  • Diagnosis and treatment of ventricular septal defect with infective endocarditis in 40 patients

    ObjectiveTo analyze the diagnosis and treatment of patients with ventricular septal defect complicated with infective endocarditis.MethodsWe retrospectively analyzed the clinical data of 40 patients with ventricular septal defect complicated with infective endocarditis in our hospital from 2001 to 2016. There were 25 males and 15 females, aged 20-62 (39.92±11.16) years. They were divided into two groups according to the duration from admission to surgery: a group A (an early operation group whose surgery was performed within 7 days after admission) and a group B (a conventional treatment group with the duration from admission to surgery>7 days). Among them, there were 27 patients in the group A including 15 males and 12 females with an average age of 39.56±11.80 years, and 13 patients in the group B including 10 males and 3 females with an average age of 40.69±10.13 years. All patients were examined by echocardiogram and blood bacterial culture to investigate their etiology, echocardiogram results and treatment status. And the clinical data of the two groups were compared.ResultsTwo patients died before operation in the group B, one died of heart failure, and one cerebral infarction. No reoperation during hospitalization, cerebral infarction, thromboembolism or other complications occurred. The ventilation time in the group A was significantly shorter than that in the group B (18.00±14.85 h vs. 31.00±29.57 h, P=0.015). There was no statistical difference in the extracorporeal circulation time, myocardial block time, or postoperative hospital stay between the two groups (P>0.05). After discharge, the patients continued antibiotic therapy for 3-6 weeks. Patients were followed up for 12-127 (75.74±6.01) months, 1 died of malignant tumors in the group A, 1 developed atrial fibrillation and 1 developed cardiac insufficiency in the group B, and the rest of patients did not complain of obvious discomfort. There was no residual shunt, recurrence of infective endocarditis, reoperation, postoperative stroke or thromboembolism.ConclusionPreoperative echocardiography and blood bacteriological culture are helpful for the diagnosis and treatment of patients with ventricular septal defect complicated with infective endocarditis. Early surgery is safe and effective for these patients, and can improve the long-term survival rate.

  • Postoperative Care of One Infective Endocarditis Patient with Isolated Kidney on the Right Side

    ObjectiveTo discuss the key nursing points for patients with infective endocarditis and congenital isolated kidney after valve replacement. MethodsIn December 2012, one infective endocarditis patient with isolated kidney underwent heart valve replacement in our hospital. In addition to actively preventing postoperative infection of the heart valve, our nursing focused mainly on the isolated kidney protection and monitoring, and the related complications. ResultsThe surgery was successful, and the isolated kidney was effectively protected. The patient recovered and was discharged from the hospital. ConclusionFor patients with congenital isolated kidney with infective endocarditis, patients' urine output per hour and 24 h discrepancy quantity should be closely observed after valve replacement surgery. It is also very important to intervene early and carry out comprehensive protection of the renal function.

    目的 总结感染性心内膜炎(IE)外科治疗的临床经验。 方法 回顾性分析1998年6月至2005年12月收治的23例IE患者外科手术治疗的临床资料,其中先天性心脏病13例,风湿性心瓣膜病7例,原发性细菌性心内膜炎3例。术前16例行血液细菌培养,阳性7例。术前心功能分级(NYHA)II级2例、III级12例、IV级9例。行主动脉瓣置换术4例、二尖瓣置换术4例、主动脉瓣和二尖瓣双瓣膜置换术3例、主动脉瓣、二尖瓣、三尖瓣置换术2例;单纯心内分流修补术8例,主动脉瓣置换加心内分流修补术2例。术后应用足量的敏感抗生素4~6周。 结果 术后第5d和9d分别死亡2例,其中1例因全身肢端和多器官栓塞、脑出血死亡,1例因心脏骤停死亡。术后20例患者心功能恢复至I~II级,治愈出院;1例心功能III级患者好转出院。术后随访19例,随访时间6个月至75年,18例心功能I~II级,1例心功能III级;发生主动脉瓣瓣周漏2例,其中1例于术后3年自愈,1例随访2.6年,无任何自觉症状,心功能I级。 结论 IE经外科手术治疗能取得较好的治疗效果。

