  • Intra-pulmonary Artery Infusion of Antitumor Necrosis Factor-α Antibody Attenuates Lung Injury after Cardiopu-lmonary Bypass

    Objective To investigate the protective effects of antitumor necrosis factor-α antibody (TNF-αAb) on lung injury after cardiopulmonary bypass (CPB) and their mechanisms. Methods Forty healthy New Zealand white rabbits,weighting 2.0-2.5 kg,male or female,were randomly divided into 4 groups with 10 rabbits in each group. In groupⅠ,the rabbits received CPB and pulmonary arterial perfusion. In group Ⅱ,the rabbits received CPB and pulmonary arterial perfusion with TNF-αAb. In group Ⅲ,the rabbits received CPB only. In group Ⅳ,the rabbits only received sham surgery. Neutrophils count,TNF-α and malondialdehyde (MDA) concentrations of the blood samples from the left and right atrium as well as oxygenation index were examined before and after CPB in the 4 groups. Pathological and ultrastructural changes of the lung tissues were observed under light and electron microscopes. Lung water content,TNF-α mRNA and apoptoticindex of the lung tissues were measured at different time points. Results Compared with group Ⅳ,after CPB,the rabbitsin group Ⅰ to group Ⅲ showed significantly higher blood levels of neutrophils count,TNF-α and MDA(P<0.05),higherTNF-α mRNA expression,apoptosis index and water content of the lung tissues (P<0.05),and significantly lower oxyg-enation index (P<0.05) as well as considerable pathomorphological changes in the lung tissues. Compared with group Ⅱ,after CPB,the rabbits in groups Ⅰ and Ⅲ had significantly higher blood concentrations of TNF-α (5 minutes after aortic declamping,220.43±16.44 pg/ml vs.185.27±11.78 pg/ml,P<0.05;249.99±14.09 pg/ml vs.185.27±11.78 pg/ml,P<0.05),significantly higher apoptosis index (at the time of CPB termination,60.7‰±13.09‰ vs. 37.9‰±7.78‰,P<0.05;59.6‰±7.74‰ vs. 37.9‰±7.78‰,P<0.05),significantly higher blood levels of neutrophils count and MDA (P<0.05),significantly higher TNF-α mRNA expression and water content of the lung tissues (P<0.05),and significantly loweroxygenation index (P<0.05) as well as considerable pathomorphological changes in the lung tissues. Compared with groupⅠ,rabbits in group Ⅲ had significantly higher above parameters (P<0.05) but lower oxygenation index (P<0.05) only at 30 minutes after the start of CPB. Conclusion Pulmonary artery perfusion with TNF-αAb can significantly attenuate inflammatory lung injury and apoptosis of the lung tissues during CPB.

  • THE RECONSTRUCTION OF LARGE BONY DEFECT IN UPPER LIMB AFTER RESECTION OF TUMOR

    Seven cases with bone tumor in upper limb were reported. Five cases were treated by using free vascularized fibular graft, 2 cases by using fusion between humorus and clavicle. A follow-up study of six patients showed that the graft bone was united within 3 months in 5 cases, in 6 months in one case. Partial function of upper limb in 6 patients have been restored.

  • 十二指肠乳头肿瘤局部切除术应用

    【摘要】 目的 探讨十二指肠乳头肿瘤局部切除术在基层医院的应用。 方法 2006年4月-2009年10月,对5例十二指肠乳头肿瘤患者开腹行局部切除术。 结果 5例十二指肠乳头肿瘤局部切除术均成功完成,无手术死亡和并发症。 结论 严格掌握适应证,局部切除术对十二指肠乳头肿瘤是一种适于基层开展、安全有效的可选择术式。

  • STUDY OF CONCURRENT RECONSTRUCTION OF POSTERIOR WALL OF VAGINA WITH PEDICLED MUSCULAR FLAP OF UTERUS AFTER RESECTION OF RECTUM CARCINOMA

    OBJECTIVE To investigate the reconstructional method of posterior wall of vagina after the resection of rectum carcinoma. METHODS From August 1991 to March 1996, 10 patients with rectum carcinoma were adopted in this study, among them, there were 4 cases belong to B stage of Dukas, and 6 cases belong to C stage of Dukes. In operation, rectum carcinoma and posterior wall of vagina were resected, and concurrent reconstruction was finished by using pedicled muscular flap of uterus. RESULTS The effect of operation were satisfactory except one case who was failed because of insufficient blood supply of the flap. Followed-up for 3-6 months, the posterior wall of vagina healed in 7 cases, the width of vagina was 2-3 fingers and the depth was 5-6 cm. CONCLUSION Reconstruction of the posterior wall of vagina with pedicled muscular flap of uterus was available in clinic. For its simplicity and feasibility, it’s suitable for the resection of rectum carcinoma in which the posterior wall of vagina was infiltrated.

