  • Investigation of mid-to-long term quality of life and influencing factors in patients after heart valve replacement

ObjectiveTo investigate the mid-to-long term quality of life (QOL) and influencing factors in the patients after heart valve replacement.MethodsConvenient sampling method was used to recruit 224 patients who had heart valve replacement surgery in our hospital from June 2008 to June 2017. There were 63 males and 161 females, aged 18-83 (51.49±10.60) years. General information questionnaire and MOS SF-36 scale were used to collect data. Univariate analysis and multiple linear regression analysis were used to analyze and determine the influencing factors for the QOL.ResultsThe total score of QOL of 224 patients was 71.78±17.60 points. All these scores of the patients were significantly lower than those of the general population of Chengdu (P<0.05) except the dimension of vitality. The QOL scores differences between sex, marital status, family income, working condition, the occurrence of anticoagulant complications and other complications were significant (P<0.05). Multivariate analysis showed that family income, working condition, the occurrence of anticoagulant complications and other complications could be included in the regression model, explaining 26.8% of total variance. ConclusionThe mid-to-long term QOL is lower in the patients after heart valve replacement. Family income, working condition, anticoagulant complications and other complications are the main influencing factors for mid-to-long term QOL for patients after heart valve replacement.

  • Effects of enhanced external counterpulsation for patients with chronic heart failure: A systematic review and meta-analysis

Objective To evaluate the effects of enhanced external counterpulsation (EECP) on exercise capacity and quality of life in patients with chronic heart failure. Methods PubMed, The Cochrane Library, EMbase, CNKI, Wanfang Data, VIP and CBM databases from January 1, 2010 to October 1, 2022 were searched by computer for the randomized controlled trial (RCT) about the intervention of EECP in patients with heart failure. Two researchers independently screened literature and extracted data. The meta-analysis was performed by RevMan 5.3. Results Nineteen RCTs were included. After EECP treatment, 6-minute walk distance (MD=57.37, 95%CI 40.89 to 70.85, P<0.001) and left ventricular ejection fraction improved (SMD=0.85, 95%CI 0.55 to 1.14, P<0.001). B-type natriuretic peptide decreased significantly (SMD=−0.67, 95%CI −1.09 to −0.25, P=0.002). The left ventricular end diastolic diameter (MD=−7.77, 95%CI −11.49 to −4.04, P<0.001), and the left ventricular end systolic diameter were significantly reduced (MD=−8.53, 95%CI −13.47 to −3.60, P<0.001). The quality of life of patients was improved (MD=16.34, 95%CI 0.59 to 32.10, P=0.04). Conclusion EECP can improve the exercise ability and the quality of life in patients with heart failure. However, more and larger well-designed RCTs are still needed to verify this conclusion.

  • The interpretation of the 2017 American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care: Pediatric Basic Life Support and Cardiopulmonary Resuscitation Quality

In November 2017, the American Heart Association updated the pediatric basic life support and cardiopulmonary resuscitation (CPR) quality. The new guidelines focused on the clinical value of chest compression-only CPR versus CPR using chest compressions with rescue breaths in children, rather than a comprehensive revision of the 2015 edition guidelines. The Pediatric Task Force of the International Liaison Committee on Resuscitation updated part content of the guidelines according to the continuous evidence review process. Guidelines recommend CPR using chest compressions with rescue breaths should be provided for infants and children with cardiac arrest. Bystanders provide chest compressions if they are unwilling or unable to deliver rescue breaths. This article mainly interprets the updated content.

  • Heart sound classification algorithm based on bispectral feature extraction and convolutional neural networks

Cardiovascular disease (CVD) is one of the leading causes of death worldwide. Heart sound classification plays a key role in the early detection of CVD. The difference between normal and abnormal heart sounds is not obvious. In this paper, in order to improve the accuracy of the heart sound classification model, we propose a heart sound feature extraction method based on bispectral analysis and combine it with convolutional neural network (CNN) to classify heart sounds. The model can effectively suppress Gaussian noise by using bispectral analysis and can effectively extract the features of heart sound signals without relying on the accurate segmentation of heart sound signals. At the same time, the model combines with the strong classification performance of convolutional neural network and finally achieves the accurate classification of heart sound. According to the experimental results, the proposed algorithm achieves 0.910, 0.884 and 0.940 in terms of accuracy, sensitivity and specificity under the same data and experimental conditions, respectively. Compared with other heart sound classification algorithms, the proposed algorithm shows a significant improvement and strong robustness and generalization ability, so it is expected to be applied to the auxiliary detection of congenital heart disease.
