  • Study on the Prevalence and Risk Factors of Hypertension in Anyue County from 2011 to 2013

    ObjectiveTo explore the prevalence and risk factors of hypertension in Anyue County from June 2011 to June 2013. MethodsUsing stratfied random cluster sampling method, 5 391 people over 15 years of age were selected from 3 residential areas and 3 natural villages to finish a questionnaire and blood pressure measurement. ResultsThe total prevalence rate of hypertension in Anyue County was 18.77%. The prevalence rates of hypertension in urban areas and rural areas were 21.75% and 16.20%, and the difference was significant (χ2=27.120, P<0.001). In both urban and rural areas, the prevalence rate of hypertension increased with age (χ2=475.634, P<0.001; χ2=394.026, P<0.001). The percentages of awareness, treatment and control in Anyue County were 31.30%, 24.41%, and 9.09%. The percentages of awareness, treatment and control in urban areas were 40.15%, 33.70%, and 11.23% and were 20.68%, 13.65%, and 6.61% in rural areas. There were significant differences in the percentages of awareness, treatment and control between urban and rural areas (χ2=44.475, P<0.001; χ2=54.861, P<0.001; χ2=8.202, P=0.004). The logistic regression analysis showed that age (OR=1.061, P<0.001), diabetes (OR=1.550, P<0.001), hyperlipemia (OR=2.372, P<0.001) and smoking (OR=1.335, P<0.001) were the risk factors for hypertension; and it showed that high level of education was a protective factor for hypertension (OR=0.755, P<0.001). ConclusionBecause of high prevalence and low percentages of awareness, treatment and control in Anyue County, the prevention and control situation of hypertension are grim. We should focus on the control of smoking, blood lipid and blood glucose.

  • Disease burden and future trends of rheumatoid arthritis in mainland China from 1990 to 2021

    Objective To analyze the trends and influencing factors of rheumatoid arthritis disease burden in mainland China from 1990 to 2021 and predict its disease burden over the next 15 years. MethodsData on RA incidence, prevalence, mortality, and disability-adjusted life years (DALYs) in mainland China were extracted from the Global Burden of Disease Study 2021 (GBD 2021). Joinpoint regression was used to analyze temporal trends, while an age-period-cohort model assessed age, period, and birth cohort effects. Decomposition analysis explored the contributions of population aging, population growth, and epidemiological changes. An ARIMA model was applied to predict future disease burden. ResultsFrom 1990 to 2021, the number of RA cases in mainland China increased by 93.5% (incidence), 133% (prevalence), 115% (deaths), and 107% (DALYs), though age-standardized rates showed smaller changes. The disease burden was significantly higher in women than in men, with sex-specific peaks in onset and prevalence. Joinpoint regression revealed rising age-standardized incidence and prevalence rates (AAPC=0.54% and 0.51%, respectively) but declining mortality (AAPC=−0.78%). Cohort effects indicated higher RA risk in later-born populations (RR=1.53 for the 2012 cohort). Decomposition analysis identified population growth as the primary driver of increased burden. Projections suggested that by 2036, the age-standardized incidence and prevalence would rise to 13.92/100 000 and 248.84/100 000, respectively, while DALYs rates might decline to 42.09/100 000. ConclusionThe RA disease burden in mainland China is driven by both population aging and epidemiological factors, with notable sex disparities and cohort effects. Targeted interventions for high-risk populations, optimized healthcare resource allocation, and further research on influencing factors are needed to develop precise prevention and control strategies.

  • Epidemiological Sampling Survey on Chronic Obstructive Pulmonary Disease in Urban Area of Chongqing City

    Objective To study the prevalence and risk factors of chronic obstructive pulmonary disease ( COPD) and provide reliable data for COPD prevention and management. Methods By cluster-random-sampling survey, the residents aged 40 yrs or older in a community in South Bank District of Chongqing were investigated.The post-bronchodilator FEV1 /FVC lt; 0. 7 was defined as COPD. Those individuals with FEV1 /FVC lower than 0. 7 would received history inquiry, X-ray of chest and ECG to exclude other disease that could impair pulmonary function. Results 1518 residents were enrolled( 517 male and 1001 female) . The prevalence of COPD was 12. 78% ( male 23. 02% and female 7. 49% ) . The major risk factors were cigarettes smoke( OR: 2. 88, 95% CI:2. 118-3. 928) and indoor cooking smoke( OR: 1. 98, 95% CI: 1. 685-3. 317) . There were 22.06% smokers were diagnosed as COPD. Only 9. 30% patients had known themselves disease condition about chronic bronchitis,emphysema or COPD. Conclusion The prevalence of COPD in Chongqing city is significant higher than average level of the whole country and the patients have poor knowledge about the disease.

