  • Ensuring Medical Supplies for the Medical Rescue after the Wenchuan Earthquake

Objective To summarize the experience of providing medical supplies for the medical rescue after the Wenchuan earthquake so as to provide information for similar tasks in the future. Methods We collected and analyzed relevant data from the Medical Supply Group of the Headquarters for Earthquake Resistance and Disaster Relief. Results ① The Medical Supply Group of the Headquarters for Earthquake Resistance and Disaster Relief was set up immediately after the quake. Professional software for the medical supply information management was developed to ensure the completeness and timeliness of relevant data. ② Within the first 3 days after the quake, 12 460 medical device parts were delivered to the disaster area, which effectively ensured the availability of medical supplies for the “golden 72 hours”. ③ A great amount of donated material from other provinces and overseas also arrived. These donations reached a peak 2 weeks after the quake. A total of 428 411 drug products, 196 504 medical device parts and 5 999.4 tons of disinfection material were successfully delivered to most of the quake-stricken areas within 24 hours after its delivery to Sichuan province. ④ Three months after the quake, the remaining 193 831 drug products, 96 423 medical device parts and 2 000 tons of disinfection material were disposed appropriately. A regional medical supply warehouse was established to reserve 129 895 drug products, 74 018.4 medical device part and 231.28 tons of disinfection material. On the 115th day after the quake, a portion of these reserves were allocated to the quake-hit areas in Panzhihua, Liangshan, and Yunnan province. ⑤ Nearly 20 tons of drug products, 10 tons of medical devices and 724.54 tons of disinfection materials were properly destroyed due to expiration or disqualification. ⑥ Based on the data collected during the disaster relief work, a series of articles were published. • Effect of Lymph Node-Targeted Chemotherapy with Carbon Nanoparticles Absorbing 5-Fluorouracil on Expressions of bcl-2, bax and Caspase-3 in Gastric Cancer Tissues and Metastatic Lymph Nodes

Objective To determine whether lymph node-targeted chemotherapy with carbon nanoparticles absorbing 5-FU affects expressions of bcl-2, bax and caspase-3 in gastric cancer tissues, metastatic lymph nodes and normal gastric mucosa. Methods Twenty-eight patients with gastric cancer in our department were divided into lymph node-targeted chemotherapy (LNTC) group and control group from October 2005 to August 2006. The patients were treated with carbon nanoparticles absorbing 5-FU before operation in LNTC group and those were operated directly in control group. The gastric cancer tissues, metastatic lymph nodes and normal gastric mucosa were collected after operation. The expressions of bcl-2, bax and caspase-3 in those tissues were determined by immunohistochemical technique. Results In LNTC group, the positive expression rate of bcl-2 in gastric cancer tissues and metastatic lymph nodes was significantly lower than those in control group (28.6% vs . 78.6% , 25.0% vs . 70.0% , P < 0.05), the positive expression rate of bax (85.7% vs . 28.6% , 80.0% vs . 30.0% ) and caspase-3 (57.1% vs . 14.3% , 55.0% vs . 15.0% ) in gastric cancer tissues and metastatic lymph nodes was significantly higher than those in control group ( P < 0.05). The positive expression rate of bcl-2, bax and caspase-3 in normal gastric mucosa was not significantly different between two groups ( P > 0.05). Conclusion The lymph node-targeted chemotherapy with carbon nanoparticles absorbing 5-FU can down-regulate the expression of bcl-2 and up-regulate the expression of bax and caspase-3 in gastric cancer tissues and metastatic lymph nodes, and therefore by affecting the expression levels of these apoptosis molecules may be one of the ways to induce tumor cell apoptosis.

  • The awareness and satisfaction of medical aid policy among the poor population in Sichuan province: a cross-sectional survey

ObjectivesTo explore the poor population’s cognition and satisfaction on medical assistance policies in Sichuan province, so as to provide evidence for improving health poverty alleviation policies. Methods A telephone survey was conducted between October and December 2017 among 1 280 poor individuals in Sichuan Province, with multi-stage stratified random sampling. The contents of the survey included general demographics of the poor population, and knowledge and satisfaction of health poverty alleviation policies.ResultsThe awareness rate of medical assistance policy was 91.80%, and the satisfaction rate was 91.88%. Poor individuals from non-poor counties, who had been out of poverty, and who reported that they had not signed up for family doctors, had low awareness of poverty alleviation policies. Poor individuals from non-poor counties, who usually went to the municipal hospital, who reported that they have not signed up for family doctors, and who do not know about health policies for poverty alleviation had a lower satisfaction rate.ConclusionsThe overall awareness rate and satisfaction rate of medical assistance policies in Sichuan province are relatively high, however, there are still some shortage. In the future, more attention should be paid to strengthen the promotion of health poverty alleviation policies for non-poor areas and those who had been lifted out of poverty, speeding up the contract service of family doctors and exploring ways to further alleviate the burden of medical expense of patients with serious diseases.
