  • Choroidal thickness of Chinese population and its relevant factors

    Objective To observe the choroidal thickness and its relationship with age and refraction status in Chinese population. Methods 180 healthy volunteers (360 eyes) were enrolled in this study. Based on the age, the volunteers were divided into A (20-29 years old), B (30-39 year old), C (40-49 years old), D (50-59 year old), E (60-69 year old), and F (70-85 year old) group, with 33, 31, 29, 30, 31, 26 volunteers respectively. And the volunteers were divided into <60 years old group with 123 volunteers and ge; 60 years old group with 57 volunteers. Enhanced depth imaging (EDI) choroidal scans were obtained in all eyes by using spectraldomain optical coherence tomography. Subfoveal choroidal thickness (CT) and CT at 1 mm/3 mm temporal, nasal, superior, inferior to the fovea (S 1 mm, I1 mm, T1 mm, N 1 mm, S, I3 mm, T3 mm, N3 mm) were measured. The differences of CT between different quadrants, genders, eyes and ages were comparatively analyzed. The correlations between age, refraction status and CT in the volunteers of <60 years old and ge; 60 years old group were analyzed. Results The subfoveal CT was (262.78plusmn;84.38) mu;m. The differences were significant between subfoveal CT and all the quadrants CT (P<0.05) except for S1 mm and T1 mm (P>0.05 ). There was no difference between genders or eyes in subfoveal CT (P>0.05 ). There was no difference between A, B, C, D group in subfoveal CT (P>0.05 ). The subfoveal CT of E and F group were thinner than A, B, C, D group (P<0.05). In the <60 years old group, there was a positive correlation between refraction status and CT (r=0.147,P<0.05); but no correlation between age and CT (r=-0.055, P>0.05 ). In the ge; 60 years old group, there was a significant negative correlation between CT and age (r=-0.543, P<0.05), but no correlation between refraction status and CT (r=-0.008, P>0.05). Conclusions The average subfoveal CT in Chinese population was (262.78plusmn;84.38) mu;m. The refraction status is the main influence factors in subjects <60 years old, while the age is the main influence factors in subjects ge;60 years old.

  • Clinical characterisitics of choroidal excavation in the macula

    ObjectiveTo observe the clinical characterisitics of choroidal excavation in the macula. MethodsA total of 22 patients (22 eyes) with choroidal excavation diagnosed by spectral domain high definition optical coherence tomography (HD-OCT) were enrolled in this study. The patients included 12 males (54.50%) and 57 females (45.50%). The age was ranged from 21 to 82 years old, with an average of (41.44±13.17) years. All the patients were affected unilaterally, including 9 right eyes and 13 left eyes. The corrected vision, slit lamp microscope with preset lens, fundus photography, HD-OCT and fluorescence fundus angiography (FFA)were measured for all patients. The clinical characterisitics and concomitant diseases were observed. Seventeen eyes were followed for a period between 3 to 12 months. The lesions change were evaluated by HD-OCT. ResultsThere were 18 eyes (81.8%) with symptoms of micropsia and metamorphopsia, 4 eyes (18.2%) without symptoms. The corrected vision was ranged from 0.3 to 1.2, 12 eyes (54.54%) with moderate or high myopia. Fundus examination presents yellowish-white exudation in 12 eyes (54.54%), yellowish-white exudation accompanied with hemorrhage in 9 eyes (40.91%), grayish yellow reflex halo in 1 eye (4.55%). HD-OCT showed that the retinal pigment epithelium (RPE) layer was involved in the excavation, and the photoreceptor outer segment and pigment junction (OPR) layer was disappeared in all eyes. The external limiting membrane and the photoreceptor inner segment/outer segment junction layer were preserved in 13 eyes (59.09%) and disappeared in 9 eyes (40.91%). There were 10 eyes (18.18%) with a single lesion, 4 eyes (18.18%) with idiopathic choroidal neovascularization, 4 eyes (18.18%) with punctate inner choroidopathy, 1 eye (4.55%) with polypoidal choroidal vasculopathy, 1 eye (4.55%) with macular preretinal menbrance, 1 eye (4.55%) with central serous chorioretinopathy. FFA showed hypofluorescence in early phase, hyperfluorescence in late phase, without obvious leakage. There was no noticeable changes in size and morphological changes in the follow-up period. ConclusionsChoroidal excavation in the macula occurs mostly in middle-aged people with myopia. It can be associated with many fundus diseases. The excavation is located in RPE layer, and OPR layer disappeared. Choroidal excavation in the macula develops slowly.