  • Prevalence of Overweight and Obesity and Its Influence Factors among Inhabitants Aged 15-69 Years in Zhuhai

    Objective To understand the prevalence of overweight and obesity and its influence factors in Zhuhai inhabitants. Methods Applying multi-stage cluster random sampling in the three administrative areas of Zhuhai including Xiangzhou, Doumen and Jinwan, A questionnaire-based survey was performed in conjunction of the measurement of height and weight among 961 inhabitants aged 15-69 years. In addition, a multivariate unconditional logistic regression model was employed to analyze the influence factors of overweight and obesity. Results The prevalence and standardized rates of overweight and obesity in the study population were 18.1%, 17.6%, 6.6%, 6.5%, respectively. The standardized rates of overweight and obesity in male and female were 18.4%, 5.8%, 16.5%, 7.2%, respectively. Age, drinking, smoking and regional difference were identified as the 4 risk factors of overweight and obesity, their OR values being 1.028, 1.683, 0.677, 1.404, Plt;0.05, respectively. Conclusion The prevalence of overweight and obesity in Zhuhai’s inhabitants was over the average level of Chinese residents, and overweight and obesity has become a major risk factor influencing the health of Zhuhai’s inhabitants. In view of the influence factors of overweight and obesity, timely and effective prevention and control measures should be taken.

  • Prevalence rate of gestational diabetes mellitus in China: a meta-analysis

    ObjectiveTo systematically review the prevalence rate of gestational diabetes mellitus in Chinese population.MethodsPubMed, EMbase, The Cochrane Library, CNKI, WanFang Data and VIP databases were electronically searched to collect relevant literature of the prevalence rate of gestational diabetes in Chinese population from inception to October 2017. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, then, meta-analysis was performed by using R 3.4.1 software.ResultsA total of 27 studies were included involving 183 338 peoples, of whom 23 834 were diagnosed as gestational diabetes mellitus by oral glucose tolerance test (75g). The prevalence rate was 13% (95%CI 10 to 16). The prevalence rate of gestational diabetes mellitus in north area was 13% (95%CI 9 to 19), central area was 11% (95%CI 2 to 39), east area was 15% (95%CI 12 to 19), south area was 12% (95%CI 11 to 14), northwest area was 5% (95%CI 3 to 8) and southwest area was 4% (95%CI 3 to 4). The prevalence rate of gestational diabetes mellitus during January 2005 to December 2012 was 11% (95%CI 7 to 16), during December 2012 to June 2016 was 17% (95%CI 14 to 20). The prevalence of specialist hospitals was 9% (95%CI 6 to 14), and which of comprehensive hospital was 14% (95%CI 11 to 18). The prevalence of gestational diabetes screening at 24 to 28 weeks gestation remained at 13% (95%CI 10 to 17).ConclusionThe prevalence rate of gestational diabetes mellitus is high in China. The prevalence rate of gestational diabetes mellitus in north China, central China and east China is significantly higher than that in southern China, northwest China and southwest China. The prevalence rate during 2012-2016 is significantly higher than that in 2005 to 2012. Improvement of lifestyle and living standard should be considered be closely related with that. The prevalence of comprehensive hospitals is higher than specialist hospitals. It is related to the China’s economic conditions, medical care situations and the medical habits of patients.

  • Epidemiological investigation of diabetic retinopathy in Beixinjing blocks, Shanghai

    Objective To investigate the incidence and the correlative factors of diabetic retinopathy (DR) in patients with diabetes mellitus(DM)who lives in Beixinjing blocks, Shanghai. Methods Residents with DM were enrolled according to resident health archives. The data of disease history, visual acuity, eye disease and introcular pressure were collected by inquiry and examination. Photography of ocular fundus was used to confirm the diagnosis of DR. Results A total of 535 residents excepted the examination with the participating rate of 90.68%, in whom 146 (27.29%) were identified as with DR. The incidence of single and proliferative DR was 22.29% and 4.30%, respectively. Duration of DM was the independent risk factor of DR, while long duration of DM, accompanied with peripheral neuropathy and body mass index was the in-order independent factor of proliferative DR. Conclusions The incidence of DR is high in residents with DM. Monitoring DR progress in DM residents with risk factors is recommended. (Chin J Ocul Fundus Dis, 2006, 22: 31-34)

  • Prevalence and disease burden of knee osteoarthritis in China: a systematic review

    ObjectivesTo systematically review the prevalence and disease burden of knee osteoarthritis (KOA) in China.MethodsPubMed, EMbase, CNKI, WanFang Data and VIP databases were searched to collect cross-sectional studies about the prevalence and disease burden of KOA in China from January 1st 1995 to August 31st 2017. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then meta-analysis was performed by using R statistical software.ResultsA total of thirty-three studies were included. The results of meta-analysis showed the prevalance rate of KOA was 18% (95%CI 14% to 22%), and it was higher in women (19%, 95%CI 16% to 23%) than in men (11%, 95%CI 9% to 13%) (P<0.05). The prevalence rates of KOA in different regions were as follows: 11% (95%CI 8% to 14%) in north, 17% (95%CI 15% to 20%) in north-east, 21% (95%CI 13% to 32%) in east, 21% (95%CI 13% to 33%) in north-west, 22% (95%CI 6% to 57%) in south-west, and 18% (95%CI 13% to 23%) in south-central, respectively.ConclusionsThe prevalence of KOA in China is high, and the disease burden is heavy. Due to the quantity and quality of included studies, more high-quality studies are required to verify the above conclusions in future.

