  • The value of three triage methodologies for critically ill patients in earthquake mass casualty

    ObjectiveCurrently, simple triage and rapid transport (START) is widely used as a method to evaluate the severity of mass casualty events in a disaster. Modified physiological triage tool (MPTT) and modified physiological triage tool-24 (MPTT-24) are newly introduced triage methodologies that may offer improvements over START. However, the feasibility and accuracy of these two methods cannot be adequately assessed without sufficient data support, whether in earthquakes or other disasters. Our study aimed to analyze the value of the three triage methodologies in mass casualty events due to earthquakes.MethodsA total of 36 604 injured patients from the West China Hospital database were evaluated using START, MPTT and MPTT-24, respectively. The triage methodologies were then evaluated based on death and ICU acceptance, using the area under the receiver-operator curve (AUC). The sensitivity and specificity of the three methodologies were compared under different standards and correlations with the injury severity score (ISS) were analyzed. ResultsFor deaths, the AUCs for the triage methodologies were 0.711, 0.775 and 0.686 for START, MPTT, and MPTT-24, respectively. For ICU acceptance, the AUCs of the triage methodologies after correction for bias were 0.579, 0.618 and 0.603. The correlation coefficients of the triage methodologies and ISS score were 0.041, 0.087, and 0.115.ConclusionsMPTT is superior to START and MPTT-24 in the evaluation of critically ill patients in mass casualty events caused by earthquakes.

  • Biomechanical characteristics of lower limbs after discoid lateral meniscus injury surgery

    ObjectiveTo review the research progress on the lower limb biomechanical characteristics of patients with discoid lateral meniscus (DLM) injury after surgery. MethodsBy searching relevant domestic and international research literature on DLM, the postoperative characteristics of knee joint movement biomechanics, tibiofemoral joint stress distribution, lower extremity force line, and patellofemoral joint changes in patients with DLM injury were summarized. ResultsSurgical treatment can lead to varying degrees of changes in the lower limb biomechanical characteristics of patients with DLM injury. Specifically, the kinematic biomechanics of the knee joint can be significantly improved, but there are still problems such as extension deficits in the affected knee joint. The peak stress of the tibiofemoral joint decreases with the increase of the residual meniscus volume, and the degree of change is closely related to the residual meniscus volume. Preserving a larger volume of the meniscus, especially the anterior horn volume, helps to reduce stress concentration. The lower extremity force line will deviate outward after surgery, and the more meniscus is removed during surgery, the greater the change in the lower extremity force line after surgery. There are conditions such as cartilage degeneration, position and angle changes in the patellofemoral joint after surgery. ConclusionThe changes in the lower limb biomechanical characteristics after DLM injury are closely related to the choice of surgical methods and rehabilitation programs. However, the mechanisms of biomechanical changes in multiple lower limb joints and individual differences still need to be further studied and clarified.

  • Research progress of pancreas stellate cells in pancreas fibrosis

    Objective To review research progress of pancreas stellate cells in pancreas fibrosis and understand characteristics and activation of pancreas stellate cells and its mechanism on pancreas fibrosis. Method The relevant literatures about pancreas stellate cells and its studies in pancreas fibrosis were reviewed. Results The activation of pancreatic stellate cell is associated with fibrosis of pancreatitis and end stage of pancreas transplantation, but its effect and regulation mechanisms for the extracelluar and intracellular molecular network need to be further investigated. Conclusion Elucidation of activation of pancreas stellate cells will facilitate understanding of pancreas fibrosis and searching new target in treatment of pancreas fibrosis.

  • Visual analysis of domestic and international thyroid cancer nursing research based on CiteSpace

    ObjectiveTo explore the research hotspots of thyroid cancer nursing, and provide reference and guidance for future research on thyroid cancer nursing. MethodRelevant literature on thyroid cancer care was retrieved through the China National Knowledge Infrastructure (CNKI) and Web of Science (WOS) Core Database, and then analyzed the data using CiteSpace 6.2R4 software. ResultsThis study included a total of 1 096 Chinese literature and 263 English literature. The total number of publications was on the rise. The top 10 keywords in CNKI were: surgery, anxiety and depression, perioperative care, quality of life, complications, postoperative care, clinical pathway, rapid recovery surgery, differentiated thyroid cancer, and psychological care. Except for rapid recovery surgery, the centrality of other key words was greater than 0.1. The top 10 keywords in WOS were: quality of life, management, 131I, papillary thyroid cancer, surgery, impact, anxiety, prevalence, risk factor, and differentiated thyroid cancer. Among them, quality of life played an important intermediary role in co-occurrence networks, with the highest centrality of 0.34. ConclusionsQuality of life, mental health, perioperative period, and papillary thyroid cancer are research hotspots in thyroid cancer care both domestically and internationally. The management of quality of life and symptom management are the development trend of future research. Further research on thyroid cancer care needs to be strengthened, and important measures should be taken to address clinical issues and promote the recovery of thyroid cancer patients. We should strengthen domestic multi center cooperation, expand the breadth and depth of research, and promote the prosperous development of thyroid cancer nursing.
