  • CONSTRUCTION OF INDUCIBLE LENTIVIRAL VECTOR CONTAINING HUMAN BONE MORPHOGENETIC PROTEIN 2 GENE AND ITS EXPRESSION IN HUMAN UMBILICAL CORD BLOOD MESENCHYMAL STEM CELLS

    Objective To construct inducible lentiviral vector containing human bone morphogenetic protein 2 (hBMP-2) gene and to study its expression in human umbil ical cord blood mesenchymal stem cells (HUMSCs). Methods hBMP-2 gene was ampl ified by PCR from a plasmid and was cloned into pDown by BP reaction. pLV/EXPN2-Neo-TRE-hBMP-2 and pLV/EXPN2-Puro-EF1A-reverse transactivator (rtTA) were obtained with GATEWAY technology, and then were sequenced and analyzed by PCR. The recombinant vectors were transfected into 293FT cells respectively through l ipofectamine, and the lentiviral viruses were harvested from 293FT cells, then the titer was determined. Viruses were used to infect HUMSCs in tandem. In order to research the influence of induction time and concentration, one group of HUMSCs was induced by different doxycl ine concentrations (0, 10, 100 ng/mL, and 1, 10, 100 μg/mL) in the same induction time (48 hours), and the other by the same concentration (10 μg/mL) in different time points (12, 24, 48, and 72 hours). The expression of target gene hBMP-2 was indentified by ELISA method. After 2-week osteogenic induction of transfected HUMSCs, the mineral ization nodes were detected with Al izarin bordeaux staining method. Results Therecombinant inducible lentiviral vectors (pLV/EXPN2-Neo-TRE-hBMP-2 and pLV/EXPN2-Puro-EF1A-rtTA) were successfully constructed. The lentiviruses were also obtained and mediated by 293FT cells, and the virus titers were 3.5 × 108 TU/mL and 9.5 × 107 TU/mL respectively. HUMSCs could expression hBMP-2 by induction of doxycycl ine. The expression of hBMP-2 reached the peak at 10 μg/mL doxycl ine at 48 hours of induction. After 2-week osteogenic induction, a lot of mineral ization nodes were observed. Conclusion The recombinant inducible lentiviral vectors containing hBMP-2 gene can be successfully constructed, which provide an effective and simple method for the further study of stem cells and animal experiment in vivo.

  • APPLICATION OF BLOCKING VESSELS IN OPERATIVE THERAPY OF NON-LIMB HEMANGIOMA

    Objective To study the surgical method to reduce bleeding in treating hemangioma at non-limb sites. Methods From November 1998 to November 2003,49 cases of non-limb hemangioma were treated, aged 3 months to 63 years, including 21 males and 28 females. There were 14 cases of capillary hemangioma, 25 cases of cavernous hemangioma, 7 cases of arterial racemose angioma and 3 cases of mixture hemangioma. According to the position and type of hemangioma, the various methods of blocking blood vessels were adopted to assist resect tumors. Afterthe pulsatile artery was felt in arterial racemose angioma of neck and face by palpation, we sutured and knotted it with 7-0 silk string to block the bleeding.We found out the common iliac artery or external iliac artery or femoral arteryand blocked them temporarily to resect arterial racemose angioma in inguen and thigh. We sutured and knotted vessel with 7-0 silk string to block the bleedingin capillary hemangioma and cavernous hemangioma of neck and face and truncus. Results Intraoperative bleeding obviously decreased and the tumor size reducedto various extent. Of the 49 cases, 47 cases achieved complete success, 2 casesbled within two days after operation. A postoperative follow-up of 6 months to4 years showed that the appearance and function were satisfactory. Conclusion The preoperative method of blocking blood vessels obviously can reduce intraoperative bleeding and decrease operative difficulty, which makes it possible to eradicate hemangioma and lower recurrence rate.

