ObjectiveTo systematically review the trend of tuberculosis among Chinese students. MethodsPubMed, Web of Science, EMbase, CBM, WanFang Data and CNKI databases were electronically searched to collect cross-sectional studies on the incidence and trend of tuberculosis among students in China from inception to August 2021. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies; then, meta-analysis was performed by using Stata 15.0 software. ResultsA total of 97 cross-sectional studies were included. The results of meta-analysis showed that: the overall incidence of tuberculosis among students in China was 18.63 per 100 000 persons. The incidence in the northwest, south, northeast, north, east, central, and southwest 46.81, 11.22, 24.38, 12.77, 12.03, 18.95, and 39.26 per 100 000 persons, respectively. The incidence among university students, senior high school students, junior school students, and primary school students 38.17, 33.84, 8.85, and 1.68 per 100 000 persons, respectively. ConclusionCurrent evidence shows that the incidence of tuberculosis among students in China is high. Among them, the incidence rate of tuberculosis in the central and western regions, universities and high school students is relatively high.
Objectives To systematically review the efficacy of conservative treatment and open reduction with internal fixation for multiple rib fractures. Methods We searched WanFang Data, CNKI, VIP, PubMed, EMbase, The Cochrane Library and Web of Science from inception to December 2017 to collect studies on conservative treatment and open reduction with internal fixation for multiple rib fractures. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. RevMan 5.3 software was used for meta-analysis. Results A total of 16 studies were included, involving 1 374 patients, 723 patients in the surgical group and 651 patients in the conservative group. The meta-analysis showed that the length of stay in the ICU (MD=–3.41, 95%CI –4.92 to –2.43, P<0.000 01), total length of stay (MD=–7.60, 95 %CI–10.67 to–4.53,P<0.000 01), incidence of pulmonary arylene (RR=0.40, 95%CI 0.29 to 0.54,P<0.000 01), incidence of lung infections (RR=0.43, 95%CI 0.30 to 0.61,P<0.000 01), and incidence of chest wall malformation (RR=0.05, 95%CI 0.03 to 0.11,P<0. 0.000 01) in the surgical group were superior to the conservative group. Conclusions Compared with conservative treatment, open reduction with internal fixation can significantly improve the recovery time of patients with multiple rib fractures, reduce hospitalization time, the incidence of perioperative complications, and significantly enhance the prognosis of patients, which is more conducive to the rehabilitation of patients.
ObjectiveTo systematically review the risk of hemorrhage in pregnant cerebral arteriovenous malformation (CAVM) patients. MethodsThe Web of Science, PubMed, Cochrane Library, WanFang Data, VIP and CNKI databases were electronically searched to collect clinical studies related to hemorrhage in pregnant CAVM patients from inception to June, 2023. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies. Meta-analysis was then performed by using Stata 13.1 and R 4.0.4 software. ResultsA total of 13 studies involving 2 273 patients were included. The results of meta-analysis showed that the pooled incidence of hemorrhage in pregnant CAVM patients was 0.18 (95%CI 0.12 to 0.24). The subgroup analyses were carried out based on world regions and the study duration, with the higher pooled incidence of 0.37 (95%CI 0.05 to 0.80) in Europe and 10 to 20 years of study duration of 0.37 (95%CI 0.17 to 0.57). Conclusion Current evidence shows that the incidence of hemorrhage in pregnant CAVM patients is about 18%. Due to the limited quality and quantity of the included studies, more high quality studies are needed to verify the above conclusion.
ObjectiveTo systematically review the incidence and influencing factors of early enteral nutritional feeding interruptions in critically ill patients. MethodsThe PubMed, Web of Science, Embase, Cochrane Library, CNKI, WanFang Data and CBM databases were electronically searched to collect observational studies on the early enteral nutritional feeding interruptions in critically ill patients from inception to January 2, 2024. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies. Meta-analysis was then performed by using Stata 17.0 software. ResultsA total of 12 studies including 1 121 patients were included. Meta-analysis showed that the incidence of early enteral nutritional feeding interruptions in critically ill patients was 75.0% (95%CI 64.0% to 84.0%). Influenced by feeding intolerance, airway management, tube problems, radiological examination, and endoscopy, surgery and so on, interruptions of early enteral nutritional feeding frequently occur in critically ill patients. ConclusionCurrent evidence shows that early enteral feeding interruptions in critically ill patients are affected by many factors, and the incidence is high. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusion.