  • Operative Opportunity for Active Infective Endocarditis

    【摘要】 目的 探讨活动期感染性心内膜炎(infectiue endocarditis,IE)患者心脏手术的最佳时期。 方法 回顾分析1999年9月-2009年9月行外科治疗的92例IE患者的临床资料。IE诊断标准为修订的Duke标准。采用SPSS 12.0软件包,分析了年龄、性别、是否是院内感染IE、合并症(糖尿病、慢性阻塞性肿疾病、癌症)、病原菌、手术时间等因素与手术并发症及6个月病死率的关系。 结果 56例患者在确诊为IE后7 d内手术,36例患者在确诊7 d后,并抗生素治疗完成后手术。葡萄球菌为主要感染菌株,与栓塞、脓肿及感染性休克显著相关。最常见的手术指征是重度的瓣膜关闭不全合并心功能不全。6个月的病死率为12%。早期手术与晚期手术比较,病死率增高。单因素分析显示,与6个月病死率相关的因素包括葡萄球菌感染和感染性休克。多因素分析显示感染性休克为6个月内死亡的预测因子。感染性休克的患者尽管行了早期手术,病死率仍为67%。严重瓣膜关闭不全的患者,若未出现心衰,无手术(早期或晚期)死亡。 结论 手术患者的预后由是否发生过感染性休克决定。晚期手术组患者结果好于早期手术组,但结果的差异可能并不是手术的时期不同,而是感染性心内膜炎的严重程度不同造成的。对于有重度瓣膜返流但无心衰的患者,早期手术可能在缩短住院时间,预防心衰发生上有帮助。【Abstract】 Objective To discuss the optimal time of cardiac operations in patients with infective endocarditis (IE). Methods We analyzed the clinical data of 92 patients with IE diagnosed by the modified Duke criteria between September 1999 and September 2009. SPSS 12.0 was used to analyze predictors of 6-month mortality, including age, sex, nosocomial origin of infection, comorbid conditions (diabetes, chromic obstructive pulmonary disease, cancer), the causative microorganisms, the timing of cardiac operation, and the complications. Results Fifty-six patients underwent operation within the first 7 days after diagnosis of infective endocarditis, and 36 received operation at the completion of antibiotic treatment 7 days after the diagnosis. Staphylococci predominated and were significantly associated with embolism, abscess, and septic shock. The most frequent indication for operation was severe regurgitation with heart failure. The 6-month mortality was 12%. Early operation showed an increased mortality compared with late operation. Univariate analysis showed that factors associated with 6-month mortality included staphylococci infection and septic shock. Multivariate analysis revealed that septic shock was a predictor of 6-month mortality. Despite early operation for patients with septic shock, 67% of them died. No death occurred to patients with severe regurgitation but without heart failure after undergoing (early or late) operations. Conclusions The prognosis for surgically treated patients is determined by the occurrence of septic shock. The outcome in patients undergoing late operations is favorable compared with patients undergoing early operations. This difference is probably not due to the timing of the surgical intervention but to the severity of infective endocarditis. In patients with severe regurgitation without heart failure, early operation may offer benefits in shortening the length of hospitalization and preventing development of heart failure.

  • 先天性心脏病继发感染性心内膜炎的外科治疗

    目的 总结先天性心脏病继发感染性心内膜炎41例外科治疗的经验。方法 手术均在体外循环下进行畸形矫正和赘生物清除。室间隔缺损(VSD)直接缝合16例,补片修补10例;动脉导管未闭(PDA)均采用直视缝合法。同期行肺动脉瓣成形术5例,三尖瓣置换术2例,主动脉瓣置换术12例,主动脉瓣和二尖瓣置换术2例。结果 1例急诊VSD修补同时行主动脉瓣置换术,术后并发脑栓塞死亡;其余患者均痊愈出院。随访35例,随访时间3~102个月,无1例死亡和心内膜炎复发。结论 正确把握手术时机和使用有效抗生素是提高治愈率的关键,应强调对先天性心脏畸形的早期治疗,以防止严重并发症。

  • Research progress of mitral valvuloplasty in patients with infective endocarditis

    Infective endocarditis is one of the severest valvar diseases, commonly affecting the mitral valve. Currently, valvuloplasty and replacement are the main surgical options for mitral infective endocarditis. However, the complexity of the infectious lesions has caused a raging debate on which surgical approach offers more benefits. With the development of surgical treatment for endocarditis, mitral valvuloplasty may be a superior solution. It can preserve the integrity of the valve structure, avoiding complications caused by replacement. However, there is a lack of evidence from randomized clinical trials and other evidence-based medical supports. Furthermore, issues regarding the timing of surgery, repair methods, and material choices for mitral valvuloplasty in these patients have not been standardized. Therefore, this article summarizes existing literature to assist clinicians in making appropriate treatment decisions.

