  • Clinical Application of Trans abdominal Operation in Cardiac Cancer

    目的:探讨不同手术入路在贲门癌治疗中的临床应用。方法:回顾性分析我院2003年8月至2009年1月期间收治的153例贲门癌患者的临床资料,根据不同手术入路分为经腹组(n=74)、胸腹联合组(n=27)和经胸组(n=52),对3组患者的手术中、术后恢复情况以及随访结果进行对比分析。结果:手术切除率100%,围手术期无死亡病例。经腹组手术时间短于经胸组和胸腹联合组,术中出血量也少于经胸组和胸腹联合组,但差异无统计学意义(Pgt;0.05)。胸腹联合组和经腹组清扫的淋巴结数目明显多于经胸组,差异有统计学意义(Plt;0.05)。经腹组患者术后住院时间和术后并发症发生率明显少于胸腹联合组和经胸组,差异有统计学意义(Plt;0.05)。3组患者的术后局部复发率和远处转移率的差异均无统计学意义(Pgt;0.05),3年和5年生存率的差异也无统计学意义(Pgt;0.05)。结论:经腹贲门癌切除术是安全和有效的,配合吻合器的使用,操作也是可行的。

  • Non-contact Heart Rate Estimation Based on Joint Approximate Diagonalization of Eigenmatrices Algorithm

    Based on the imaging photoplethysmography (iPPG) and blind source separation (BSS) theory the author put forward a method for non-contact heartbeat frequency estimation. Using the recorded video images of the human face in the ambient light with Webcam, we detected the human face through software, separated the detected facial image into three channels RGB components. And then preprocesses i.e. normalization, whitening, etc. were carried out to a certain number of RGB data. After the independent component analysis (ICA) theory and joint approximate diagonalization of eigenmatrices (JADE) algorithm were applied, we estimated the frequency of heart rate through spectrum analysis. Taking advantage of the consistency of Bland-Altman theory analysis and the commercial Pulse Oximetry Sensor test results, the root mean square error of the algorithm result was calculated as 2.06 beat/min. It indicated that the algorithm could realize the non-contact measurement of heart rate and lay the foundation for the remote and non-contact measurement of multi-parameter physiological measurements.

  • Prediction of the therapeutic response after target-combined chemotherapy treatment for patients with liver metastasis from colorectal cancer using computed tomography texture analysis

    This study aims to investigate the value of pre-treatment computed tomography (CT) texture analysis in predicting therapeutic response of liver metastasis from colorectal cancer after combined targeting chemotherapy. A total of 82 patients with colorectal cancer liver metastases who underwent chemotherapy combined with targeted therapy (cetuximab) between March 2011 and October 2017 comprised this retrospective study population. According to the RECIST1.1, the best curative effect evaluation of patients was recorded. Complete response (CR) and partial response (PR) were assigned to the response group, and the stable disease (SD) and progressive disease (PD) were assigned to the non-response group. The CT texture analysis was based on the Omini-Kinetics software, and the three-dimensional (3D) texture analysis was performed on the marked lesion on portal phase. The differences of texture parameters between the response group and the non-response group were compared. The receiver operating characteristic (ROC) curves were depicted on the parameters which with statistically difference, to characterize value in predicting the response to target-combined chemotherapy. The differences of Entropy, Energy, Variance, std. Deviation, Quantile95 and sumEntropy between the two groups in pre-treatment lesions were significant (P < 0.05). And lesions with higher Entropy, lower Energy, higher Variance, higher std Deviation and higher sumEntropy seemed to indicate a better therapeutic response. When sumEntropy > 0.867, good diagnostic efficiency could be obtained, with sensitivity of 60.5% and specificity of 79.5%, respectively. In conclusion, texture parameters derived from baseline CT images of colorectal cancer liver metastasis have the potential value acting as imaging biomarkers in predicting tumor response to combined target chemotherapy.

  • COMBINED SCAPULAR/PARASCAPULAR BILOBAR FLAPS FOR RECONSTRUCTION OF SEVERE NECK CONTRACTURE

    Objective To discuss the reconstruction of severe neck contracture by transplanting combined scapular/parascapular bilobar flaps, and the probability to reestablish three-dimensional movement of the neck. Methods From January 2003 to November 2004, 9 cases of sustained severeneck contractures were treated (aged 9-32 years). The combined scapular/parascapular bilobar flaps, pedicled on the circumflex scapular vascular bundle, were microsurgically used to cover the soft tissue defect after excision of hypertrophic scar and release of contracture. The maximum size of the combined bilobar flap was 20 cm×8 cm to 20 cm×11 cm,while the minimum one was 15 cm×4 cm to 15 cm×6 cm. Results The combined scapular/parascapular flapswere successfully used to treat 9 cases of severe neck contracture. All patients were satisfied with the final functional and aesthetic results. There was no recurrence during 3-9 months follow-up for 8 patients. The cervicomental angle was 90-105°.Conclusion The combined bilobar scapular/parscapular flap, providing a large area of tissue for coverage in three dimensions with a reliable blood supply by only one pedicle anastomosis during operation, is agood option for reconstruction of the severe neck contracture.