  • LIMB SALVAGE SURGERY FOR WRIST IN BONE TUMOR BY USING FREE VASCULARISED FIBULAR GRAFT WITH FIBULAR HEAD OR SIMPLE FIBULAR GRAFT

    Objective To compare the long-term results of vascularised fibulargraft and simple autologous fibular graft for reconstruction of wrist after distal bone tumor resection.Methods From January 1979 to September2002, 15 patients with wrist defects due to distal bone tumor resection were treated with vascularised fibular graft or simple autologous fibular graft and followed up 1 year. The results were graded with Enneking’s system and evaluated radiographicallyaccording to the “International Symposium on Limb Salvage”. The grade system included limb function, radiological examination and the function of ankle. Results The limb function of 8 patients with vascularised fibular graft restored to 80% of normal function and the bone union was achieved within 6 months. The limbfunction of 6 patients with simple autologous fibular graft restored to 67% of normal function. The bony union was achieved within 6 months in 4 cases with thebone graft less than 5 cm and in the 13th and 16th months in 2 cases with the bone graft more than 12 cm. Conclusion It is suitable to use the headof fibular boneas a substitute for the distal radius. The healing of vascularised fibular graft is very quick and haven’t the bone resorption. So in the procedure for reconstruction and limb salvage after bone tumor resection of distal radius, the free vascularised fibular graft with fibular head is an ideal substitute.

  • ABSTRACTSTREATMENT OF PROSTHESIS REPLACEMENT FOR MALIGNANT TUMOR OF THEPROXIMAL HUMORAL

    After freatment of prosthesis replacement for 12 malignant tumer of the proximal humoral werereported. The operation were performed after the indications and methods of treatment had beendefined. All of the 12 cases were followed-up, in the average of 8 yeare. On functional evaluation , 8cases had satisfactory result , but per in 1 cases and 3 cases died from recurrence or metastasis withinfive years. The indieation of the procthetic replantation, the complications, and revision weredisecused.

  • Significance of p16 and Rb Gene Protein Expression in Primary Gallbladder Carcinoma

    Objective To study the expression of p16 and Rb gene in primary gallbladder carcinoma (PGC). MethodsFiftysix cases of PGC and 26 of gallbladder benign lesions were observed by immunohistochemistry (SP). ResultsThe positive rates of p16 and Rb protein in PGC were 46.3% and 71.4% respectively. No association of Rb expression with histological grade, histological type, invasion, metastasis and prognosis was found. There were close correlation between p16 expression and invasion, metastasis as well as prognosis. The inverse correlation of p16 and Rb gene expression in PGC was confirmed. Conclusion The loss of p16 or Rb gene protein is an important molecular event in PGC,and closely related to the block in the negative regulatory pathway of all cell cycle. The detection of p16 protein may be useful in evaluating biological characteristics and prognosis in PGC.

  • Expression of Tumor Necrosis Factor-Related Apoptosis-Inducing Ligand Receptors in Pancreatic Cancer

