate l igament tibial eminence avulsion fracture (type II or III) have good results in terms of functional outcome and stabil ity. However, some patients show flexion contractures of 5° or 10°.

  • Immediate Graft Flow Outcomes of 8-0 Prolene Sutures in Off-pump Coronary Artery Bypass Grafting

    ObjectiveTo evaluate immediate outcomes of 8-0 Prolene sutures for distal anastomosis during off-pump coronary artery bypass grafting (OPCAB). MethodsA total of 101 patients who underwent OPCAB in Department of Cardiac Surgery of Beijing Anzhen Hospital in 2010 and 2012 respectively were enrolled in this study. There were 87 male and 14 female patients with their age of 46-82 (61.35±8.24)years. All the 36 patients in 2010 received 7-0 Prolene sutures for distal anastomosis, and the 65 patients in 2012 received 8-0 Prolene sutures for distal anastomosis. After anastomosis, transit time flow measurement was used to measure blood flow indexes of graft vessels[left internal mammary artery (LIMA)and saphenous vein (SV)] including blood flow volume (BFV), pulsatility index (PI)and diastolic filling fraction (DF), which were compared between patients receiving 7-0 and 8-0 Prolene sutures. ResultsBFV of LIMA grafts with 8-0 Prolene sutures (n=44)was significantly larger than that with 7-0 Prolene sutures (n=30)[ (33.70±21.13)ml/min vs. (27.50±17.34)ml/min, P=0.032], while PI of LIMA grafts with 8-0 Prolene sutures was significantly smaller than that with 7-0 Prolene sutures (2.15±0.69 vs. 2.58±1.01, P=0.047). BFV and PI of SV grafts with 8-0 Prolene sutures (n=21) were not statistically different from those with 7-0 Prolene sutures (n=6)[ (34.19±16.00)ml/min vs. (29.00±15.48)ml/min, P > 0.05;2.07±0.53 vs. 1.95±0.55, P > 0.05]. DF of all the grafts was greater than 50%, and there was no statistical difference in DF between the 2 groups (P > 0.05). Conclusion8-0 Prolene sutures for LIMA-to-left anterior descending artery can improve BFV and decrease PI of LIMA grafts, which reflects better immediate graft patency of the distal anastomosis and helps improve surgical success rate and long-term prognosis.

  • EFFECTIVENESS OF ARTHROSCOPIC ULTRA-Braid SUTURE PLANE FIXATION FOR ANTERIOR CRUCIATE LIGAMENT TIBIAL EMINENCE AVULSION FRACTURES

    ObjectiveTo investigate the surgical technique and effectiveness of arthroscopic ULTRA-Braid suture plane fixation for anterior cruciate ligament (ACL) tibial eminence avulsion fractures. MethodsBetween June 2012 and October 2014, 16 cases of ACL tibial eminence avulsion fracture were treated with ULTRA-Braid suture plane fixation under arthroscopy. There were 10 males and 6 females, aged from 17 to 38 years (mean, 25.8 years). The left knee was involved in 5 cases and the right knee in 11 cases. The causes were traffic accident injury in 9 cases, falling from height injury in 4 cases, and sports injury in 3 cases. The average interval from injury to operation was 7 days (range, 5-10 days) except 1 patient who received operation at 6 weeks after injury. The knee joint swelling was obvious; the result of Lachman test was positive; and the knee joint Lysholm score was 45.38±9.87. According to classification standard introduced by Meyers-McKeever-Zaricznyj, 7 cases were rated as type Ⅱ, 8 cases as type Ⅲ, and 1 case as type Ⅳ, excluding ligament and meniscus injury. ResultsAll the incisions healed by first intention. The patients received follow-up of 6-18 months (mean, 10 months). The postoperative X-ray and CT showed anatomic reduction (12 cases) or near anatomic reduction (4 cases); all fractures healed at 6 months after operation. The result of Lachman test was negative in the other 15 patients except 1 patient (Ⅱ degree). One patient had slight knee pain at 6 months postoperatively, and pain symptom disappeared after 1 year; the other cases resumed daily activities. Lysholm score at last follow-up was 98.13±2.34, showing significant difference when compared with preoperative score (t=-20.801, P=0.000). ConclusionArthroscopic ULTRA-Braid suture plane fixation for ACL tibial eminence avulsion fractures is an effective procedure with the advantages of minimal trauma, reliable fixation, satisfactory functional recovery, and simultaneously avoiding the second surgery.

