  • THE RECONSTRUCTION OF LARGE BONY DEFECT IN UPPER LIMB AFTER RESECTION OF TUMOR

Seven cases with bone tumor in upper limb were reported. Five cases were treated by using free vascularized fibular graft, 2 cases by using fusion between humorus and clavicle. A follow-up study of six patients showed that the graft bone was united within 3 months in 5 cases, in 6 months in one case. Partial function of upper limb in 6 patients have been restored.

  • RETROAURICULAR ISLAND FLAP FOR EYE SOCKET RECONSTRUCTION

This paper reported the use of superficial temporal vessels pedicled postauricular island flap for the reconstruction of eye socket. Six cases were treated by this method since 1988 with universal satisfactory results. The disign of the flap was diseribed. The operative procedure was detailed, and the advantages of the flap were evaluated: 1. The donor defect was hidden behind the ear; 2. The flap has a relatively long pedicle, hence easy for distant transfer, and 3. the blood supply was highly reliable.

  • USE OF RANDOM PEDICLED OVER-THIN FLAP IN HAND INJURY

Eighteen cases with hand injury were repaired by 20 pediclcd over-thin skin flaps with success. The area of pedicled skin flap is from 1.5×1.2 to 12×8.5cm. The pedicles of skin flap were divided at 5-7 days postoperation after clamp training of the pedicle, and the flap all survived. The operative method is introduced and the mechanism of skin flap survival is discussed.

  • DELOYERS OPERATION FOR LONG TYPE HIRSCHSPRUNG S DISEASE

Deloyers method was pcrformed in 17 cases from 1968-1991 with satisfactory results. Some technical problems were discussed. The author recognized that the technique was excellent because the ileocecal valve and part of the ascending colon were preserved. Thus ensuring the absorption of water, the electrolyses and the nutrition. This method were used for the long type Hirchsprung s disease, however, could also be used in some common types and the cases equiring resection part of the rect, sigmoid colon, descending colon and the transverse colon.

  • REGIONAL ANATOMIC STUDY OF SUBSCAPULAR ARTERY PEDICLED GRAFT OF AXILLARY BORDER OF SCAPULAR IN REPAIR OF DEFECT OF HUMERUS

Fifty cases of adult cadaveric subscapular arteries and its circumflex scapulae branches and from 102 of adult skelet on the axillary border of scapulae were measured. The results were that: the external diameter of the subscapular artery was 4.3cm with the length of 2.9cm; the axillary border of the scapula was 13.4cm; the thicknesses of the upper 1/3, middle 1/3, and lower 1/3were 13.9cm,11.0cm, and 7.4cm respectively. From the results of measurement, the subscapular artery pedicled graft of axillary border of scapular could be designed for repair the defects of upper or middle ments of humerus.

  • ARCUS VENOSUS DORSALIS PEDIS GRAFTS TO RECONSTRUCT THE SUPERFICIAL PALMAR ARCH IN INJURIES OF HAND

This article reported 4 cases of reconstrution of the superficial palmar arch from transplantation of the vein arch of the dorsalis pedis by microsurgery. This technique was best reserved for those having significant damage of the palmar vesels. There were no arterial failures in these patients. The operative indications and some problems about the arterialized vein arch were discussed.

  • Retinal degeneration in transgenic mice induced by oncostatin M through gp130/OSMRβ receptor

ObjectiveTo determine the signal pathway of specifically expressed oncostatin M(OSM) in lens inducing retinal degeneration in transgenic mice.MethodsA sequence-truncated OSM cDNA (661 bp) of mice was linked to αA-crytallin promoter, and was micro-injected into unicellular embryo to set up the model of transgenic mice. Reversal transcription-polymerase chain reaction (RT-PCR) was used to detect the mRNA expression of gp130/OSMRβ receptor in the retinae of OSM transgenic and non-transgenic mice. Rabbit anti-phosphorylated STAT-3 antibody was used to detect the protein expression of phosphorylated STAT-3,and mouse anti-cytochrome C antibody was used to detect the distributing of cytochrome C in retinae. ResultsExpression of gp130/OSMRβmRNA was found in retina of non-transgenic mice. At the 17.5th day in the embryonic stage, significant accumulation of the phosphorylated STAT-3 was detected in the retinal nucleolus in OSM transgenic retina. At the first day after birth, intensive staining of cytochrome C in OSM transgenic retina was found. Conclusionsspecifically expressed OSM in lens may act on gp130/OSMRβ receptor in retinae, activate STAT-3, and cause the release of cytochrome C from mitochondria, which eventually induces widespread retinal degeneration.(Chin J Ocul Fundus Dis, 2005,21:167-169)

  • ROTATIONAL LANDMARKS AND TOTAL KNEE ARTHROPLASTY IN OSTEOARTHRITIC KNEES

Objective To investigate the rotational mismatch of total kneereplacement with medial 1/3 of tibial tuberosity as bony landmark in osteoarthritic patients with varus or valgus deformity. Methods Axial images on computed tomography of 62 knees (including 55 varus deformities and 7 valgus deformities) in 32 Chinese osteoarthritic patients who had total knee arthroplasty were analyzed, compared with that of 10 healthy knees. On images of the distal femur, the angle between the lines of surgical epicondylar axis(SEA) and posterior condylar axis was measured as posterior condylar angle (PCA), and on images of the proximal tibia, a baseline for the anteriorposterior axis of each component was drawn based on the SEA for the femur and the medial 1/3 of the tibial tuberosity for the tibia. The angle between these lines (Angle α) was defined as therotational mismatch between the components when they were aligned to the anatomic landmarks of each bone. Results The sulcus of medial epicondyle of femur could be identified on CT images of over 80% osteoarthritic knees; the median value of PCA was +2.36°, with an individual variation of 0° to +7.5°. Angle α was +6.45±3.68°(range, 0° to +11.8°) in 10 healthy knees, which increased significantly to +10.85±10.47°(range, 0° to +28.1°)in 55 varus knees (P<0.05), which also increased significantly to +11.6±7.3°(range, -6.5° to +26.8°) in 7 valgus knees (P< 0.05). Conclusion With the medial 1/3 of the tibial tuberosity as the rotational landmark for the tibial component, there was a tendency to align the tibial component in external rotational position relative to the femoral component in knees with normal alignment, the rotational mismatch increased in Chinese osteoarthritic knees with varus and valgus deformity.
