  • Characteristics of exudative age-related macular degeneration optical coherence tomography

    Objective To verifying the characteristics of optical coherence tomography(OCT) in exudative age-related macular degeneration (AMD). Methods The patients being investigated in this series included 16 cases (19 eyes) of exudative age-related macular degeneration diagnosed by FFA and OCT examinations, among them 4 cases (6 eyes) were examined with ICGA. The color photographs of ocular fundi, FFA, ICGA and OCT were investigated by contrasting each other. Results As compared with the FFA and ICGA examinations, the characteristic findings found in OCT in patients with exudative AMD in this series were as the following:①serous detachment of neurosensory epithelium in 11 eyes,②retinal hemorrhage in 2 eyes,③serous detachment of retinal pigment epithelium in 5 eyes,④hemorrhagic detachment of retinal pigment epithelium in 10 eyes,⑤disciform scar in 4 eyes,⑥fibrovascular pigment epithelial detachment and occult CNV in 12 eyes. Conclusion OCT can supply a comprehensive survey of exudative AMD, in making the diagnosis as an important complementary examination of FFA and ICGA. (Chin J Ocul Fundus Dis,2000,16:220-223)

  • Fundus photochromy and optical coherence tomography in epimacular membrane of traumatic eyes following virtreoretinal surgery

    Objective To evaluate the clinical application value of ocular fundus photochromy and optical coherence tomography (OCT) in epimacular membrane of traumatic eyes following vitreorential surgery. Methods Ocular fundus photochromy and OCT were used to observe the morphological changes of macula in 139 consecutive patients with unilateral ocular trauma after vitreoretinal surgery between July 2003 and September 2004. Results At the 1st, 2nd, 3rd, 6th, and 12th month postoperatively, epimacular membranes were found in 5, 8, 17, 19, 20 patients, respectively by using photochromy; and in 7, 10, 20, 26, 27 patients, respectively by using OCT. There was no significant difference in epimacular membrane between photochromy and OCT(chi;2=0.173,Pgt;0.05). The results of OCT showed that epimacular membranes completely adhered to the macular region in 8 patients(29.63%), and retinal neurosensory layer swelled in 12 patients(44.44%)with the fovea thickness of (290.18plusmn;288.05) mu;m. Conclusion Ocular fundus photochromy and OCT are helpful in observing and discovering the epimacular membrane after virtreoretinal surgery of the patients with traumatic retinal detachment during the early and long-term follow-up period; and OCT can provide important information of the structue and position of these epimacular membranes hence to conduct to appropriate treatment. (Chin J Ocul Fundus Dis, 2006, 22: 236-238)

  • Analysis of changes of visual acuity, multifocal electroretinogram and optical coherence tomogram before and after successful idiopathic macular hole surgery

    Objective To observe the changes of visual acuity、multifocal electroretinogram (mfERG) and optical coherence tomogram(OCT)before and after successful idiopathic macular hole (IMH) surgery, and evaluate the efficiency of the IMH surgery. Methods A total of 28 eyes of 28 patients with IMH who underwent vitrectomy during February 2001 and May 2002 in our hospital were collected. visual acuity, mfERG, and OCT were examined preoperatively and 1, 3, 6, and 12 months postoperatively, respectively. The results were analyzed statistically compared with 33 eyes in control group. Results (1) OCT showed that 27 eyes (96.43%) had anatomic closure of the macular hole 1 month after the surgery without recurrence in 12 months. Just 1 eye (3.57%) failed in the closure within 1 year. (2) The visual acuity was much higher in the eyes 1 year after surgery, especially within 6 months postoperatively, than that before the vitrectomy(P<0.01). (3) Before the operation, the mfERG topographies of IMH eyes seem like volcanoes becase the response densities of wave P1 of ring 1 and ring 2 were lower than control group while the other three rings kept in normal. The central peaks of mfERG topographies reappeared little by little during 1 year after the surgery, and the response densities of wave P1 of ring 1 and ring 2 were higher than before, which was still lower than the control group(P<0.05). (4) The thickness of neuroepithelial layer (NEL) of fovea measured by OCT of 27 eyes with anatomic closure was no longer varied and kept lower than control group throughout the follow-up time (P<0.05). (5) One year after the operation, there were positive correlations among visual acuity, mfERG and NEL(P<0.05). Conclusions Vitrectomy is useful for IMH, which could be evaluated by visual acuity mfERG and OCT efficiently. The three factors had consistency. (Chin J Ocul Fundus Dis,2004,20:221-225)

  • Characteristics 0f fundus fluorescein angiography and optical coherence tomography in juvenile retinoschisis