  • Clinical Application of Autologous Peripheral Blood Stem Cells Transplantation in Treatment for Thromboangiitis Obliterans

    Objective To investigate the efficiency and safety of autologous peripheral blood stem cell transplantation (ABSCT) in treatment for thromboangiitis obliterans. Methods Fifty patients (62 affected limbs) with thromboangiitis obliterans were treated by ABSCT. A series of subjective indexes including improvement of pain and cold sensation and objective indexes including intermittent claudication distance, ankle brachial index (ABI), skin temperature, and improvement of foot skin ulcer were evaluated. Results Due to necrosis in middle and lower part of leg, 4 of 50 patients (4 lower limbs) were taken extremity amputation on 3 weeks after ABSCT, 46 patients kept their legs successfully. On 1 month after ABSCT, the legs pain and cold sensation of 46 patients (58 affected limbs) vanished, and the score of feet pain and cold sensation after ABSCT were better than those before ABSCT (P<0.05). The intermittent claudication distance, skin temperature, and ABI of 46 patients with kepting their legs on 3 months after ABSCT significantly increased as compared with before ABSCT 〔intermittent claudication distance:(80.38±45.53) m versus (330.56±142.31) m;skin temperature:(26.50±0.46) ℃ versus (31.49±0.45) ℃;ABI:0.41±0.02 versus 0.71±0.05〕, the differences were statistically significant(P<0.05). Six months after ABSCT, different degree neonatal lateral vessels were found in 58 affected limbs of 46 patients by lower extremity arteriography. The complications were not found in all the patients by laboratory or CT detection, such as malignant tumors, retinal hyperplasia, aneurysm and so on. After ABSCT, 40 patients were followed up for 9 to 36 months (mean 22.5months), the symptom had improved. Due to leg pain aggravated after 6 months, score of pain feelings was 4 in 6 patients and with toe ulcers, who had ABSCT again. Eighteen months after transplantation, the patients had only debilitation of lower extremity. The pain feeling was improved (score of pain feeling was 1). The toe ulcer was healed and no angiosclerotic myasthenia happened. Conclusions ABSCT is a simple, safe, and effective method, especially in treatment for patients with severe lower limb ischemia who is no arterial reconstruction is feasible. It could improve the quality of life of patients and might be avoided amputation of lower extremity or foot.

  • Clinical application of descending hilar plate technology in laparoscopic heminephrectomy for intrahepatic bile duct calculus

    ObjectiveTo investigate the clinical application effect of descending hilar plate technology in laparoscopic heminephrectomy for intrahepatic bile duct calculus.MethodsThe clinical data of 40 patients with intrahepatic bile duct calculus who underwent laparoscopic heminephrectomy in our hospital from January 2015 to December 2019 were retrospectively analyzed. The patients were grouped according to different surgical procedures, 21 patients with Pringle method of total hepatic vascular exclusion were classified in the control group, and 19 patients with descending hilar plate technology of blood occlusion technology were classified in the observation group. The operation time, intraoperative bleeding volume, postoperative hospital stays, liver function recovery, and postoperative complications were compared between the two groups.ResultsThere was no statistically significant difference between the two groups in the intraoperative bleeding volume and operation time (P>0.05), but the postoperative hospital stays in the observation group shortened (P=0.025). The changes on the ALT, TB, and AST in the observation group was obvious than those of the control group (P<0.05). There was no statistically significant incidence between the two groups in the total incidence of complications (P=0.128).ConclusionsCompared with Pringle method of total hepatic vascular exclusion, descending hilar plate technology in laparoscopic heminephrectomy can fully expose the Glisson pedicles of the left and right livers, and it is convenient to implement hemihepatic blood flow occlusion. It has less damage to healthy side of the liver and quicker liver function recovery, and it can reduce postoperative complications and shorten postoperative hospital stay.

  • The Impact of Palliative Operation on Body and Growth of Pulmonary Arteries in Patients with Congenital Heart Diseases of Diminutive Pulmonary Blood

    Objective To investigate the impact of three kinds of palliative operation on the body and growth of pulmonary artery in patients with congenital heart diseases of diminutive pulmonary blood. Methods Clinical data was reviewed in 28 cases of congenital heart diseases with diminutive pulmonary blood who had been performed cavopulmonary connection (n = 9), systemic-pulmonary shunt (n = 8 ), and palliative reconstruction of right ventricular outflow tract (n=11). The period between re-hospitalized and the first was 5-54 months (19.07±10. 06 months ). Hematocrit (HCT), hemoglobin (Hb), percutaneous oxygen saturation (SpO2), body surface area (BSA), and pulmonary artery index (PAI) etc. were observed both before palliation and before the second operation. Results After the second hospitalization, there were 7 cases of death from hemorrhage, failure of circulation and extracorporeal circulation accident etc. The time of respirator, intensive care unit and total amount of dopamine in patients of palliative reconstruction of right ventricular outflow tract were longer and more than those in patients of cavopulmonary connection (P〈0. 05). HCT, Hb before the second operation were decreased than thoes before palliative operations in all patients, SpO2, BSA and PAI increased significantly (P 〈 0. 01 ). Before the second operation, BSA of patients with cavopulmonary connection, BSA and PAI of patients with systemic-pulmonary shunt, SpO2, BSA and PAI of patients with palliative reconstruction of right ventricular outflow tract were increased than those before palliative operations(P〈0. 01). HCT of palliative reconstruction of right ventricular outflow tract was decreased(P〈0. 05). Conclusion This results suggests that pulmonary blood of patients with congenital heart diseases of diminutive pulmonary blood can be increased, development of pulmonary arteries can be improved efficiently by systemic-pulmonary shunt and palliative reconstruction of right ventricle outflow tract, but it can not be found in cavopulmonary connection patients.