    【Abstract】 Objective To investigate the expression and significance of tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) receptors in pancreatic cancer. Methods Thirty-two samples of pancreatic cancer tissue were collected from year 2002 to 2004. All of them were verified by histopathology and there were 9 cases of well-differentiated, 12 of moderately differentiated, and 11 of poorly differentiated, in which 12 cases were in the stage of Ⅰor Ⅱand 20 in the stage of Ⅲ or Ⅳ according to the TNM staging method. Eighteen normal pancreatic tissues were used as control group. The expressions of TRAIL receptors (death receptor 4, death receptor 5, decoy receptor 4 and decoy receptor 5) mRNA were assayed by semi-quantitive reverse transcription polymerase chain reaction (RT-PCR) in the pancreatic cancer tissues and the normal pancreatic tissues. Results The expressions of death receptor 4 (DR4) and death receptor 5 (DR5) were detected in all the pancreatic cancer tissues and the normal pancreatic tissues and the levels of DR4 and DR5 were significantly higher than those of the normal pancreatic tissues (P<0.01). Decoy receptor 1 (DcR1) and decoy receptor 2 (DcR2) were also expressed in normal pancreatic tissues, whereas DcR1 and DcR2 were only expressed in 18 and 20 pancreatic cancer tissues, respectively. However, there were no significant difference of the expression of DcR1 and DcR2 between the pancreatic cancer tissues and the normal pancreatic tissues (Pgt;0.05). The expression level of DR5 in pancreatic cancer tissue was correlated with tumor differentiation and clinical stage, and the levels in stage Ⅲ and stage Ⅳwere significantly lower than those of stageⅠand stageⅡ(P<0.05). The expressions of DR4, DcR1 and DcR2 were not correlated with tumor differentiation and clinical stage (Pgt;0.05). Conclusion ①The expression of TRAIL receptors in pancreatic cancer tissues is prevalent, but the types of receptors expressed in different tissues were also different. High expression of death receptors may play an important role in TRAIL recptors regulated pancreatic cancer apoptosis. ②The expression of DR5 is correlated with the differentiation degree of pancreatic cancer cell and clinical stage of tumor. The expressions of DR4, DcR1 and DcR2 should not be considered as related indexes of differentiation degree or clinical stage of pancreatic cancer.

  • TREATMENT OF PROXIMAL HUMERAL DEFECT DUE TO BONE TUMOR BY USE OF NONINTERNAL FIXATION FIBULARAUTOGRAFT

    Objective To study the reparative and reconstructive for proximal humerus defect due to the excision of bone tumor with noninternal fixation non-vascularised fibular autografts. Methods From June 1991 toDecember 2003, 26 non-vascularised fibular grafts were used as substitutes for repair and reconstruction after resection for bone tumors on proximal humerus. Fifteen cases were given curettage and fibular supporting internal fixation, the other 11 cases were given tumor resection and joint reconstruction with proximal fibular graft. The age ranged from 6 to 41 years. Out of 26 patients, 5 had giant cell tumor, 9 had bone cysts, 8 had fibrous dysplasia and 4 had enchondroma. Results Twenty-six patients were followed up from 1 to 12 years (3.4 years on average). Local recurrence was found in 2 cases, and 1 of them died of lung metastasis. Both outlook and function of the reconstructed joints have good results in 15 proximal humeral joint surface reserved cases. Of them, 3 children gained normal shoulder function 3 weeks after operation. Part function were obtained in the other 11 fibular grafts substituted proximal humeral defect. Conclusion Non-vascularised fibular grafts is an appropriate treatment option for proximal humerus bone defect due to excision of bone tumor.

  • Radiotherapy for refractory choroidal hemangioma

    ObjectiveTo observe the effectiveness of radiotherapy for refractory choroidal hemangioma. MethodsEight patients (8 eyes) with choroidal hemangioma were enrolled in this retrospective study. All the patients had received laser or photodynamic therapy before without effectiveness. The patients included 7 males and 1 females. The age was ranged from 11 to 54 years old, with an average of (27.50±15.18) years. All the patients were affected unilaterally, including 3 right eyes and 5 left eyes. There were 5 eyes with circumscribed choroidal hemangioma, 3 eyes with diffused choroidal hemangioma. All eyes had extensively exudative retinal detachment. The vision was from light sensation to 0.01. The volume of the tumors was ranged from 1.96 to 5.35 cm3, with a mean of (3.37±1.06) cm3. All the patients were treated with X rays by conventional fractional radiotherapy. Four of 8 patients were applied 24Gy totally in 8 fractions, while the other 4 patients were applied 46Gy in 23 fractions. Follow-up period ranged from 7 to 95 months, with medium of 42 months. ResultsRetinas reattached in all the eyes while exudation being absorbed. No retinal detachment happened again. To the last follow-up, the vision was from light sensation to 0.6. Visual activity improved in 6 eyes while 2 eyes improved obviously. Visual acuity was stable in remaining 2 eyes. The volume of the tumors decreased to 1.24-2.16 cm3, with a mean of (1.68±0.30) cm3. The percentage of the tumor decreased by 14.6-72.7, with an average of (44.89±21.30)%. No radiotherapy-associated complication occurred. ConclusionRadiotherapy is an efficient and safe treatment for refractory choroidal hemangioma.