    Objective To observe the characteristics of fundus fluorescein angiography(FFA)and optical coherence tomography(OCT)in juvenile retinoschisis. Methods The photochromes of the ocular fudus of 7 cases(14 eyes)who were diagnosed as juvenile retinoschisis were taken,among whom,5(10 eyes)were examined bv FFA,and 6(12 eyes)bv OCT. Results In 8 eyes with cystiform stellate maculopathy under ophtalmoscope,the result of FFA showed granular fluorescence in different density and shape without exact connection of the configuration between these granules and the cystlike maculopathy.In 2 eyes with pigment disorder in the macula under ophthalmoscope,blocky fluorescence was found in FFA.In 3 eyes with peripheral schisis,FFA discovered distorted and dilated retinal capillaries with different extent,and flecks of non-perfusion area.OCT images revealed thickening of the macular neuroepithelium with laminal separation,and cystic low-reflect areas in the inner layer. Conclusions In juvenile retinoschisis, pigment proliferation and degeneration in the macular area could be found.Granular fluorescence and cystic low—reflect areas could be seen in FFA and OCT,respectively.   • Characteristics and quantitative measurement of idiopathic macular holes with optical coherence tomography

    Objective To investigate the characteristics of optical coherence tomography (OCT) images in idiopathic macular hole. Methods OCT、color photography and fundus fluorescein angiography were performed in 65 cases(70 eyes) of macular holes and which were then graded by connecting to their clinical characteristics. Results Among the 70 eyes the number of 1~4 stages of macular holes were 11,12,36 and 11 eyes respectively.In eyes of stage 1 OCT images showed flattening or disappearing of fovea and minimally reflective space within or beneath the neurosensory retina;stage 2 showed a fullthickness hole with an attached operculum and surrounding edema;stage 3 displayed a full-thickness hole with surrounding edema and stage 4 showed a full-thickness hole and a complete separation of the poterior hyaloid membrane from the retina.The dimeter of the macular holes in stage 2,3 and 4 were (241.75plusmn;107.08),(699.78plusmn;160.99), (631.36plusmn;243.46)mu;m,respectively. Conclusions OCT can display the characteristics of idiopathic macular holes and measure the diameters of holes quantitatively. (Chin J Ocul Fundus Dis, 1999, 15: 205-208)

  • Early influence of laser photocoagulation on macular retinal thickness in diabetic retinopathy

    Objective To investigate the early influences of laser photocoagulation on macular retinal thickness in diabetic retinopathy(DR). Methods Optic coherence tomography examination was performed in 30 eyes with DR(phase Ⅲ~Ⅳ) before, and on the 3rd day and the 7th day after photocoagulation respectively. The thickness of neuroretina and pigment epithelium were measured in the areas of fovea macula and 750 μm from fovea macula. Results Three days after photocoagulation, significant thickening of neuroretina was observed in the fovea macula, which is positively related with age, fasting blood sugar and duration of DR. There was no significant changes in the thickness of pigment epithelium in macula and in the thickness of neuroretina 750 μm from fovea macula. Conclusion Significant thickening of neuroretina in fovea macula in DR early after photocoagulation reveals progressed macular edema induced by photocoagulation which is positively related with age, fasting blood sugar and duration of DR.   • Correlation analysis of macular microvascular structure and intraocular fluidin patients with diabetic macular edema

    ObjectiveTo observe and analyze the correlation between the changes of macular microvascular structure and the level of intracocular fluid cytokines in patients with diabetic macular edema (DME). MethodsA prospective clinical study. From December 2022 to June 2024, 20 patients with 25 eyes of DME diagnosed by Department of Ophthalmology of Linyi People's Hospital were included in the study. Among them, 14 males had 17 eyes and 6 females had 8 eyes. Age was (55.08±10.34) years. Optical coherence tomography (OCT) and OCT angiography (OCTA) were used to scan the macular region at a range of 6 mm×6 mm. Central retinal thickness (CRT), blood flow density of superficial retinal capillary plexus (SCP) and area of fovea avascular zone (FAZ) were measured. The anterior aqueous humor was extracted before the first intravitreal injection of anti-vascular endothelial growth factor (VEGF), the concentrations of interleukin (IL-6), IL-8, VEGF, vascular cell adhesion molecule (VCAM), placental growth factor (PLGF) and monocyte chemotactic protein-1 (MCP-1) were detected. The correlation between macular microvascular structure and aqueous humor cytokines was analyzed by Spearman correlation analysis. ResultsThe CRT of the affected eyes was (617.40±167.64) μm, the SCP flow density was (39.56±1.55)%, and the FAZ area was (0.46±0.13) mm². The concentrations of IL-6, IL-8, VEGF, VCAM, PLGF and MCP-1 in aqueous humor were (301.36±690.52), (29.15±20.56), (71.37±29.32) and (5 621.22±7 241.06), (72.40±13.43), (464.07±163.26) pg/ml, respectively. Correlation analysis showed that there was a significant positive correlation between CRT and the concentrations of aqueous cytokines VEGF and PLGF (r=0.460, 0.462, P<0.05). FAZ area was positively correlated with VEGF and MCP-1 concentrations (r=0.414, 0.465; P<0.05). There was a significant negative correlation between SCP blood flow density and IL-6 (r=0.401, P<0.05). ConclusionThere was a significant correlation between the morphological structure of macular area and the damage degree of microvessels around macular area in DME patients and the concentration of aqueous cytokines.