  • VASCULARIZED FIBULAR HEAD TRANSPLANTATION IN THE TREAT-MENT OF GIANT CELL TUMOR OF THE LOWER END OF RADIUS

    From 1979, a total of 5 cases of giant cell tumor of the lower end of radius were treated by segmental resection, and vascularized fibular head transplantation, and reconstruction of the inferior radio-ulnar joint. The bone healed within 2-3 months. The patients were followed for 5-10 years. There was no recurrence, nor distant metastasis, and the functional recovery of extremities was satisfactory.The clinical materials, the operative techniques and the assessment of the long-term results were introduced.

  • Characteristics of Coronary Artery in Patients with Hypertension and Coronary Heart Disease

    Objective To investigate the characteristics of blood pressure and coronary artery impairment in patients with essential hypertension (EH) combining coronary heart disease (CHD). Methods A total of 358 patients with EH combining CHD and other 144 patients with CHD were measured with ambulatory blood pressure monitoring (ABPM), and the parameters of ambulatory blood pressure were analyzed. All the patients underwent coronary angiography. The severity of coronary artery stenosis was evaluated in accordance with the number of impaired arteries. Results Compared to the patients with simplex CHD, those with EH combining CHD had much heavier artery stenosis and more diffuse lesions, with significant differences (χ2=6.03, P=0.019). The 24h systolic blood pressure (SBP), day SBP, night SBP, 24h pulse pressure (PP), day PP and night PP were higher in the patients with EH combining CHD compared to those of the patients with simplex CHD (The t values were 2.580, 2.045, 2.675, 2.037, 2.601, and 1.995, respectively, while the P values were 0.015, 0.037, 0.009, 0.041, 0.017, and 0.047, respectively). Conclusion Compared to the patients with simplex CHD, the patients with EH combining CHD suffer from much severe coronary artery impairment, so a good controlling of blood pressure is advisable to improve the coronary artery impairment for the patients with EH combining CHD.

  • EMERGENCY AND TREATMENT OF FIREARM WOUNDS OF PERIPHERAL BLOOD VESSELS

    From March 1979 to February 1987, 500 cases of firearm wounds of blood vessels were treated. Of them, 465 cases were recovered, 15 cases were disabled, 13 cases had amputation, and 7 cases died.   • Expression of CK20 mRNA in Peripheral Blood of Patients with Colorectal Cancer and Its Clinical Significance

    Objective To detect expression of CK20 mRNA in peripheral blood of patients with colorectal carcinoma and its clinical significance. Methods Using the reverse transcriptase-polymerase chain reaction (RT-PCR),CK20 mRNA expression was examined in peripheral blood from 42 patients with colorectal carcinoma before and after operation, 20 healthy volunteers, 20 fresh colorectal carcinoma samples. Results The positive expression rates of CK20 mRNA were 45.24%(19/42) and 33.33%(14/42) before and after operation in 42 colorectal carcinoma patients respectively. All 20 fresh colorectal carcinoma samples revealed expression of CK20 mRNA, but the 20 normal blood samples did not. Conclusion The detection of CK20 mRNA in peripheral blood is helpful to early diagnose, assess the prognosis and make a correct treatment of colorectal carcinoma.

  • RECONSTRUCTION OF THUMB BY VASCULARIZED SECOND METATARSO-PHALANGEAL JOINT AND BIG-TOE NAIL COMPOSITE GRAFT

    Since Oct. 1990, the 2nd metatarso-phalangeal joint and big-toe nail composite graft with the neuro-vascular bundle was transplanted to reconstruct the thumb in 4 cases. The transplants were all survived. The follow-up through 5 months, a comparatively good function and appearance were achieved.The applied anatomy, the surgical technique and the matters needing attention were detailed.