  • MULTI-SPOT SUTURE FIXATION UNDER ARTHROSCOPE FOR TREATING TIBIAL INTERCONDYLAR ANTERIOR EMINENCE FRACTURE

    Objective To explore the surgical method and effectiveness of multi-spot suture fixation under arthroscope in treating tibial intercondylar anterior eminence fracture. Methods Between February 2006 and January 2010, 15 patients with tibial intercondylar anterior eminence fracture were treated with multi-spot suture fixation under arthroscope after fracture reduction. There were 14 males and 1 female with a median age of 33 years (range, 7-55 years). The injury causeswere traffic accident in 9 cases, sport in 2 cases, and fall ing in 4 cases. The anterior drawer test, Lachman test, and pivot shift test in all the cases turned out to be positive. The Lysholm score was 64.7 ± 1.3. According to Meyers-McKeever classification, 5 cases were classified as type II, 7 cases as type III, and 3 cases as type IV. Of them, 13 cases had fresh fracture and 2 cases had old fracture. Assisted with anterior cruciate l igament-aimer, bone tunnels (2 mm in diameter) were built through the fracture fragments after fracture reduction, through which Ethibond 2 polyester sutures were guided into the joint by No.18 spine needles; after knot of 2 sutures or knot of only 1 suture was made, the other ends of sutures were fastened together outside the joint. Results The incisions of all the cases healed by first intention with no compl ication of infection or deep venous thrombosis. Fifteen patients were followed up 14-48 months with an average of 24.2 months. The X-ray examination indicated fractures healed in all the cases at 2-4 months (mean, 3 months). At 6 months after operation, 14 cases showed negative results of Lachman test and anterior drawer test but 1 case positive, while all the cases showed negative results in pivot shift test. The extension and flexion spheres of knee activity were normal in 13 cases and were l imited in 2 cases. The Lysholm score was 95.1 ± 2.1, showing significant difference when compared with the preoperative score (t=43.020, P=0.000). Conclusion Multi-spot suture fixation under arthroscope is an effective method with small invasion, rel iable fixation, and simple operation for treating tibial intercondylar anterior eminence fracture.

  • EFFECTIVENESS OF DOUBLE-NEEDLE SUTURE FOR MIXED MENISCUS TEAR REPAIR UNDER ARTHROSCOPE

      Objective To investigate the effectiveness of double-needle suture for mixed meniscus tear repair under arthroscope. Methods Between April 2006 and January 2011, 22 patients with mixed meniscus tear were treated with double-needle suture under arthroscope. There were 14 males and 8 females, aged 18-41 years (mean, 31.3 years). All injuries were caused by sports. The time between injury and admission ranged from 2 days to 4 years (median, 11 months). International Knee Documentation Committee (IKDC) score was 42.5 ± 15.2, Lysholm score was 45.5 ± 13.5, and Tegner score was 2.9 ± 1.6. Seventeen cases complicated with anterior cruciate ligament injury. Results Healing of incision by first intention was achieved in all patients. No injury of nerve and blood occurred. The patients were followed up 12-48 months with an average of 27.6 months. According to Barrett et al. standard, 19 cases (86%) got clinical healing. The IKDC, Lysholm, and Tegner scores were improved to 77.1 ± 8.9, 79.8 ± 9.9, and 6.8 ± 1.6 respectively at last follow-up, showing significant differences when compared with preoperative scores (P lt; 0.05). Conclusion Arthroscopic meniscus repair using double-needle suture can provide good effectiveness because it has high firmness.