  • Comparison of postoperative nausea and vomiting after laparoscopic sleeve gastrectomy and single anastomosis sleeve ileal bypass: a randomized control study

    ObjectiveTo compare the postoperative nausea and vomiting (PONV) after laparoscopic sleeve gastrectomy (LSG) and single-anastomosis sleeve ileal bypass (SASI), and to explore the risk factors for PONV. MethodA total of 220 patients at the Affiliated Hospital of Xuzhou Medical University from June 2022 to December 2022 were prospectively collected, and were randomly divided into the LSG group and the SASI group; the general condition of the patients was recorded. PONV was assessed on the POD0, POD1, POD2, POD30, POD60, POD90, using the Rhodes index score.ResultsThe total Rhodes index score in the LSG group was higher than that in the SASI group [(25.56±13.54) vs. (16.06±11.28), P<0.05]. The results of multiple linear regression analysis showed that female (P=0.014), LSG (P<0.001), young age (P=0.050), and low BMI (P=0.019) were risk factors for PONV. ConclusionsCompared with LSG, the Rhodes index score after SASI is low. For young women with low BMI, special attention should be paid to the prevention and treatment of PONV after LSG.

  • Review of joint attention deficit intervention based on virtual reality for children with autism

    Joint attention deficit is one of the core disorders in children with autism, which seriously affects the development of multiple basic skills such as language and communication. Virtual reality scene intervention has great potential in improving joint attention skills in children with autism due to its good interactivity and immersion. This article reviewed the application of virtual reality based social and nonsocial scenarios in training joint attention skills for children with autism in recent years, summarized the problems and challenges of this intervention method, and proposed a new joint paradigm for social scenario assessment and nonsocial scenario training. Finally, it looked forward to the future development and application prospects of virtual reality technology in joint attention skill training for children with autism.

  • Diagnostic accuracy of TRECs in newborn screening for severe combined immunodeficiency: a meta-analysis

    ObjectiveTo systematically review the accuracy of T-cell receptor excision circles (TRECs) in screening newborns for severe combined immunodeficiency (SCID). MethodsThe PubMed, EMbase, Cochrane Library, Web of Science, CBM, WanFang Data and CNKI databases were electronically searched to collect the diagnostic accuracy studies related to the objects from inception to October 26, 2022. Two reviewers independently screened the literature, extracted data, and assessed the risk of bias of the included studies using the QUADAS-2 scale. Meta-analysis was performed using Stata 15.0 and Meta-Disc 1.4 software. ResultsA total of 18 studies involving 6 243 718 neonates were included. The results of meta-analysis showed that the pooled sensitivity (SEN), specificity (SPE), positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnosis odds ratio (DOR) were 0.97 (95%CI 0.92 to 0.99), 1.00 (95%CI 1.00 to 1.00), 1447.05 (95%CI 528.49 to 3962.11), 0.13 (95%CI 0.08 to 0.22) and 11698.21 (95%CI 2853.44 to 47958.98), respectively. The area under the summary receiver operating characteristic curve (SROC) was 0.97. ConclusionThe application of TRECs in screening neonatal SCID has high accuracy, which is helpful for early diagnosis of SCID. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusion.

  • Interpretation of Specifications for Diagnosis and Treatment of Primary Liver Cancer (2017 Edition)

    Liver cancer is one of the world’s most prevalent malignancies, and is also the third leading cause of cancer death in China. Hepatitis and cirrhosis background is a major feature of liver cancer patients in China, which makes specific requirements that suits the national conditions in many aspects of prevention and control like screening diagnosis, treatment options, and prognosis follow-up. The Specifications for Diagnosis and Treatment of Primary Liver Cancer (2017 Edition), which is based on China’s practice, proposes liver cancer staging in line with China’s national conditions and forms a multi-disciplinary joint diagnosis and treatment model based on surgical treatment. Liver transplantation is included in liver cancer as one of the surgical treatments option. It also emphasizes the support of evidence-based medicine. The Specifications for Diagnosis and Treatment of Primary Liver Cancer (2017 Edition) may have laid a solid foundation for future diagnosis and treatment of liver cancer in China.

  • Lenvatinib + transarterial chemoembolization + PD-1 antibody in the treatment of hepatocellular carcinoma with main portal vein tumor thrombus and cavernous transformation: four case reports

    Objective To summarize the effect of lenvatinib + transarterial chemoembolization (TACE) + programmed cell death protein-1 (PD-1) antibody in the treatment of hepatocellular carcinoma with main portal vein tumor thrombus and cavernous transformation. Methods In this study, we reported the clinical data of four patients with hepatocellular carcinoma with main portal vein tumor thrombus and cavernous transformation who received conversion therapy with lenvatinib combined with TACE and PD-1 antibody in West China Hospital. Results Among the four patients, two patients achieved complete response and two achieved partial response; tumor markers were significantly decreased after combination treatment. However, all four patients failed to undergo hepatectomy. ConclusionsLenvatinib + TACE + PD-1 antibody is effective for hepatocellular carcinoma with main portal vein tumor thrombus and cavernous transformation. However, there are still many problems worthy of further discussion.