ObjectiveTo investigate the clinicopathologic and epidemiological characteristics of patients with gastric cancer at our hospital in the past six years. Methods A total of 958 patients with gastric cancer were selected from January 2004 to December 2009 and clinicopathologic characteristics such as basic data, pathological type, tumor location, and TNM stage were retrospectively analyzed. ResultsOf the 958 patients,697 cases (72.8%) were male and 261 cases (27.2%) were female, and the age ranged from 26 to 91 years old (mean 62.6 years old). The occurrence rate of gastric cancer was higher in the patients of 46-65 years old (49.4%, 473/958) than that in the patients of ≥66 years old (42.3%, 405/958) and ≤45 years old (8.3%, 80/958). Regarding pathologic type, the majority of advanced gastric cancers (70.1%) were Borrmann Ⅱ, the adenocarcinoma and signetring cell carcinoma accounted for 82.3% and 7.8%, respectively. The cancer of gastric cardia accounted for 53.0%. Stagestratified analysis revealed that the majority of gastric cancers (47.4%) were stage Ⅲ. Conclusion①A prevalence of gastric cancer is found in middle, elderly male patients. Poorly differentiated adenocarcinoma in cardia is prominent. ②Patients with high malignant ganstrie cancer are younger. ③The majority of patients who underwent the surgical treatments have advanced or metastatic tumor, therefore it is necessary to improve the early diagnosis of gastric cancer.
ObjectiveTo explore the influence of nursing interruption event intervention on the incidence of nursing risk events. MethodsIn January 2012, we carried out intervention on nursing interruption events. And general situation questionnaire was used on December 30th, 2011 (control group) and December 30th, 2012 (intervention group) respectively to investigate 190 clinical nurses. ResultsThe occurrence of the interruption events was positively correlated with the incidence of nursing risk events. Effective intervention significantly reduced the incidence of risk events (P<0.05). ConclusionEffective prevention of adverse outcomes caused by interruption events reduces the risk of nursing, improves the quality of care, and ensures the safety of the patient.
ObjectiveTo systematically review the characteristics of adverse transfusion reaction in Grade Three Class-A hospitals in China, and to provide scientific evidence for related control and prevention work. MethodsSuch databases as the PubMed, EMbase, The Cochrane Library (Issue 11, 2014), CNKI, VIP, WanFang Data and CBM were searched for studies investigating the characteristics of adverse transfusion reaction with ≥6 months observation time in Grade Three Class-A hospitals in China up to November 2014. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data and assessed the methodological quality of included studies. Then, meta-analysis was performed by using the R 3.1.1 software. ResultsSeventy-two studies involving 1 660 472 cases of blood transfusion were included. The results of pooled analysis showed that the total number of adverse transfusion reaction rate was 0.58% (95%CI 0.48 to 0.69). Sub-group analysis shows that the blood transfusion adverse reaction rates in the eastern, central and western regions were 0.42% (95%CI 0.31 to 0.55), 0.55% (95%CI 0.41 to 0.70), 0.56% (95%CI 0.43 to 0.70), respectively; and there were statistical differences between regions (P<0.05). Different types of adverse transfusion reaction rates were 38.85% (95%CI 34.27 to 43.53) for non hemolytic febrile reaction and 55.67% (95%CI 50.79 to 60.50) for allergic. The adverse reactions incidence of different kinds of blood products transfusion occurred were 0.41% (95%CI 0.32 to 0.50) in red blood cell, 0.57% (95%CI 0.43 to 0.72) in plasma, 1.00% (95%CI 0.73 to 1.30) in platelet. The incidence of adverse reaction of platelet transfusion was significantly higher than that of red blood cell and plasma (P<0.05). ConclusionThere is a big gap between domestic and foreign blood transfusion adverse reaction reports, therefore comprehensive measures should be taken for further prevention and control.