  • Changes of retinal nerve fiber layer thickness and its correlation with visual field mean defects in early Parkinson′s disease

      Objective To observe the changes of retinal nerve fiber layer (RNFL) thickness and its correlation with visual field mean defects(MD)in Parkinsonprime;s disease (PD).Methods Fifteen eyes of 15 PD patients in early stage and 18 eyes of 18 normal controls undertook RNFL examination by Stratus OCT3. Circular scans (diameter is 3.46 mm) were taken around the optic nerve head including eight quadrants (superior, inferior, temporal, nasal, temporalsuperior, temporalinferior, nasalsuperior and nasalinferior). The RNFL thickness in different quadrants in the two groups was analyzed. The visual field of PD patients was measured by central 302 program of Humphery750 visual field analyzer, and the MD was recorded. The correlation between RNFL thickness and MD was analyzed by linear correlation and regression analysis.Results RNFL thicknesses of superior, inferior, temporal, nasal, temporalsuperior, temporalinferior, nasalsuperior, nasalinferior and average RNFL thickness in the control group were (132.7plusmn;17.4), (141.5plusmn;15.3) ,(83.2plusmn;17.5), (83.7plusmn;22.3) ,(120.8plusmn;21.2), (117.9plusmn;24.5) ,(109.6plusmn;20.6),(110.2plusmn;27.7), and(109.9plusmn;8.5)mu;m respectively, while in the PD group they were (128.1plusmn;25.3) , (128.6plusmn;13.2) , (68.7plusmn;13.5) , (76.5plusmn;17.8) ,(102.6plusmn;23.7), (103.3plusmn;14.1) ,(101.2plusmn;20.9),(96.6plusmn;15.0),(102.3plusmn;11.9) mu;m. Compared with each other, the differences of RNFL thickness of inferior, temporal, temporalsuperior, temporalinferior and average RNFL thickness were statistically significant(t=2.595,2.700,2.330,2.153,2.131;P=0.014,0.011,0.026,0.040,0.041). There was a close negative relationship between average RNFL thickness and MD in PD patients (r=-0.933,P<0.0001). Conclusions RNFL thickness was significantly thinner in PD patients than that in the normal controls. There was a negative relationship between RNFL thickness and MD in PD patients.

  • Three-dimensional computed tomography reconstruction of the eyes before and after removing the scleral encircling buckles

      Objective To measure the changes of eye shape and axial length of the eyeball before and after removing the scleral encircling buckles.Methods This is a prospective and controlled study. Twenty eyes (20 patients) with rhegmatogenous retinal detachment and the fellow eyes were enrolled in this study. All patients underwent scleral encircling buckling, and the buckles were removed 2.0-3.5 years after the surgery. The eye shape and axial length of both eyes were measured by three-dimensional computed tomography (3D-CT) before and one,three,six months after the removing surgery. The axial length was also measured by intraocular lens (IOL) Master.Results 3D-CT showed that buckled eyeball depressed at the equator, resulting in a gourd-shaped eyeball. One month after removing the encircling buckle the depression disappeared. By 3D-CT scanning, the axial lengths of buckled eyes were (27.65plusmn;1.22), (27.3plusmn;1.56), (27.29plusmn;1.46) and (27.12plusmn;1.49) mm before and one, three, six months after the removing surgery respectively. The difference between before and after removing surgery was not statistically significant (t=2.89,P=0.723). By IOLMaster, the axial length of operated eyes were (28.32plusmn;1.94), (28.17plusmn;1.87), (28.21plusmn;1.94), (28.25plusmn;1.93) mm respectively. The difference between before and after removing the encircling band was not statistically significant (t=3.304, P=0.93). There was no significant difference in these two measuring modes (t=3.705,P=0.847).Conclusions Encircling buckling can cause eyeball indentation, removing the encircling band can rescue the indentation. There are no changes in the axial length before and after removing the encircling buckles.

  • Consistency analysis of optical coherence tomography angiography and fundus fluorescein angiography in the diagnosis of central retinal vein occlusion

    ObjectiveTo assess the consistency of diagnostic results using optical coherence tomography angiography (OCTA) and fundus fluorescein angiography (FFA) in the central retinal vein occlusion (CRVO). MethodsThis is a retrospective case series of 26 eyes of 26 patients with CRVO. There were 10 females (10 eyes) and 16 males (16 eyes). The mean age was (49.19±10.50) years. The mean course of the disease was (27.81±21.60) days. Simultaneous OCTA and FFA were performed in all patients using 7-standard field of Early Treatment Diabetic Retinopathy Study (ETDRS) to evaluate the microaneurysms, nonperfused areas, optical disc/retinal neovascularization and macular edema. The consistency was evaluated using weighted Kappa statistic values. Kappa≥0.75, consistency is excellent; 0.60≤Kappa < 0.75, consistency is good; 0.40≤Kappa < 0.60, consistency is general; Kappa < 0.40, consistency is poor. ResultsBased on OCTA, microaneurysms were found in 23 eyes, nonperfused areas in 16 eyes, optical disc/retinal neovascularization in 8 eyes and macular edema in 21 eyes. Based on FFA, 23 eyes were diagnosed to have microaneurysms, 14 eyes have nonperfused area, 8 eyes have optical disc/retinal neovascularization, 22 eyes have macular edema. The consistency was excellent for microaneurysms and optical disc/retinal neovascularization (Kappa=0.772, 0.766; P < 0.01), good for nonperfused areas and macular edema (Kappa=0.703, 0.600,P < 0v01). ConclusionThere is high consistency between OCTA and FFA in the diagnosis of microaneurysms, macular edema, nonperfused areas and optical disc/retinal neovascularization in CRVO patients.