  • Relationship between the thickness of macula and the changes of multifocal electroretinograms in diabetic retinopathy

    Purpose To investigate the relationship between the changes of the thickness of retina in macula and the abnormalities in multifocal electroretinog rams (mERG) in diabetic retinopathy. Methods mERG and optical coherence tomography (OCT) examination were performed in 38 patients (60 eyes) with DR (phase Ⅲ~Ⅳ). The data were processed with software SPSS and line relation analysis was done. Results The response densities of N1, P1 and N2 in central 5deg; area was significantly negative related to the thickness of neuroretina in macular fovea (correlation efficient -0.252~-0.266,Plt;0.05). The response density of N2 in central 10deg; area was also significantly negative related to the thickness of neuroretina in macular fovea (correlation efficient -0. 332,P=0.01).There was no significant relationship between the latencies of N1 in central 5deg;, 10deg; area and the thickness of macula, whereares the latenc ies of P1 and N2 in central 5deg; were negatively related to the thickness of retinal pigment epithelium in the macular fovea (correlation efficient-0.271~ - 0.322,Plt;0.05). Conclusion The changes of the thickness of neuroretina in macula may affect the local retinal function in macula, which may be revealed by the reduction of response densities in mERG in patients with diabetic retinopathy.   • The changes of optical coherence tomography images of the macula after phacoemul sification in diabetics 

    Objective To investigate the effects of phacoemulsification on macula in diabetics. Methods Thirty eyes of cataract in diabetics were chosen randomly for measurement of the thickness of fovea of retina using OCT before phacoemulsification and 1 month after surgery . The other eyes in these patients and 30 eyes of cataract in nondiabetic pati ents with phacoemulsification were as control. Results In 30 eyes of diabetics with phacoemulsification, the mean fovea thickness were (148.5plusmn;27.7) mu;m preoperatively and (219.4plusmn;68.23) mu;m postoperatively, and the difference was significant (Plt;0.05). In 30 eyes of diabetics without surgery, the mean foveal thickness were (147.4plusmn;27.5) mu;m preoperatively and (148.2plusmn;27.3) mu;m postoperatively and the difference was not significant (Pgt;0.05). In 30 eyes of cataract in nondiabetic patients, the mean fovea thickness were (142.37plusmn;12.7) mu;m preoperatively and (151.9plusmn;23.7) mu;m postoperatively and the difference was not significant (Pgt;0.05). In 30 eyes of diabetic s with phacoemulsification, 11 eyes had new macula edema after surgery and 3 eye s had significant retinal thickening. In 6 eyes with macular edema before surgery, the macular edema were aggravated in 3 eyes after surgery. The macular stru ctural changes were not found in two control groups. Conclusion The thickness of retina is inreased after phacoemulsification in deabetics,and morbidity and its severity of postopevative macular edema are increas ed as well. (Chin J Ocul Fundus Dis, 2001,17:175-177)

  • Pathological changes of branch retinal artery occlusion detected by optical coherence tomography

    Objective To observe the pathological changes of branch retinal artery occlusion (BRAO) by optical coherence tomography (OCT). Methods Twenty-six eyes of 26 patients with BRAO diagnosed in our Center from December 2002 to June 2005 were examined by OCT. The intervals of disease onsets and OCT examinations in all patients were within 2 weeks. The OCT scan modes were horizontal or vertical lines, and the locations of OCT scanning were macular area and the posterior pole of retina. The retinal thicknesses of macular foveola were measured and the macular thicknesses in different obstructive locations were compared using ANOVA analysis. The correlations of visual acuity and retinal thickness were analyzed by Pearson correlation analysis. Results Among the 26 eyes with BRAO, 9 eyes with temporosuperior artery occlusion, 8 eyes with temporoinferior artery occlusion, 7 eyes with arteriole occlusion and 2 eyes with retinal ciliary artery occlusion were observed. The pathological characteristics of OCT images of BRAO were increased retinal thickness and reflectivity in the obstructive locations, and widened dark area of photoreceptors (edema), while there was no obvious foveal edema were observed. The retinal images in other locations were normal. The average macular foveal thickness in the groups of temporosuperior artery occlusion, temporoinferior artery occlusion and retina arteriole occlusion were (161.11plusmn;17.66) mu;m, (148.38plusmn;18.48) mu;m and (136.29plusmn;14.94) mu;m, respectively (F=4.137,P=0.031,Pgt;0.01). There was no correlation of visual acuity with retinal thickness in 24 eyes (r=0.285,P=0.176,Pgt;0.01). Conclusion OCT could display the pathological changes of retinal tissue of BRAO in vivo. The increases of macular foveal thicknesses in BRAO eyes are not so obvious, and no correlations could be seen between visual acuity and macular foveal thickness. OCT is indicated on the old patients and the patients with systemic diseases for whom FFA is contraindicated. The unique characteristics of pathological changes of BRAO indicated by OCT images supply the objective signs for the instant clinical diagnosis. (Chin J Ocul Fundus Dis, 2007, 23: 173-176)