Objectives To systematically review the relationship between indoor decoration and childhood leukemia in China. Methods CNKI, WanFang Data, VIP, CBM, PubMed, EMbase and The Cochrane Library databases were electronically searched to obtain case-control studies of the relationships between indoor decoration and childhood leukemia from inception to December 2016. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then, meta-analysis was performed by using RevMan 5.3 software. Results A total of 13 studies involving 1 727 cases and 2 468 controls were included. The results of meta-analysis showed that indoor decoration could increase the risk of childhood leukemia in China (OR=2.69, 95%CI 1.82 to 3.98, P<0.000 01). Conclusions The current evidence suggests that indoor decoration is a risk factor for childhood leukemia in Chinese. Due to limited quality and quantity of the included studies, more high-quality studies are required to verify above conclusions.
ObjectiveTo analyze the incidence and risk factors of tuberculosis in the city of Mianyang based on data from active cases.MethodsFrom March 2018 to April 2019, 199 182 residents were selected for the study. Data were collected using a questionnaire, digital radiography (DR), physical examination and laboratory tests. The incidence of tuberculosis was estimated, and multivariate logistic regression was used to identify factors associated with the disease.ResultsThroughout the process, 103 residents were diagnosed with active tuberculosis, corresponding to an incidence of 51.71 per 100 000. Risk of tuberculosis was significantly higher among individuals who were over age 60 (OR=1.74, 95%CI 1.11 to 2.73, P=0.02), males (OR=4.39, 95%CI 2.74 to 7.04, P<0.001), medical workers (OR=11.18, 95%CI 2.99 to 41.84, P<0.001), and those with a history of tuberculosis (OR=16.43, 95%CI 8.10 to 33.33, P<0.001). Conversely, individuals with higher levels of education were associated with lower risk of tuberculosis: compared to those with primary school or less, those with a junior high school education had an OR of 0.53 (95%CI 0.30 to 0.88, P=0.02); high school/technical school had an OR of 0.36 (95%CI 0.15 to 0.92, P=0.03); junior college or above had an OR of 0.23 (95%CI 0.06 to 0.88, P=0.04).ConclusionsAnalyzing tuberculosis epidemiology based on active cases can help detect the disease as well as control or even prevent epidemics. Individuals who are more senior, males, medical workers, with a history of tuberculosis, and those with lower levels of education may be at higher risk of the disease. These results may improve screening efforts and allow timely intervention.
Objective To investigate the incidence of urolithiasis in infants aged 0-3 years old fed by melaminetainted milk powder in Gansu province. Methods Questionnaires were distributed to 7 856 infants who were screened for urolithiasis in the Second Hospital of Lanzhou University by ultrasound examination from September 25, 2008 to November 15, 2008. Data were collected by Excel and analyzed by SPSS 13.0 software. Results A total of 4 090 (52.06%) male infants and 3 766 (47.94%) female infants underwent ultrasound examination in the Second Hospital of Lanzhou University. The mean age of the infants with urolithias was 18.30±9.68 months. Of those, 704, including 434 males (61.65%) and 270 females (38.35%), were diagnosed with urolithiasis, with an incidence of 8.96%, and 99.14% of calculus was located in kidney. And, 58 aged 0-6 months with an incidence of 3.16%, 218 aged 6-12 months with an incidence of 11.01%, 275 aged 12-24 months with an incidence of 12.55%, and 153 aged 24-36 months with an incidence of 8.27%. The relationship between the incidence of urolithiasis and gender as well as age was assessed by using the Pearson Chi-square test. The results showed significant differences in the incidence of urolithiasis among infants of different genders and different ages (Plt;0.05). The result of multiplelogistic regression analysis indicated that gender was related to the incidence of urolithiasis (Plt;0.05). Conclusion The incidence of urolithiasis in the infants aged 0-3 years old in Gansu province is relatively high. The incidence of urolithiasis focuses on the infants aged 6-24 months and has anatomical specificity.