  • Effects of local foveal photoreceptor defect on visual acuity

    Objective To observe the effects of local macular foveal photoreceptor defects on visual acuity.Methods Thirty-one patients (31 eyes) with photoreceptor defect in macular fovea (case group) diagnosed by spectral domain optical coherence tomography (SD-OCT) and 30 patients (30 eyes) age- and diopter- matched normal subjects (control group) were enrolled in this study. There were 22 eyes with full photoreceptor defects and 9 eyes with outer segment defects in case group. All subjects were examined for best corrected visual acuity (BCVA), slit-lamp microscopy, direct ophthalmoscope and SD-OCT. Independent sample t-test was used to compare central foveal thickness (CFT) between case group and control group. Difference of logMAR BCVA, CFT, maximum width and height of photoreceptor defects, defected area and residual retinal thickness in macular between patients with full photoreceptor defects and outer segment defects were also compared.Results The CFT of case group and control group were (225.32plusmn;19.70),(240.02plusmn;10.70) mu;m, the difference was not statistically significant (t=-1.96, P>0.05). In full photoreceptor defects and outer segment defects patients, the mean logMAR BCVA were 0.22plusmn;0.31, 0.32plusmn;0.43; the mean CFT were (224.09plusmn;20.57), (228.33plusmn;18.17) mu;m; the maximum width of photoreceptor defects were (131.32plusmn;108.18), (143.22plusmn;66.93) mu;m; the mean defected area were (0.022plusmn;0.054), (0.019plusmn;0.019) mm2; the mean maximum height of photoreceptor defects were (77.41plusmn;6.62), (44.89plusmn;4.26) mu;m; the mean residual retinal thickness were (87.00plusmn;20.31), (128.33plusmn;23.54) mu;m respectively. There was no statistical significance between full photoreceptor defects and outer segment defects patients in the mean logMAR BCVA, CFT, maximum width of photoreceptor defects and defected area (t=-0.76, -0.538, -0.305, 0.166; P>0.05), but there were significant difference in mean maximum width of photoreceptor defects and residual retinal thickness (t=12.72, -4.91;P<0.05). Conclusions The local photoreceptor defects in macular fovea can lead to decrease of visual acuity. The wider the photoreceptor defects, the worse the visual acuity.

  • Macular thickness measurement of six-year-old children and analysis of its relevant factors

      Objective  To observe the distribution of macular thickness and its relationship with axial length, gender, refraction status and race in six-year-old children. Methods 1543 six-year-old children from 34 schools in Sydney were randomly selected. The thickness of center macula(CM), inner macula(IM)and outer macula(OM)were measured by optical coherence tomography(OCT), the differences of macular thickness between different genders and races were comparatively analyzed. The axial length and refraction status of all children were measured by IOL Master and autorefractor RK-F1 respectively. Results The CM, IM and OM thickness of all those six-year-old children were normally distributed,on average they were 193.6, 264.3 and 236.9 mu;m respectively,the differences were statistically significant(t=0.40,0.08,0.20;P<0.05). The CM,IM and OM average thickness was 194.2,264.9,229.0 mu;m for boys and 189.3,262.5,231.5 mu;m for girls respectively. CM and IM thickness of boys were thicker than those in girls(t=0.15,0.11;P<0.05). The CM,IM and OM average thickness was 196.0,265.2,237.5 mu;m for White kids,186.0,262.3,236.5 mu;m for East Asia kids and 19.4,263.7,233.8 mu;m for Central East kids respectively. The CM and IM thickness of White and Central East children were significantly thicker than East Asia children (t=0.12,0.15;Plt;0.05). There was no difference between these three races in OM thickness(t=0.16,P=0.6). The IM and OM were thinner with longer axial length (Plt;0.05). The CM,IM and OM thickness was thicker with higher hyperopic degrees (Plt;0.05). Conclusions Macular thickness was normally distributed in six-year-old children and was significantly affected by gender, race, axial length and refraction status.

