  • PEDICLED SUPERIOR GLUTEAL ARTERY PERFORATOR BILATERAL QUADRILOBED FLAPS FOR REPAIR OF LARGE SACROCOCCYGEAL PRESSURE SORES

    Objective To investigate the effectiveness of pedicled superior gluteal artery perforator bilateral quadrilobed flaps for repairing large sacrococcygeal pressure sores. Methods Between June 2003 and August 2011, 6 paraplegia patients with large sacrococcygeal pressure sores were repaired with the pedicled superior gluteal artery perforator bilateral quadrilobed flaps. There were 2 males and 4 females with an average age of 45.6 years (range, 37-62 years). The mean disease duration was 8.4 months (range, 3-26 months). According to National Pressure Ulcer Advisory Panel (NPUAP) standard, 6 cases rated as degree IV. The size of pressure sores ranged from 15 cm × 13 cm to 18 cm × 16 cm. The size of flaps ranged from 18 cm × 14 cm to 21 cm × 15 cm. Results After operation, all flaps survived successfully. The wounds healed by first intention in 5 cases; partial dehiscence of incision occurred in 1 case, which was cured after dressing change for 26 days. Six patients were followed up 6-24 months (mean, 12.5 months). The appearance and texture of the flaps were smooth and soft with good elasticity and no ulceration. Conclusion Pedicled superior gluteal artery perforator bilateral quadrilobed flaps can repair large sacrococcygeal pressure sores. The appearance of flaps is smooth and has good compression-resistance effect.

  • BASIS AND CLINICAL APPLICATIONS OF VASCULARIZED EPIPHYSEAL TRANSPLANTATION

    To introduce the basic research and cl inical appl ications of the vascularized epi physeal transplantation. Methods The appl ied anatomy, experimental researches, and cl inical appl ication about the vascularized epi physeal transplantation were summarized in the past two decades. The effectiveness of epi physeal transplantation were discussed in the article. Results The epiphysis flap of fibular head with inferior lateral genicular artery and the epiphysis flap of il iac with deep superior branch of superior gluteal artery can be used as the donor sites of epiphyseal transplantation. Animal experiments proved that the vascularized epi physis survived and maintained growth after transplantation. In a typical caseundergoing distal ulnar reconstruction by the graft of peroneal epiphyseal, the 18-year follow-up results showed that the repaired ulna was nearly as long as the contralateral side and the function of the forearm was good. Conclusion It is an effective way to repair epiphyseal defects of long bones in children with vascularized epiphyseal transplantation.

  • AN EFFECT OF MULTI-ISLAND FLAP WITH SHALLOW BRANCH OF GLUTEUS UPPERARTERY ON REPAIR OF SACRUM SOFT TISSUE DEFECT

    Objective To explore a safe, highlyefficient and rapid approach to the repair of the sacrum soft tissue defect and/or partial exposure of the bone.Methods From February 2003 to April 2006, 6 patients (4 males, 2 femals; aged 28-67 years) with the sacrum soft tissue defect were surgically treated by the multiisland flap with shallow branches of the gluteus upper artery. The soft tissue defects ranged in area from 15 cm×12 cm to 25 cm×20 cm,averaged 20 cm×16 cm.The obtained flaps ranged in area from 18 cm×15 cm to 30 cm×25 cm. Of the patients, 5 had a sacral ulcer (Grade Ⅲ in 3 patients, Grade Ⅳ in 2) and 1 had a tumor, with their illness course from 3weeks to 20 years. Results All the flaps survived completely in the 6 patients, in whom 5 had an incision healing of the first intention, and the remaining 1had a healing of the second intention 32 days after the treatment for the minorischemia and necrosis at the edges of the flap.The follow-up for 2-38 months (average, 19.3 months) revealed that all the flaps grew well with no recurrence of the sacralulcer. Conclusion The surgical treatment with the multi-island flap with shallow branches of the gluteus upper artery is a safe, highly-efficient and rapid approach to repair of the sacrum soft tissue defect and/or partial exposure of the bone. This kind of treatment has advantages of simpler procedures, better blood circulation of the flap, fewer complications, and higher success rates. 