  • Prevalence of Depression in Chronic Obstructive Pulmonary Disease and Risk Factors Analysis

    Objective To investigate the prevalence of depression in patients with chronic obstructive pulmonary disease ( COPD) and risk factors. Methods 215 patients with stable COPD were recruited in the study. The Patient Health Questionnaire-9 ( PHQ-9) standardized scale were performed for the depression survey. Meanwhile CAT, pulmonary function test and the history taking ( sex, age, height, weigh, smoking status, education level) were applied to evaluate all the COPD patients. The COPD patients were classified into A, B, C, and D groups according to 2011 GOLD new COPD classification system. Pearson chi-square test and multivariate logistic modeling were used to estimate the prevalence of depression and the risk factors. Results The prevalence of depression in COPD patients whose PHQ-9 scores ≥7 was 37.7% with 18.5% , 35.8% , 46.5% , and 62.5% respectively in A, B, C, and D groups. There was significant difference in prevalence between each groups ( Plt;0.05) . Compared with the COPD patients without depression, the COPD patients with depression had younger age, more female, higher education level, higher CAT scores and smaller means of body mass index ( BMI) . Logistic analysis revealed that depression in COPD was associated with age, sex, education level, BMI, FEV1% pred and CAT scores. The patients who with younger age, female sex, higher education level, lower FEV1% pred, lower BMI and higher CAT scores were had higher rsik to be complicated with depression. However no significant difference was observed between different FEV1 /FVC or smoking status. Conclusions This study confirms the high prevalence of depression in patients with stable COPD, and the prevalence in A, B, C, and D groups of COPD are different. Sex, age, education level, FEV1% pred, BMI, and CAT scores are the risk factors of depression in COPD.

  • An Epidemiological Survey and Risk Factors Analysis of Chronic Obstructive Pulmonary Disease in Jiaxing City of Zhejiang Province

    ObjectiveTo investigate the prevalence of chronic obstructive pulmonary disease (COPD) and related risk factors in two districts and five counties in Jiaxing city of Zhejiang province. MethodsWe randomly selected people over the age of 40 in 10 communities of two districts and five counties in Jiaxing city by cluster sampling and random sampling to investigate the prevalence of COPD from January 2013 to December 2013. All included patients received pulmonary function test at the same time. ResultsA total of 1 256 cases of adults over the age of 40 were included, among which, 1 056 cases with complete data and qualified for pulmonary function test. There were 410 males with mean age of 58.74±11.62, and 646 females with mean age of 59.45±11.72. The overall prevalence of COPD was 13.60%. Male and female prevalence rates were 13.90% and 13.41%, respectively. Divided by age, the prevalence of the age of 40 to 50 was 3.38%, the age of 51 to 60 was 9.57%, the age of 61 to 70 was 19.15%, and the age of above 70 was 29.35%. The results of multivariate logistic regression analysis showed that the main risk factors of COPD were smoking, passive smoking, history of dust exposure, chronic respiratory disease history and age. ConclusionThe total prevalence of COPD in Jiaxing area is high, and the female prevalence rate is especially higher than the national average level, which is the epidemiological characteristic of this area. Because the risk factors of COPD are complex, an integrated control strategy should be adopted.

  • Epidemic Situation and Risk Factors of Chronic Obstructive Pulmonary Disease among Rural Residents in Dayi County of Chengdu

    ObjectiveTo investigate the prevalence and risk factors for chronic obstructive pulmonary disease (COPD) in Dayi County of Chengdu. MethodsRandomly cluster sampled residents between 40 and 70 years of age from two natural villages in the rural communities from February to December, 2010 were included in our study. We used questionnaire survey, physical examination and portable spirometry to collect data. Post-bronchodilator forced expiratory volume in 1 second/forced vital capacity of less than 70% was defined as the diagnostic criterion of COPD. Univariate analysis and logistic regression mode were used to define the risk factors. ResultsA total of 1 017 residents were enrolled in this survey, and 782 participants were valid for analysis, with a valid response rate of 76.89%. There were 330 males and 452 females, with an average age of (51.97±8.17) years old. The overall prevalence of COPD was 10.61%. After population standardization, the prevalence rate was 12.37%, and the overall prevalence increased with increment of age. The prevalence in male (13.33%) was higher than that in female (8.62%) with significant difference (P<0.01) and the prevalence increased with age. Univariate analysis showed that sex, age, education level, smoking and amount of smoking (pack-years) were the risk factors for COPD with significant difference. Logistic regression analysis showed that age, amount of smoking (pack-years) were the main risk factors for COPD. ConclusionCOPD is highly prevalent in Dayi County of Chengdu and it is important to prevent COPD by controlling smoking and improving education level.