  • Network-based questionnaire survey on cardiac surgeons’ understanding of surgical indications and timing for infective endocarditis

    Objective To investigate the knowledge level of Chinese cardiac surgeons regarding the management of infective endocarditis (IE), in order to identify the gap between clinical practices and the latest guidelines, and provide evidence-based support for improving the clinical management of IE. Methods A nationwide survey was conducted through an online questionnaire from December 5, 2024, to December 31, 2024. Descriptive analysis of the survey data was performed. Results A total of 67 valid responses were received from 18 provincial-level administrative divisions across China. While 56.7% (38/67) of respondents demonstrated familiarity with the modified Duke criteria, only 43.3% (29/67) comprehended the 2023 Duke- International Society of Cardiovascular Infectious Diseases criteria. Conversely, 43.3% (29/67) exhibited limited understanding of the former, and 56.7% (38/67) showed deficient knowledge of the latter diagnostic standards. Only 46.3% (31/67) reported proficiency in current IE management guidelines/consensus. Regarding surgical timing, 26.9% (18/67) advocated intervention within 7-14 days of antimicrobial therapy, 22.4% (15/67) during 14-28 days, and 10.5% (7/67) beyond 28 days. Notably, a significant proportion of respondents opted for delayed surgical intervention beyond guideline recommendations when managing patients with heart failure, uncontrolled infection, or neurological complications. Conclusion A knowledge gap and practice discrepancies exist among Chinese surgeons regarding the management of IE. There is an urgent need to promote updated concepts regarding surgical indications and timing for IE in order to optimize treatment strategies and improve patient prognosis.

  • Clinical application of minimally invasive mitral valvuloplasty in patients with infective endocarditis complicated with mitral valve insufficiency

    Objective To investigate the clinical efficacy of minimally invasive mitral valvuloplasty (MVP) in the treatment of infective endocarditis (IE) with mitral regurgitation (MR). Methods A retrospective analysis was conducted on the clinical data of patients who underwent MVP for IE with MR in the Department of Cardiovascular Surgery at Zhongshan Hospital, Fudan University from January 2016 to December 2020. Patients were divided into two groups based on the surgical incision: those with a right mini-thoracotomy were classified as a minimally invasive surgery (MIS) group, and those with a median sternotomy were classified as a median sternotomy (MS) group. All patients had isolated mitral valve involvement. Perioperative data were analyzed, and mid- to long-term outcomes were compared between the two groups. Results A total of 86 patients were included, with 40 in the MIS group (22 males and 18 females, with a mean age of 39±15 years ranging from 8 to 71 years) and 46 in the MS group (27 males and 19 females, with a mean age of 49±16 years ranging from 14 to 71 years). The patients in the MIS group were relatively younger (P=0.004) with better preoperative cardiac function (P=0.004). There was no statistical difference in preoperative fever, gender, or comorbidities between the two groups (P>0.05). The MIS group had shorter postoperative ventilation times, less postoperative 24-hour drainage, less blood transfusion, and shorter total hospital stays compared to the MS group (P=0.001, 0.018, 0.005, 0.005). There was no statistical difference in cardiopulmonary bypass times or ICU stays between the two groups (P>0.05). The perioperative complication rates and mortality rates were not significantly different between the two groups (P>0.05). Follow-up was conducted for 11-92 months, with a mean duration of 49±19 months and an overall follow-up rate of 91.6%. During the follow-up, 3 patients in each group required reoperation for mitral valve issues, with no statistical difference in incidence (7.5% vs. 6.5%, P=0.691). There were no warfarin-related complications, recurrences, or deaths in either group during follow-up. Multivariate regression analysis identified age, preoperative cardiac function, and surgeon experience as influencing factors for the choice of surgical approach. Conclusion Minimally invasive MVP for IE with MR is relatively safe in the perioperative period and shows significant efficacy, with clear mid- to long-term outcomes. It is recommended for younger patients with better preoperative cardiac function and when performed by surgeons with extensive experience in mitral valvuloplasty.