  • Ophthalmoscopic image characteristics and relation with vision and course of disease in nonarteritic anterior ischemic optic neuropathy

      Objective To observe the characteristics of fundus fluorescein angiography(FFA)and optical coherence tomography(OCT)in nonarteritic anterior ischemic optic neuropathy (NAION),and investigate its relation with visual acuity and course of disease.Methods The clinical data of 47 patients (47 eyes) with NAION were retrospectively analyzed. All the patiens had undergone visual acuity,fundus and visual field examination,meanwhile FFA and OCT were carried out at first visit. FFA and visual field were carried out by routine. OCT was carried out by line and circle shape scanning in macula and optic disc. Thirtyfive NAION patients were checked with OCT at half, one, two, three and six month after onset in respectively. Take the healthy fellow eyes of 36 NAION patiens as control group.The FFA,visual field,OCT characteristics and relation with visual acuity and course disease were comparatively analyzed.ResultsFFA showed that all the eyes appear as delayed filling of the optic disc in early stage and hyperfluorescence leakage of the optic disc in late stage,besides hyperfluorescence presented to macular area in 24 eyes. OCT showed that optic papilla swelling and physiological depression narrow or nearly disappearance, neuroepithelial layer thickening or neuroepithelial layer eminence and subretinal fluidity area opaca between optic disc and macula. There were 14 eyes with normal physiological depression and 22 eyes with small physiological depression or non physiological depression in control group. Half month after onset,the neuroepithelial layer thickness of macula fovea, the maximum thickness of neuroepithelial layer between optic disc and macula,and the average retinal nerve fiber layer(RNFL)thickness in NAION group were higher than those in the control group,the difference were statistically significant (F=6.51,26.12,75.49;P<0.05).Two months after onset,the maximum thickness of neuroepithelial layer between optic disc and macula, the average RNFL thickness, and the RNFL thickness of temporal optic disc in NAION group were significant thinner, but the elevated height of the optic disc in NAION group were near those of the control group. Three months after onset,the average RNFL thickness and the RNFL thickness of temporal optic disc in NAION group decreased continually, they were lower than those of the control group, the difference were statistically significan(F=75.49,37.92;P<0.05).Visual field showed that inferior defect were found in 21 eyes (45%). With progress, the superior RNFL thickness obviously decreased, coincidence with appearance of visual field. It indicate that the superior optic atrophy serious. Visual acuity had significant negative correlation with the neuroepithelial layer thickness of macula fovea, the neuroepithelial layer maximum thickness between optic disc and macula, the average RNFL thickness, the RNFL thickness of temporal optic disc(r=-0.394,-0.424,-0.412,-0.464;P<0.05).Conclusions FFA showes that hyperfluorescence leakage appearanced in part macula. OCT showes that RNFL becomes thinner as the disease duration increases. The results of OCT and visual field examinaion in the configuration of optic disc and changes of RNFL are accordant.

  • The preliminary application of intraoperative optical coherence tomography in vitreous retinal surgery

    ObjectiveTo observe the preliminary application of intraoperative optical coherence tomography (iOCT) in vitreous retinal surgery and the influencing factors of the imaging quality. MethodsA retrospective case study. A total of 132 subjects were chosen in June 2015 and April 2016 in our hospital. All the subjects were taken vitreous retinal surgery and iOCT scanning. The patients included 46 males and 86 females, with an average age of 61.7 years. 111 cases had macular diseases and 21 cases had vitreous hemorrhage. Cases are divided into high myopia and non-high myopia group according to ocular axial length (AL), 28 eyes in high myopia group (AL≥26 mm),104 eyes in non-high myopia (AL<26 mm).The scanning time, image quality and eye conditions were observed during the operation. The discordance between surgeon microscope visualization of the pathology and the findings of the iOCT and the postoperative adverse reactions were also observed. ResultsThe iOCT images were identified in 124 of 132 subjects, while were not identified in 8 eyes. For the macular area morphology change, iOCT tips and surgeon judgment rate was inconsistent in 22 eyes (16.7%). For the operation guidance value, change the operation rate of 12/132,accounted for 9.1%. The iOCT imaging quality of high myopia groups was lower than the non-high myopia groups (χ2=17.13, P=0.001). Corneal edema and operation time were considered as influencing factors on the quality of the imaging in the non-high myopia groups (r=3.75, 6.18; P=0.049, 0.013). There were no complications such as endophthalmitis. ConclusionsThe surgeon can observe morphological changes in the macular area through iOCT which is difficult to be observed by operating microscope and selected reasonable operation method during the surgery. High myopia, corneal edema and operation time affect the image quality.
